<italic>Objectives</italic>. To clarify the difference between Lyapunov exponents (LEs) for cleft palate (CP) patients with hypernasality versus without hypernasality and to investigate the relationship between their LEs and nasalance scores (NSs).<italic>Material and Methods</italic>. Six CP patients with severe hypernasality (mean age 9.2 years) and six CP patients without hypernasality (mean age 8.0 years) were enrolled. Five Japanese vowels were recorded at 44.1 KHz, and the NSs were measured simultaneously. The mean first LE (mLE₁) from all one-second intervals was computed.<italic>Results</italic>. The mLE₁for /o/ in patients with hypernasality was significantly higher than that in patients without hypernasality. The correlation coefficients between the mLE₁and NS for all vowels were not statistically different.<italic>Conclusion</italic>. The voice signal of /o/ for the patients with hypernasality was more instable than in those without hypernasality. The chaotic phenomenon was independent of nasal resonance in CP voice.

Identification and characterization of local molecules directing the differentiation of chondrocytes to either transient or permanent cartilage are major issues in cartilage biology. Here, we found CCN family protein 3 (CCN3) was abundantly produced in rat developing epiphyseal cartilage. Evaluations in vitro showed that CCN3 repressed epiphyseal chondrocyte proliferation, while promoting matrix production in multiple assays performed. Furthermore, CCN3 enhanced the articular chondrocytic phenotype; whereas it repressed the one representing endochondral ossification. Additionally, the phenotype of growth plate chondrocytes and chondrogenic progenitors also appeared to be affected by CCN3 in a similar manner. These findings suggest a significant role of CCN3 in inducing chondrocytes to articular ones during joint formation. (C) 2011 Federation of European Biochemical Societies. Published by Elsevier B.V. Comprehensive Evaluation of Three-Dimensional Philtral Morphology 査読

This study was designed to evaluate detailed philtral morphology involving sex and racial differences and to clarify the objective parameters related to subjective judgments regarding the clarity of the philtrum. Fifty-five Japanese and 20 Mexican normal subjects aged 24 to 40 years were recruited, and three-dimensional facial data were acquired using a noncontacting optical surface scanner. The visual classification of the philtral column morphology in Japanese adults using facial photographs revealed that the predominant philtral column shape was different by sex. The three-dimensional anthropometric data showed significant sex differences, which were reflected in the more pronounced visual impression of the philtrum in men. A comparative study between Japanese and Mexican women also revealed certain racial differences. Furthermore, both the philtral depth and central angle of Cupid's bow were extracted as explanatory variables related to the subjective judgments for the clarity of the philtrum. These results suggest that the comprehensive evaluation of philtral morphologic features by three-dimensional analysis is a useful means of identifying the ideal parameters in philtral construction in cleft surgery.

Chondrocytes forming articular cartilage are embedded in a vast amount of extracellular matrix having physical stiffness and elasticity, properties that support the mechanical load from bones and enable the flexible movement of synovial joints. Unlike chondrocytes that conduct the growth of long bones by forming the growth plate, articular chondrocytes show suppressed cell proliferation, unless these cells are exposed to pathological conditions such as mechanical overload. In the present study, we found that one of the members of the CCN family, CCN3, was significantly expressed in chondrocytes isolated from the epiphyseal head in developing rat synovial joints. Evaluation of the effect of recombinant CCN3 on those chondrocytes revealed that CCN3 promoted proteoglycan synthesis, whereas this factor repressed the proliferation of the same cells. These results suggest a critical role for CCN3 in the regulation of the biological properties of articular chondrocytes. © The International CCN Society 2011.

Histologic evaluation of human alveolar sockets treated with an artificial bone substitute material 査読

This study involved a histologic, enzyme histologic, immunohistologic, and three-dimensional microstructure evaluating the extent of osteogenesis and repair in the human alveolar extraction socket achievable with an artificial bone substitute. After tooth extraction in 7 patients, extraction sockets were filled with Mastergraft (15% hydroxyapatite, 85% β-tricalcium phosphate complex). Radiomicrographs and histologic examinations were performed on samples obtained during dental implant placement procedure. On micro-computed tomography, new bone was observed in all collected samples, and osteogenesis was observed to have taken place around the artificial bone substitute. Histologically, active osteogenesis was found throughout the region observed. Addition of new bone around the Mastergraft was observed, and osteoblast-like cells were present. Cells that had partially invaded the artificial bone included tartrate-resistant acid phosphate-positive and CD34-positive cells. These findings indicate that the Mastergraft artificial bone induced osteogenesis in the jawbone and seemed effective for repairing bone defects. Copyright © 2011 by Mutaz B. Habal, MD.

Preliminary study evaluating the accuracy of MRI images on CBCT images in the field of orthodontics 査読

Objective: The purpose of this study was to explore the 3-dimensional (3D) accuracy of magnetic resonance imaging (MRI) on cone-beam computed tomography (CBCT) images after the registration of MRI images on CBCT images. Materials and Methods: Three Japanese adult females volunteered for this study. To transform digital imaging and communication in medicine (DICOM) data derived from MRI and CBCT images into polygon data, five software programs were used. CBCT and MRI images were obtained within one week, and both were registered by the iterative closest point (ICP) method. To assess the accuracy of the composite MRI-CBCT, the measurement errors of the MRI-CBCT were verified. Measurement values were compared using frontal and cephalometric soft-tissue landmarks. Differences were analyzed using the non-parametric Mann-Whitney U test. Results: There were no significant linear measurement errors (P 0.05) when the images were measured from the superimposed MRI-CBCT images. Conclusion: The MRI images attained from MRI-CBCT registration showed accurate 3D linear measurements.

A new modification for safer submental orotracheal intubation 査読

In patients in whom a tracheal tube cannot be inserted through the nostrils due to multiple facial trauma or hypoplasty of the nose, submental orotracheal intubation (SOI) is performed to avoid tracheostomy. We report a new modification for SOI to minimize the risk of apnea. A 20-year-old man was scheduled for sagittal split ramus osteotomy. As the patient had severe hypoplasia of the nose, SOI was planned. Following orotracheal intubation with a spiral tube (first tube), a submental tunnel was surgically created. A second tube that had been confirmed, in advance, to snugly fit into the proximal end of the first tube was passed into the submental tunnel via a polypropylene cylinder and connected between the first tube and the breathing circuit. After careful withdrawal of the second tube through the submental tunnel, the first tube was directly connected to the breathing circuit after removal of the second tube. Although this technique requires additional time, apnea time is minimal even in patients in whom withdrawal of the tracheal tube through the submental tunnel takes time, because the second tube forms a link between the first tube and the breathing circuit, making it possible to ventilate the patient throughout the procedure. © Japanese Society of Anesthesiologists 2011.

Atypical Lipomatous Tumor of the Tongue: Report of a Case 査読

The term atypical lipomatous tumor (ALT) is synonymous with well-differentiated liposarcoma (WDL). This tumor occurs very rarely in the tongue. Thus, it is difficult to predict its prognosis. Although recurrence of ALT/WDL is thought to be unlikely after complete excision, long-term follow-up is necessary when considering the pathologic conditions of this tumor at other sites. Here, we report a case of an ALT of the tongue, with a review of the literature. A 68-year-old man was referred to our hospital because of a tumor on the left side of his tongue. Upon palpation, the tumor was 12mm in diameter, circumscribed, elastic and hard, well demarcated, movable, and painless. We diagnosed the lesion as a lipoma and extirpated the tumor under local anesthesia. Because the specimen was histopathologically diagnosed as an ALT, as a precaution, we excised an additional 5mm from the area surrounding the original tumor under general anesthesia. Three years after the operation, the tongue demonstrated good healing without paresthesia or dysfunction, and to date there has been no evidence of recurrence.

Herein, we report a very rare case of pilomatricoma in the perioral region. A 27-year old Japanese female was referred to our clinic with a subcutaneous tumor in the angle of the left side of the mouth. The tumor was observed as a subcutaneous well-circumscribed and bony hard mass, without any changes of the skin surface, while a soft radiographic examination revealed a weak calcifying rounded mass. The tumor was extirpated using an intraoral approach, because of patient request. During the operation, the tumor was found to be partly attached to the surface skin, though easily removed from the surrounding tissue. The tumor was a rounded calcifying mass, 13. mm in diameter, and histologically diagnosed as a pilomatricoma. © 2010 Asian Association of Oral and Maxillofacial Surgeons.

症例は45歳女性で、1年前より左側上顎臼歯部に時折違和感を自覚していたが放置した。歯科治療で撮影されたパノラマX線で左側上顎洞内のX線不透過像を指摘された。顔貌は左右対称で、左側上顎眼窩下部に圧痛等の異常所見は認めなかった。左側上顎歯肉部に膨隆や腫脹、発赤等の異常所見を認めなかった。左上6、左上7は電気歯髄診断で生活反応を認めなかった。パノラマX線では左上678相当部の上顎洞底より左側上顎洞内に突出した類円形の塊状のX線不透過像が認められた。CTでは左側上顎洞内に上顎洞底から外向性に突出し上顎洞前壁、側壁、後壁と接する病変を認め、病変の内部のdensityは不均一であった。また、病変内部に歯を認めた。左側上顎骨腫瘍と診断し全身麻酔下に摘出術を施行した。病理組織学的所見は病変の内部に埋伏歯を認め、埋伏歯を取り囲むように塊状の不規則な形態の硬組織が認められた。硬組織はセメント質様、或いは骨様で、線維組織は細胞成分が豊富であった。術後経過は良好で術後6ヵ月現在、再発を認めていない。
