Role of oral sensory and serotonergic neurons in dopamine-induced tongue movement in rat

Background: Oral dyskinesia is a condition in which oral organs exhibit involuntary movements. The aim of this study was to determine the contribution of oral sensation to the control of oral dyskinesia using an animal model that exhibits vacuous tongue movements after systemic administration of dopamine (DA) agonists. Methods: Adult male Sprague-Dawley rats were used in all experiments. Tongue movement was elicited in a freely moving rat model by systemic administration of a mixture of the D1 receptor agonist SKF38393 and the D2 receptor agonist quinpirole. The number of tongue protrusions was calculated from electrical myographic data obtained from the genioglossal muscle. Extracellular concentrations of DA and serotonin (5-HT) in the nucleus accumbens were determined using in vivo microdialysis. To investigate the effects of oral sensation on vacuous tongue movement, deafferentation of the palatal mucosa was performed, and the above experiments were repeated. Results: The hyperactivity of tongue protrusion induced by administration of SKF38393 and quinpirole was significantly inhibited by palatal deafferentation. Palatal deafferentation also significantly reduced the increase in extracellular 5-HT concentration in the nucleus accumbens produced by administration of DA agonists. Conclusions: These results support the important contribution of oral sensation to vacuous tongue protrusion in a rat model of oral dyskinesia.

Some forms of Sjögren's syndrome (SS) follow a clinical course accompanied by systemic symptoms caused by lymphocyte infiltration and proliferation in the liver, kidneys, and other organs. To better understand the clinical outcomes of SS, here we used minor salivary gland tissues from patients and examine their molecular, biological, and pathological characteristics. A retrospective study was performed, combining clinical data and formalin-fixed paraffin-embedded (FFPE) samples from female patients over 60 years of age who underwent biopsies at Okayama University Hospital. We employed direct digital RNA counting with nCounter® and multiplex immunofluorescence analysis with a PhenoCycler™ on the labial gland biopsies. We compared FFPE samples from SS patients who presented with other connective tissue diseases (secondary SS) with those from stable SS patients with symptoms restricted to the exocrine glands (primary SS). Secondary SS tissues showed enhanced epithelial damage and lymphocytic infiltration accompanied by elevated expression of autophagy marker genes in the immune cells of the labial glands. The close intercellular distance between helper T cells and B cells positive for autophagy-associated molecules suggests accelerated autophagy in these lymphocytes and potential B cell activation by helper T cells. These findings indicate that examination of FFPE samples from labial gland biopsies can be an effective tool for evaluating molecular histological differences between secondary and primary SS through multiplexed analysis of gene expression and tissue imaging.

Displacement of dental implants into the maxillary sinus is one of the common dental complications. However, it is rare that dental implants cause the displacement of multiple components due to separation. Here, we describe an unusual case of a 43-year-old man who had a missing implant in the maxillary sinus after an implant procedure. There was a two-week waiting period before we performed the removal during which the cap had unexpectedly separated from the body and freely moved into the ostium by sinus activity. The body was independently extracted intraorally. The remaining cap was secondly removed by utilizing nasal endoscopy. There were no complications in the postoperative period and the patient reported no symptoms of sinusitis after two months of follow-up. We experience unexpected events in the course of treating a displaced implant into the maxillary sinus. Our case report may provide several learning points for the removal of a missing implant.

Double-faced CX3CL1 enhances lymphangiogenesis-dependent metastasis in an aggressive subclone of oral squamous cell carcinoma. 国際誌

Because cancer cells have a genetically unstable nature, they give rise to genetically different variant subclones inside a single tumor. Understanding cancer heterogeneity and subclone characteristics is crucial for developing more efficacious therapies. Oral squamous cell carcinoma (OSCC) is characterized by high heterogeneity and plasticity. On the other hand, CX3C motif ligand 1 (CX3CL1) is a double-faced chemokine with anti- and pro-tumor functions. Our study reported that CX3CL1 functioned differently in tumors with different cancer phenotypes, both in vivo and in vitro. Mouse OSCC 1 (MOC1) and MOC2 cells responded similarly to CX3CL1 in vitro. However, in vivo, CX3CL1 increased keratinization in indolent MOC1 cancer, while CX3CL1 promoted cervical lymphatic metastasis in aggressive MOC2 cancer. These outcomes were due to double-faced CX3CL1 effects on different immune microenvironments indolent and aggressive cancer created. Furthermore, we established that CX3CL1 promoted cancer metastasis via the lymphatic pathway by stimulating lymphangiogenesis and transendothelial migration of lymph-circulating tumor cells. CX3CL1 enrichment in lymphatic metastasis tissues was observed in aggressive murine and human cell lines. OSCC patient samples with CX3CL1 enrichment exhibited a strong correlation with lower overall survival rates and higher recurrence and distant metastasis rates. In conclusion, CX3CL1 is a pivotal factor that stimulates the metastasis of aggressive cancer subclones within the heterogeneous tumors to metastasize, and our study demonstrates the prognostic value of CX3CL1 enrichment in long-term monitoring in OSCC.

当科で治療を行った放射線性顎骨壊死(ORN)について、発症原因となった歯科疾患を中心に検討し、周術期口腔管理の有無による臨床像の違いも評価した。2013～2021年にORNと診断された18例(男性12例、女性6例)を対象とした。口腔管理群8例では感染源が歯周炎に関わる症例はなく、智歯周囲炎3例、咬合性外傷4例、感染源不明1例であった。非管理群10例では辺緑性歯周炎2例、根尖性歯周炎が3例、智歯周囲炎2例、義歯性潰瘍1例、感染源不明2例であった。保存療法のみ行ったのは8例であり、保存療法を行ったものの悪化を認めたため高気圧酸素療法を行ったのは3例、最終的に外科治療を行ったのは7例であった。ORNの予後や各治療方法の効果においては管理群と非管理群で明らかな違いはなかった。

53歳女性。近在歯科を受診した際に口底部の腫脹を指摘され、当院へ紹介となった。口腔内所見では口底正中部粘膜下に25×25mm大の可動性を有する無痛性で弾性軟の類球形の腫瘤が認められた。また、造影CTでは口底正中からオトガイ舌骨筋上にかけて内部不均一な低吸収域の24×23×22mmの腫瘤が認められた。MR画像所見も踏まえて、本症例は脂肪腫を疑い、局所麻酔下に生検を行ったところ、病理組織学的に舌下型類皮嚢胞(DC)と診断された。DCは開窓術により縮小し、DCの残遺病変を摘出することで治癒することができた。

Expression and function of CCN2-derived circRNAs in chondrocytes

Abstract

Cellular communication network factor 2 (CCN2) molecules promote endochondral ossification and articular cartilage regeneration, and circular RNAs (circRNAs), which arise from various genes and regulate gene expression by adsorbing miRNAs, are known to be synthesized from CCN2 in human vascular endothelial cells and other types of cells. However, in chondrocytes, not only the function but also the presence of CCN2-derived circRNA remains completely unknown. In the present study, we investigated the expression and function of CCN2-derived circRNAs in chondrocytes. Amplicons smaller than those from known CCN2-derived circRNAs were observed using RT-PCR analysis that could specifically amplify CCN2-derived circRNAs in human chondrocytic HCS-2/8 cells. The nucleotide sequences of the PCR products indicated novel circRNAs in the HCS-2/8 cells that were different from known CCN2-derived circRNAs. Moreover, the expression of several Ccn2-derived circRNAs in murine chondroblastic ATDC5 cells was confirmed and observed to change alongside chondrocytic differentiation. Next, one of these circRNAs was knocked down in HCS-2/8 cells to investigate the function of the human CCN2-derived circRNA. As a result, CCN2-derived circRNA knockdown significantly reduced the expression of aggrecan mRNA and proteoglycan synthesis. Our data suggest that CCN2-derived circRNAs are expressed in chondrocytes and play a role in chondrogenic differentiation.

今回,上顎4前歯部の歯槽骨切り術を施術した上顎前突症例について報告する.患者は初診時年齢15歳3ヵ月の男子で,上顎前歯の突出を主訴として来院した.12歳時に上顎右側中切歯の打撲の既往があった.側面観は凸型で,口唇の突出および頤筋の緊張感を認めた.∠SNAは81.9°,∠SNBは73.5°,∠ANBは8.4°で骨格性II級を呈していた.上顎前歯は著しく唇側傾斜し,オーバージェットは14.0mm,オーバーバイトは4.0mmで,大臼歯関係および犬歯関係は両側ともにアングルII級を呈していた.上顎右側第一小臼歯と下顎右側第二小臼歯は交叉咬合であった.上顎両側第一小臼歯の片顎抜去による治療方針とし治療を開始したが,動的治療開始10ヵ月後の犬歯牽引中に上顎右側中切歯の骨性癒着が疑われた.治療計画を矯正歯科治療単独から,犬歯牽引後に上顎4前歯を上顎前歯部歯槽骨切り術にて後方移動する方針に変更した.術前矯正治療を1年11ヵ月行った後に上顎前歯部歯槽骨切り術を施術した.術後矯正治療1年5ヵ月後に動的治療を終了し保定に移行した.∠SNAは80.8°,∠SNBは73.7°,∠ANBは7.1°になり,上顎前歯は標準的傾斜となり,オーバージェットは2.6mm,オーバーバイトは3.5mmに改善された.上顎4前歯部の歯槽骨切り術により上顎前歯部は後退し,凸型の側貌と口唇閉鎖不全は改善し良好な側貌が得られた.保定開始後2年3ヵ月経過時においても,良好な顔貌と咬合は維持されている.(著者抄録)

Appropriate operations in severe anterior open bite (AOB) cases are extremely complicated to perform because of the multiple surgical procedures involved, the difficulty of predicting posttreatment aesthetics, and the high relapse rate. We herein report a 16-year-old girl with skeletal Class II, severe AOB malocclusion, and crowding with short roots, and aesthetic and functional problems. Four-piece segmental Le Fort I osteotomy with horseshoe osteotomy was performed for maxillary intrusion, and bilateral sagittal split ramus osteotomy (SSRO) and genioplasty were performed for mandibular advancement. The malocclusion and skeletal deformity were significantly improved by the surgical orthodontic treatment. Functional and aesthetic occlusion with an improved facial profile was established, and no further root shortening was observed. Acceptable occlusion and dentition were maintained after a two-year retention period. This strategy of surgical orthodontic treatment with a complicated operative procedure might be effective for correcting certain severe AOB malocclusion cases.

Medication-related osteonecrosis of the jaw (MRONJ) is a side effect in patients taking bone-modifying agents (BMAs), which are highly beneficial for treating osteoporosis and cancer. Bisphosphonates are prescribed to treat secondary osteoporosis in patients with rheumatoid arthritis (RA). We recently encountered two unusual cases of intraoral ONJ in RA patients who had not been treated with a BMA and did not have features of methotrexate-associated lymphoproliferative disorder. Their ONJ stage II bone exposures were treated by conservative therapy, providing good prognoses. These cases indicate that ONJ can occur in RA patients not treated with bisphosphonates. Several risk factors are discussed.

QUAD shotによる緩和的放射線治療を適用した終末期口腔癌の1例

79歳男性。既往として40年前に右下顎歯肉癌に対し手術と放射線治療が行われていた。今回、右下顎部の疼痛ならびに開口障害を主訴に、近在の歯科医院より精査目的で当科へ紹介となった。画像所見、生検結果を踏まえて、右下顎歯肉癌の再発と診断され、患者の全身状態、希望よりQUAD shotによる緩和的放射線治療が行われた。その結果、QUAD shotとの併用放射線増感剤にPTXを選択し、1コース終了時より腫瘍は縮小し、疼痛の改善傾向がみられた。更に3コース終了後にはオピオイドを中止し、NSAIDsのみで日常生活を過ごせるようになり、摂食機能の改善で食事量の増加から体重減少や低栄養が改善された。最終的には患者はQOLを維持したまま最期を迎えることができた。

一社・日本口蓋裂学会では,学術調査委員会の2020年度の事業として,2019年1年間の会員施設における新規患者について臨床的検討を行った。49施設より965例の登録があり,以下の結果を得た。1)症型別では辰裂110例(11.4%),唇顎裂206例(21.3%),唇顎口蓋裂388例(40.2%),口蓋裂193例(20.0%),粘膜下口蓋裂52例(5.4%),その他16例(1.7%)であった。2)罹患側では左側368例(52.2%),右側183例(26.0%),両側性160例(22.7%)。両側性は唇裂で少なく,唇顎口蓋裂で他の症型に比べて多かった。3)性別では女性432例(44.8%),男性533例(55.2%)。多くの症型で男性が多い中,口蓋裂では若干であるが,女性の方が多かった。4)出生時体重では,粘膜下口蓋裂で他の症型よりも低体重であった。5)出生前診断では,外表に変化のある口唇裂を含む症型で50%程度であった。6)両親の年齢では,全体に父親が母親よりも若干高年齢であったが症型間に差はなかった。7)血族内での出生では,いずれの症型も6～8%をしめていた。8)併発疾患および9)症候群,染色体異常については,口蓋裂および粘膜下口蓋裂児において頻度が高かった。併発疾患については,1例で複数の併発がみられる場合が多かった。なお,症型間で統計処理(カイ二乗検定)を行い,性差,罹患側,生下時体重,併発疾患,症候群・染色体異常において有意差(p<0.05)を認めた。以上,1年間に出生した日本人の口唇裂・口蓋裂児の多数例の検討から有意義なデータが得られた。(著者抄録)

BACKGROUND: We investigated and compared the osteogenic potential and bone regeneration capacities of dedifferentiated fat cells (DFAT cells) and adipose-derived stem cells (ASCs). METHOD: We isolated DFAT cells and ASCs from GFP mice. DFAT cells were established by a new culture method using a mesh culture instead of a ceiling culture. The isolated DFAT cells and ASCs were incubated in osteogenic medium, then alizarin red staining, alkaline phosphatase (ALP) assays, and RT-PCR (for RUNX2, osteopontin, DLX5, osterix, and osteocalcin) were performed to evaluate the osteoblastic differentiation ability of both cell types in vitro. In vivo, the DFAT cells and ASCs were incubated in osteogenic medium for four weeks and seeded on collagen composite scaffolds, then implanted subcutaneously into the backs of mice. We then performed hematoxylin and eosin staining and immunostaining for GFP and osteocalcin. RESULTS: The alizarin red-stained areas in DFAT cells showed weak calcification ability at two weeks, but high calcification ability at three weeks, similar to ASCs. The ALP levels of ASCs increased earlier than in DFAT cells and showed a significant difference (p < 0.05) at 6 and 9 days. The ALP levels of DFATs were higher than those of ASCs after 12 days. The expression levels of osteoblast marker genes (osterix and osteocalcin) of DFAT cells and ASCs were higher after osteogenic differentiation culture. CONCLUSION: DFAT cells are easily isolated from a small amount of adipose tissue and are readily expanded with high purity; thus, DFAT cells are applicable to many tissue-engineering strategies and cell-based therapies.

Camouflage Treatment for Skeletal Maxillary Protrusion and Lateral Deviation with Classic-Type Ehlers-Danlos Syndrome.

We herein report the case of a 19-year-old female with a transverse discrepancy, skeletal Class II malocclusion, severe crowding with concerns of classic-type Ehlers-Danlos syndrome (EDS), aesthetics problems and functional problems. The main characteristics of classic EDS are loose-jointedness and fragile, easily bruised skin that heals with peculiar "cigarette-paper" scars. The anteroposterior and transverse skeletal discrepancies can generally be resolved by maxilla repositioning and mandibular advancement surgery following pre-surgical orthodontic treatment. However, this patient was treated with orthodontic camouflage but not orthognathic surgery because of the risks of skin bruising, poor healing and a temporomandibular disorder. A satisfactory dental appearance and occlusion were achieved after camouflage treatment with orthodontic anchor screws and the use of Class II elastics, including the preservation of the stomatognathic functions. Acceptable occlusion and dentition were maintained after a two-year retention period. This treatment strategy of orthodontic camouflage using temporary anchorage, such as anchor screws and Class II elastics, may be a viable treatment option for skeletal malocclusion patients with EDS.

OBJECTIVES: Anti-osteoclastic treatments for breast cancer occasionally cause medication-related osteonecrosis of the jaw. Moreover, elevated glycolytic activity, which is known as the Warburg effect, is usually observed in these breast cancer cells. Previously, we found that cellular communication network factor 2 (CCN2) production and glycolysis enhanced each other in chondrocytes. Here, we evaluated the interplay between CCN2 and glycolysis in breast cancer cells, as we suspected a possible involvement of CCN2 in the Warburg effect in highly invasive breast cancer cells. METHODS: Two human breast cancer cell lines with a distinct phenotype were used. Glycolysis was inhibited by using 2 distinct compounds, and gene silencing was performed using siRNA. Glycolysis and the expression of relevant genes were monitored via colorimetric assays and quantitative RT-PCR, respectively. RESULTS: Although CCN2 expression was almost completely silenced when treating invasive breast cancer cells with a siRNA cocktail against CCN2, glycolytic activity was not affected. Notably, the expression of glycolytic enzyme genes, which was repressed by CCN2 deficiency in chondrocytes, tended to increase upon CCN2 silencing in breast cancer cells. Inhibition of glycolysis, which resulted in the repression of CCN2 expression in chondrocytic cells, did not alter or strongly enhanced CCN2 expression in the invasive and non-invasive breast cancer cells, respectively. CONCLUSIONS: High CCN2 expression levels play a critical role in the invasion and metastasis of breast cancer. Thus, a collapse in the intrinsic repressive machinery of CCN2 due to glycolysis may induce the acquisition of an invasive phenotype in breast cancer cells.

症例は3ヵ月男児で、左側唇口蓋裂を認め、出生後早期の染色体G-banding検査にて46,XY,t(11;22)(q23.3;q11.2)と診断された。口蓋裂一貫治療目的に当科を紹介受診した。初診時にホッツ床作製のための印象採得を行い、生後3ヵ月からホッツ床による顎誘導ならびに哺乳の改善を行った。生後5ヵ月目に全身麻酔下にて口唇形成術を施行し、口蓋裂に対しては2段階による口蓋形成術を行うものとし、初回は1歳3ヵ月時に軟口蓋形成術(Furlow法)を行い、1歳11ヵ月で2回目となる硬口蓋閉鎖術を施行した。術後経過は良好であり、2歳2ヵ月から言語聴覚士による発達検査、言語治療を開始した。鼻咽腔閉鎖機能不全や構音障害はみられず、定期的な経過観察を行っている。現在5歳8ヵ月で反対咬合を認めている。

Epidemiological Correlations Between Head and Neck Cancer and Hepatitis B Core Antibody Positivity. 国際誌

BACKGROUND/AIM: Hepatitis B core (HBc) antibody positivity indicates a history of hepatitis B virus (HBV) infection and latent infection. PATIENTS AND METHODS: We conducted a retrospective case-control study of 512 and 495 head and neck cancer (HNC) and non-HNC patients treated at the Okayama University Hospital, Head and Neck Cancer Center from 2008-2017. Demographic data and risk factors that might affect HNC diagnosis were analyzed to assess their effects. RESULTS: Cancer diagnosis was found to correlate with HBc antibody positivity [odds ratio (OR)=1.50, 95% confidence interval (CI)=1.09-2.08], smoking (OR=3.03, 95%CI=2.16-4.25), and a previous history of cancer (OR=4.12, 95%CI=2.79-6.09). The HBs antigen positivity rate in both groups was very close to that observed in the general Japanese population. The HBc antibody positivity rate was very high only in the HNC group. CONCLUSION: HBc antibody positivity and HNC are epidemiologically correlated.

DESIGN: Case report Interventions: Correction involved anterior distraction of the segmented maxilla. RESULTS: The present case demonstrates that elongation of the maxilla with anterior distraction is an effective way to develop a proper dental arch, correct anterior and posterior crowding, and improve a midface deficiency.

Abstract

Intermittent parathyroid hormone (PTH) administration is known to promote bone healing after surgical procedures. However, the mechanism and influence of PTH on the mineral and collagen quality of the jaw are not well understood. Most studies have focused on analyzing the bone density and microstructure of the mandible, and have insufficiently investigated its mineral and collagen quality. Oxidative stress activates osteoclasts, produces advanced glycation end products, and worsens mineral and collagen quality. We hypothesized that PTH induces oxidation and affects the mineral and collagen quality of newly formed mandibular bone. To test this, we examined the mineral and collagen quality of newly formed mandibular bone in rats administered PTH, and analyzed serum after intermittent PTH administration to examine the degree of oxidation. PTH administration reduced mineralization and worsened mineral and collagen quality in newly formed bone. In addition, total anti-oxidant capacity in serum was significantly decreased and the oxidative-INDEX was increased among PTH-treated compared to vehicle-treated rats, indicating serum oxidation. In conclusion, intermittent administration of PTH reduced mineral and collagen quality in newly formed mandibular bone. This effect may have been induced by oxidation.

二卵性双生児で、一人は粘膜下口蓋裂、一人は先天性鼻咽腔閉鎖不全症を伴った22q11.2欠失症候群の症例を報告した。二卵性双生児の兄:5歳時に鼻咽腔閉鎖不全を指摘され、当院耳鼻咽喉科に紹介となった。特異的顔貌を認めたため、染色体検査を行い22q11.2欠失症候群と診断された。粘膜下口蓋裂に対して口蓋形成術(Furlow法)を施行し、その後は定期的に言語訓練を行うこととなった。二卵性双生児の弟:5歳時に鼻咽腔閉鎖不全を指摘され、兄とともに当院耳鼻咽喉科に紹介となった。軟口蓋挙上装置を装着し、言語訓練を行った。一旦来院が途絶えていたが再来院され、言語訓練の重要性を十分に説明した。その後は定期的に言語訓練を施行している。

Impacted primary teeth can be caused by odontogenic tumors such as odontomas, cystic diseases such as dentigerous and eruption cysts, and fibrous hyperplasia of the gingiva, while elimination of those causes reportedly results in normal eruption of primary teeth. We describe the course of a child from the age of 1 year until 4 years 10 months who had primary tooth eruption failure and was treated with dental methods. Although primary teeth expected to spontaneously erupt were followed, there were several on the left side that remained unerupted. We performed a left maxillary and mandibular gingivectomy to closely examine the cause of eruption delay and tooth germ dislocation, as well as attempt to induce eruption of the primary teeth. Based on histopathological results, the diagnosis was fibroma. Surgical procedures did not result in clear improvement of eruption failure. In order to improve masticatory function and aesthetics, the child was fixed with an artificial denture for all primary teeth not expected to erupt. (C) 2018 Japanese Society of Pediatric Dentistry. Efficacy of Maxillary Anterior Segmental Distraction Osteogenesis in Patients With Cleft Lip and Palate. 査読国際誌

OBJECTIVES: To evaluate the effects of maxillary anterior segmental distraction osteogenesis (MASDO) in patients with cleft lip and palate (CLP) and to identify risk factors for increased relapse. DESIGN: A retrospective study. PATIENTS: Thirty-one Japanese patients with CLP who underwent MASDO were eligible for study inclusion. MAIN OUTCOME MEASURES: We evaluated lateral cephalograms obtained before (T1), at 3 months (T2), and at 1 year (T3) after MASDO, and measured changes from T1 to T2 (δT1T2), from T2 to T3 (δT2T3), and from T1 to T3 (δT1T3). We also evaluated the risk factors associated with an increased relapse. RESULTS: Overall (δT1T3), MASDO improved retrusion of the maxilla. We measured a significant advancement (6.1 mm) of the anterior maxillary segment in δT1T2 (A-McNamara classification) and increases in the overjet and the SNA, ANB, and nasolabial angles. However, skeletal relapse was evident in δT2T3, and the median percentage of relapse was 10%. To explore the risk factors, we subdivided patients with a δT1T2 of >5 mm into 2 groups based on the percentage of relapse (>15% vs ≤15%). There were significant differences between these groups in the vertical positions of the anterior nasal spine and point A, and the angle formed by the SN and palatal planes (SNPP), suggestive of intraoperative counterclockwise rotation of the maxilla. CONCLUSIONS: MASDO is effective for correcting midfacial deficiencies, but counterclockwise rotation of the maxilla during surgery may cause relapse.

Odontomas are generally encountered in permanent dentition and rarely seen in primary dentition. These lesions are regarded as essentially benign, though they often cause disturbances in eruption of the associated tooth. We present clinical findings and management of 3 cases of odontomas that occurred in primary dentition, including a 3-year-old girl referred for examination of an unerupted left upper primary canine, a 6-year-old boy with an unerupted left lower second primary molar, and a 5-year-old girl who was presented with an unerupted right upper second primary molar. In all cases, periapical and panoramic radiography revealed a radio-opaque mass surrounded by a narrow radiolucent space located above the crown of the unerupted tooth. Each was diagnosed as a compound odontoma interfering with eruption of a primary tooth and surgical removal was performed under general anesthesia for all patients, with the final diagnosis based on histological findings. Following surgery, the primary canine in Case 1 spontaneously erupted, while a space maintenance appliance was applied in Case 2 and 3. It is important to recognize that an odontoma can appear in primary dentition, though the incidence is quite low. Early detection and removal of an odontoma associated with an impacted primary tooth is necessary for optimal dentition. The CCN family consists of 6 genes in the mammalian genome and produces multifunctional proteins involved in a variety of biological processes. Recent reports indicate the profound roles of CCN2 in energy metabolism in chondrocytes, and Ccn2 deficiency is known to alter the expression of 2 other family members including Ccn3. However, almost nothing is known concerning the regulation of the CCN family genes by energy metabolism. In order to gain insight into this critical issue, we initially and comprehensively evaluated the effect of inhibition of glycolysis on the expression of all of the CCN family genes in chondrocytic cells. Upon the inhibition of a glycolytic enzyme, repression of CCN2 expression was observed, whereas CCN3 expression was conversely induced. Similar repression of CCN2 was conferred by the inhibition of aerobic ATP production, which, however, did not induce CCN3 expression. In contrast, glucose starvation significantly enhanced the expression of CCN3 in those cells. The results of a reporter gene assay using a molecular construct containing a CCN3 proximal promoter revealed a dose-dependent induction of the CCN3 promoter activity by the glycolytic inhibitor in chondrocytic cells. These results unveiled a critical role of glycolytic activity in the regulation of CCN2 and CCN3, which activity mediated the mutual regulation of these 2 major CCN family members in chondrocytes.

症例は21歳女性で、13年前に左側頬部の腫瘤を自覚したが確定診断には至らなかった。左側頬部に鈍痛を覚えるようになり、腫瘤の増大を認めた。左側頬粘膜は表面正常で、触診にて咬筋相当部に可動性で軽度の圧痛を伴う30mm大の骨様硬の病変を触知した。単純CT画像では、左側狡筋は右側と比較し腫大し、内部に長径22mm大、外形不整な高吸収域を認めた。病変内部に長径3〜6mm大の複数の類円形石灰物を認めた。造影CT画像では、高吸収域に不均一な造影効果を認め、造影性の高い領域では血管腔と同程度の高い造影性を示した。腫瘤内石灰化物の多くは内部に核を有し、層板状構造に近い性状であった。MRI所見では、病変内部はT1強調画像では低信号から筋と同程度の不均一な信号強度、T2強調画像では低信号から高信号の不均一な信号強度を呈し、石灰化物に相当する部分はほぼ無信号を呈した。全身麻酔下で口腔内より腫瘤の摘出術を施行した。現在、術後3年経過しているが、再発はなく経過良好である。摘出標本の病理組織学的診断は静脈石を伴った静脈奇形であった。

Platinum-based chemotherapy plus cetuximab represents the first-line treatment for recurrent or metastatic squamous cell carcinoma of the head and neck. The most common adverse events associated with cetuximab are infusion reactions and skin reactions, and a risk of venous thromboembolic events has also recently been reported in association with cetuximab. It is well known that thrombosis is a common complication of malignancy, and represents the second most frequent cause of mortality in cancer patients. The present study reports the case of a 79-year-old man who presented with lung and liver metastases from tongue squamous cell carcinoma, for which platinum-based chemotherapy plus cetuximab was administered. After 1 cycle, the patient showed rapid growth of a left ventricular (LV) thrombus, despite ongoing antiplatelet therapy for an old myocardial infarction. Anticoagulant therapy was administered to treat the LV thrombus, which resolved within 1 week. To the best of our knowledge, this represents the first reported case of rapidly occurring LV thrombus associated with platinum-based chemotherapy plus cetuximab. Platinum-based chemotherapy plus cetuximab may be associated with a higher risk of embolic thrombus.

This report describes a case of successful orthodontic treatment using maxillary anterior segmental distraction osteogenesis with an internal maxillary distractor and bilateral sagittal split ramus osteotomy in a girl with cleft lip and palate. A 16-year-old girl with unilateral cleft lip and palate exhibited midface retrusion because of growth inhibition of the maxillary complex and mandibular excess. After the presurgical orthodontic treatment, 6.0-mm advancement of the maxillary anterior segment and 4.0-mm set back of the mandible were performed. After a retention period, the patient's midface convexity was greatly improved and the velopharyngeal competence was preserved without relapse.

Odontogenic tumors and cysts, arising in the jawbones, grow by resorption and destruction of the jawbones. However, mechanisms underlying bone resorption by odontogenic tumors/cysts remain unclear. Odontogenic tumors/cysts comprise odontogenic epithelial cells and stromal fibroblasts, which originate from the developing tooth germ. It has been demonstrated that odontogenic epithelial cells of the developing tooth germ induce osteoclastogenesis to prevent the tooth germ from invading the developing bone to maintain its structure in developing bones. Thus, we hypothesized that odontogenic epithelial cells of odontogenic tumors/cysts induce osteoclast formation, which plays potential roles in tumor/cyst outgrowth into the jawbone. The purpose of this study was to examine osteoclastogenesis by cytokines, focusing on transforming growth factor-beta (TGF-beta), produced by odontogenic epithelial cells. We observed two pathways for receptor activator of NF-kappa B ligand (RANKL) induction by keratocystic odontogenic tumor fluid: the cyclooxygenase-2 (COX-2)/prostaglandin E2 (PGE2) pathway through interleukin-1 alpha (IL-1 alpha) signaling and non-COX-2/PGE2 pathway through TGF-beta receptor signaling. TGF-beta 1 and IL-1 alpha produced by odontogenic tumors/cysts induced osteoclastogenesis directly in the osteoclast precursor cells and indirectly via increased RANKL induction in the stroma.

The present report describes a male patient with a unilateral cleft lip and palate who presented with midfacial anteroposterior and transverse deficiency. Correction involved a two-stage surgical-orthodontic approach: asymmetric anterior distraction of the segmented maxilla followed by two-jaw surgery (LeFort I and bilateral sagittal splitting ramus osteotomies). The present case demonstrates that the asymmetric elongation of the maxilla with anterior distraction is an effective way to correct a transversely distorted alveolar form and midfacial anteroposterior deficiency. Furthermore, successful tooth movement was demonstrated in the new bone created by distraction.

Objective: The antilingular prominence (AP) is a well-known landmark used during planning of intraoral vertical ramus osteotomy (IVRO) in order to prevent inferior alveolar nerve (IAN) injury, but the location of this landmark is not always clear. In this study, we analyzed the anatomical relationship between the mandibular foramen (MF) and the lateral surface shape of the mandibular ramus for use during IVRO.Patients and methods: We retrospectively analyzed 276 mandibular halves in 67 patients with mandibular deformity and 71 patients without deformity imaged at our department from April 2003 to March 2014. We defined 17 points on the mandibular ramus, and examined the anatomical relationships using multiplanar reconstruction (MPR) images and three-dimensional computed tomography (3DCT) created from preoperative CT data.Results: The prevalence of the AP was 57.6% (159/276) of all cases. The MF was located inferior to the AP in all cases; it was posterior to the AP in 50.3% (80/159) of cases and the sigmoid notch (SN) in 24.6% (68/276) of cases. The minimum horizontal distance between the mandibular posterior ramus border and the MF was 9.28 mm in all cases.Conclusion: The AP cannot always serve as a reference point to prevent IAN damage during IVRO. The osteotomy incision should be placed within 9 mm of the posterior mandibular ramus border in order to prevent intraoperative IAN damage. This case report presents the management of a female patient with unilateral cleft lip and palate presenting with skeletal Class III malocclusion and a narrow upper dental arch with a midline deviation. The treatment plan involved asymmetric transverse distraction osteogenesis of the maxilla to make the upper dental midline coincident with the facial midline. After the treatment, a good facial profile and a close intercuspation of teeth were achieved. Occlusion remained stable with normal overjet and overbite after 2-year retention.

This article reports the case of a 44-year-old male with skeletal Class III, Angle Class III malocclusion and unilateral crossbite with concerns about obstructive sleep apnea syndrome (OSAS), esthetics and functional problems. To correct the skeletal deformities, the maxilla was anteriorly repositioned by employing LeFort I osteotomy following pre-surgical orthodontic treatment, because a mandibular setback might induce disordered breathing and cause OSAS. After active treatment for 13 months, satisfactory occlusion was achieved and an acceptable facial and oral profile was obtained. In addition, the apnea hypopnea index (AHI) decreased from 18.8 preoperatively to 10.6 postoperatively. Furthermore, after a follow-up period of 7 months, the AHI again significantly decreased from 10.6 to 6.2. In conclusion, surgical advancement of the maxilla using LeFort I osteotomy has proven to be useful in patients with this kind of skeletal malocclusion, while preventing a worsening of the OSAS.

Bilateral chylothorax is a rare complication after radical neck dissection. We report a case of bilateral chylothorax that developed after left radical neck dissection. A 40-year-old man was given a diagnosis of secondary left cervical lymph-node metastasis from left-sided tongue cancer (T1N0M0), and supraomohyoid neck dissection was performed. Since multiple lymph-node metastases had been found, modified radical neck dissection was performed. We ligated the thoracic duct because it ruptured intraoperatively. On the first postoperative day, a chyle leak was diagnosed, and the thoracic duct was ligated again. The chyle leak resolved, but on the fourth postoperative day, the patient complained of dyspnea and chest discomfort. Computed tomographic scans and chest X-ray films showed bilateral pleural effusions. Diagnostic thoracentesis was performed. Milky fluid was aspirated, and chylothorax was diagnosed. We used a conservative management approach, which involved total parenteral nutrition with total enteric rest. Repeated chest X-ray films showed no evidence of pleural effusion, and the patient was started to receive a low-fat diet on the 11th postoperative day. The patient had recovered completely.

Mesothelioma is a rare neoplasm that arises from the pleura or peritoneum. Although malignant mesothelioma is known to spread hematogenously, causing metastasis to various organs, there have been few reports on oral metastasis. The patient was a 75-year-old man who had an elastic soft mass, measuring 38 × 20 mm, in the right upper jaw gingiva. On biopsy the lesion was histopathologically diagnosed as a malignant pleural mesothelioma with metastasis to the upper jaw. The lesion shrank in response to radiotherapy. We have described a case of malignant pleural mesothelioma arising in the upper jaw gingiva. We discuss the related literature and summarize our findings.

口腔癌検診研修会基礎コースを修了した歯科医師を対象に、研修会終了後にアンケート調査を行った。2013年に行われた3回の研修会の研修者の合計は46人(3人が2回研修)であった。臨床経験年数は10〜42年(平均23.6年、中央値25年)に比較的長い研修者が多かった。口腔癌症例経験ありは70%に認められ、経験症例数は1〜3例が多かった。口腔癌症例経験者の中で癌の疑いがあると診断して専門機関に紹介した症例が93%と大部分を占めた。しかし、その中で早期癌を診断できた症例は45%に過ぎず、初診時に進行していた症例も46例に認められた。また、紹介が少し遅くなった症例が9%認められたが患者が専門機関をなかなか受診してくれなかった例も1例あった。癌の部位は舌が最も多く41%認められ、下顎歯肉32%、口底13%と続いた。研修会の時間・レベルは概ね適切で、講義・実習・テキストもためになり、今後の臨床に役立つとの回答が得られた。再研修の必要性は91%と多かったが、アドバンスコースの希望者は69%に留まり、ベーシックコースの希望者も22%認められた。また、再研修までの期間は1年の35%を前後にばらつきが認められた。研修者はより実践的に役立つ研修内容を希望していた。

In recent years, artificial biological materials have been commonly used for the treatment of bone tissue defects caused by trauma, tumors, or surgical stress. Although tricalcium phosphate (TCP) is a promising absorbent bone tissue reconstruction biomaterial, it has been reported that its biocompatibility and osteoconductivity depend on its preparation method and sintering temperature. In addition, although it is thought that the microenvironment produced by the extracellular matrix plays an important role in cell growth and differentiation, there have been few studies on how the geometric structure of artificial biological materials affects cells. In the present study, a new honeycomb TCP scaffold containing through-holes with diameters of 300 mm has been developed. The influence of the sintering temperature on the crystal structure and material properties of the honeycomb TCP scaffold was investigated using scanning electron microscopy and X-ray diffraction. Its biocompatibility and osteoconductivity were also evaluated by implantation into experimental animals. It was found that a beta-TCP scaffold sintered at 1200 degrees C exhibited high biocompatibility and osteoconductivity, and when it was loaded with BMP-2, it exhibited both osteoconductivity and osteoinductivity, promoting rapid bone formation in both ectopic and orthotopic areas. It is thus a highly promising bone reconstruction material that is expected to find clinical applications. (C) 2013 Wiley Periodicals, Inc.

ABSTRACT

Periodontitis is an inflammatory disease of polymicrobial origin affecting the tissues supporting the tooth. The oral anaerobic bacterium Porphyromonas gingivalis , which is implicated as an important pathogen for chronic periodontitis, triggers a series of host inflammatory responses that promote the destruction of periodontal tissues. Among the virulence factors of P. gingivalis , hemoglobin receptor protein (HbR) is a major protein found in culture supernatants. In this study, we investigated the roles of HbR in the production of inflammatory mediators. We found that HbR induced interleukin-8 (IL-8) production in the human gingival epithelial cell line Ca9-22. p38 mitogen-activated protein kinase (MAPK) and extracellular signal-related kinase 1/2 (Erk1/2) were activated in HbR-stimulated Ca9-22 cells. Inhibitors of p38 MAPK (SB203580) and Erk1/2 (PD98059) blocked HbR-induced IL-8 production. Additionally, HbR stimulated the translocation of NF-κB-p65 to the nucleus, consistent with enhancement of IL-8 expression by activation of the NF-κB pathway. In addition, small interfering RNA (siRNA) targeting activating transcription factor 2 (ATF-2) or cyclic AMP-response element-binding protein (CREB) inhibited HbR-induced IL-8 production. Moreover, pretreatment with SB203580 and PD98059 reduced HbR-induced phosphorylation of CREB and ATF-2, respectively. Combined pretreatment with an inhibitor of NF-κB (BAY11-7082) and SB203580 was more efficient in inhibiting the ability of HbR to induce IL-8 production than pretreatment with either BAY11-7082 or SB203580 alone. Thus, in Ca9-22 cells, the direct activation of p38 MAPK and Erk1/2 by HbR caused the activation of the transcription factors ATF-2, CREB, and NF-κB, thus resulting in the induction of IL-8 production.

Objectives: The purpose of this study was to explore the voice characteristics of patients with mandibular prognathism, and to investigate the effects of mandibular setback surgery on these characteristics using nonlinear dynamics and conventional acoustic analyses.
Materials and methods: Sixteen patients (8 males and 8 females) who had skeletal 3, class III malocclusion without cleft palate, and who underwent a bilateral sagittal split ramus osteotomy (BSSRO), were enrolled. As controls, 50 healthy adults (25 males and 25 females) were enrolled. The mean first LEs (mLE1) computed for each one-second interval, and the fundamental frequency (F0) and frequencies of the first and second formant (F1, F2) were calculated for each Japanese vowel.
Results and conclusions: The mLE1s for /u/ in males, and /o/ in females and the F2s for /i/ and /u/ in males, changed significantly after BSSRO. Class III voice characteristics were observed in the mLE1s for /i/ in both males and females, in the F0 for /a/, /i/, /u/ and /o/ in females, and in the F1 and F2 for /a/ in males, and the F1 for /u/ and the F2 for /i/ in females. Delayed tooth eruption can affect the accuracy of orthodontic diagnosis and treatment planning and could delay overall treatment. A girl, aged 7 years 4 months, who had unilateral delayed eruption of several teeth was successfully treated. The evaluation of delayed tooth eruption should be considered by the orthodontist because the role of these teeth can affect the overall treatment of malocclusion. In patients with delayed tooth eruption, careful and accurate diagnosis and treatment planning will allow the orthodontist to start treatment at the proper stage and might reduce the overall orthodontic treatment time. Toluene Exposure Leads to a Change in Expression Patterns of β Defensins in the Mouse Tracheal Epithelium

Defensins are generally implicated in the quick resistance of epithelial surfaces to microbials; however, recent reports have indicated that defensins also have unknown purposes in relation to noninfectious diseases. In this study, the localization patterns of anti-microbial peptides, β defensins (BDs), in the tracheal epithelium of male C3H mice under exposure to toluene were analyzed by immunohistochemistry. Mice were exposed one to ten times to toluene for 30 min by nose-only inhalation. Expression of BDs was revealed by immunohistochemistry in serial sections of trachea after the final exposure. Expression of BD-1 was usually observed at almost the same levels in all exposure groups, and expression of BD-2 was observed in the control group; however, the signals for BD-2 decreased gradually with frequency of exposure. In the group exposed ten times, expression of BD-2 decreased to far lower than that of the control group. No expression of BD-3 was detected in any groups. Interestingly, expression of BD-4 increased to the maximum in the group exposed four times and decreased to a level lower than that of the control in the group exposed ten times. The results of the present study indicated that toluene gas might change the expression pattern of BDs in the tracheal epithelial cells. The oscillation of expression of BD-4 was quite characteristic and might contribute to morphological damage in on the epithelial cells.

Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare malignant odontogenic tumor arising from odontogenic epithelial remnants within the jawbones. PIOSCC is histopathologically divided into 3 types: solid-type carcinoma, carcinoma derived from a keratocystic odontogenic tumor, and carcinoma derived from an odontogenic cyst. In this article, we report a case of solid-type PIOSCC involving reactive bone formation in the mandible in a 60-year-old female patient together with its histopathological and imaging findings. © 2012 Elsevier Inc. The purpose of this study was to determine the accuracy of a new high resolution digital camerabased three-dimensional system called Di3D by comparing with traditional direct anthropometry using digital calipers. The study sample consisted of 20 young adults, 10 male and 10 female. Facial soft tissue landmarks were identified and five linear distances were measured between the landmarks for each subject using the two methods
Di3D camera system and a digital caliper. No statistically significant differences (P &gt
0.05) were found for all five facial soft tissue distances measured by the Di3D camera system and the digital caliper. The magnitude of differences was very small (mean = 0.73 mm). Good congruence was observed between the means derived from the Di3D system and the digital caliper. The overall mean differences of the five measurements with these two methods were small enough to be clinically insignificant. <italic>Objectives</italic>. To clarify the difference between Lyapunov exponents (LEs) for cleft palate (CP) patients with hypernasality versus without hypernasality and to investigate the relationship between their LEs and nasalance scores (NSs).<italic>Material and Methods</italic>. Six CP patients with severe hypernasality (mean age 9.2 years) and six CP patients without hypernasality (mean age 8.0 years) were enrolled. Five Japanese vowels were recorded at 44.1 KHz, and the NSs were measured simultaneously. The mean first LE (mLE₁) from all one-second intervals was computed.<italic>Results</italic>. The mLE₁for /o/ in patients with hypernasality was significantly higher than that in patients without hypernasality. The correlation coefficients between the mLE₁and NS for all vowels were not statistically different.<italic>Conclusion</italic>. The voice signal of /o/ for the patients with hypernasality was more instable than in those without hypernasality. The chaotic phenomenon was independent of nasal resonance in CP voice.

Identification and characterization of local molecules directing the differentiation of chondrocytes to either transient or permanent cartilage are major issues in cartilage biology. Here, we found CCN family protein 3 (CCN3) was abundantly produced in rat developing epiphyseal cartilage. Evaluations in vitro showed that CCN3 repressed epiphyseal chondrocyte proliferation, while promoting matrix production in multiple assays performed. Furthermore, CCN3 enhanced the articular chondrocytic phenotype; whereas it repressed the one representing endochondral ossification. Additionally, the phenotype of growth plate chondrocytes and chondrogenic progenitors also appeared to be affected by CCN3 in a similar manner. This study was designed to evaluate detailed philtral morphology involving sex and racial differences and to clarify the objective parameters related to subjective judgments regarding the clarity of the philtrum. Fifty-five Japanese and 20 Mexican normal subjects aged 24 to 40 years were recruited, and three-dimensional facial data were acquired using a noncontacting optical surface scanner. The visual classification of the philtral column morphology in Japanese adults using facial photographs revealed that the predominant philtral column shape was different by sex. The three-dimensional anthropometric data showed significant sex differences, which were reflected in the more pronounced visual impression of the philtrum in men. A comparative study between Japanese and Mexican women also revealed certain racial differences. Furthermore, both the philtral depth and central angle of Cupid's bow were extracted as explanatory variables related to the subjective judgments for the clarity of the philtrum. These results suggest that the comprehensive evaluation of philtral morphologic features by three-dimensional analysis is a useful means of identifying the ideal parameters in philtral construction in cleft surgery.

Chondrocytes forming articular cartilage are embedded in a vast amount of extracellular matrix having physical stiffness and elasticity, properties that support the mechanical load from bones and enable the flexible movement of synovial joints. Unlike chondrocytes that conduct the growth of long bones by forming the growth plate, articular chondrocytes show suppressed cell proliferation, unless these cells are exposed to pathological conditions such as mechanical overload. In the present study, we found that one of the members of the CCN family, CCN3, was significantly expressed in chondrocytes isolated from the epiphyseal head in developing rat synovial joints. Evaluation of the effect of recombinant CCN3 on those chondrocytes revealed that CCN3 promoted proteoglycan synthesis, whereas this factor repressed the proliferation of the same cells. These results suggest a critical role for CCN3 in the regulation of the biological properties of articular chondrocytes. © The International CCN Society 2011.

This study involved a histologic, enzyme histologic, immunohistologic, and three-dimensional microstructure evaluating the extent of osteogenesis and repair in the human alveolar extraction socket achievable with an artificial bone substitute. After tooth extraction in 7 patients, extraction sockets were filled with Mastergraft (15% hydroxyapatite, 85% β-tricalcium phosphate complex). Radiomicrographs and histologic examinations were performed on samples obtained during dental implant placement procedure. On micro-computed tomography, new bone was observed in all collected samples, and osteogenesis was observed to have taken place around the artificial bone substitute. Histologically, active osteogenesis was found throughout the region observed. Addition of new bone around the Mastergraft was observed, and osteoblast-like cells were present. Cells that had partially invaded the artificial bone included tartrate-resistant acid phosphate-positive and CD34-positive cells. These findings indicate that the Mastergraft artificial bone induced osteogenesis in the jawbone and seemed effective for repairing bone defects. Copyright © 2011 by Mutaz B. Habal, MD.

Objective: The purpose of this study was to explore the 3-dimensional (3D) accuracy of magnetic resonance imaging (MRI) on cone-beam computed tomography (CBCT) images after the registration of MRI images on CBCT images. Materials and Methods: Three Japanese adult females volunteered for this study. To transform digital imaging and communication in medicine (DICOM) data derived from MRI and CBCT images into polygon data, five software programs were used. CBCT and MRI images were obtained within one week, and both were registered by the iterative closest point (ICP) method. To assess the accuracy of the composite MRI-CBCT, the measurement errors of the MRI-CBCT were verified. Measurement values were compared using frontal and cephalometric soft-tissue landmarks. Differences were analyzed using the non-parametric Mann-Whitney U test. Results: There were no significant linear measurement errors (P 0.05) when the images were measured from the superimposed MRI-CBCT images. Conclusion: The MRI images attained from MRI-CBCT registration showed accurate 3D linear measurements.

A new modification for safer submental orotracheal intubation 査読

In patients in whom a tracheal tube cannot be inserted through the nostrils due to multiple facial trauma or hypoplasty of the nose, submental orotracheal intubation (SOI) is performed to avoid tracheostomy. We report a new modification for SOI to minimize the risk of apnea. A 20-year-old man was scheduled for sagittal split ramus osteotomy. As the patient had severe hypoplasia of the nose, SOI was planned. Following orotracheal intubation with a spiral tube (first tube), a submental tunnel was surgically created. A second tube that had been confirmed, in advance, to snugly fit into the proximal end of the first tube was passed into the submental tunnel via a polypropylene cylinder and connected between the first tube and the breathing circuit. After careful withdrawal of the second tube through the submental tunnel, the first tube was directly connected to the breathing circuit after removal of the second tube. Although this technique requires additional time, apnea time is minimal even in patients in whom withdrawal of the tracheal tube through the submental tunnel takes time, because the second tube forms a link between the first tube and the breathing circuit, making it possible to ventilate the patient throughout the procedure. © Japanese Society of Anesthesiologists 2011.

The term atypical lipomatous tumor (ALT) is synonymous with well-differentiated liposarcoma (WDL). This tumor occurs very rarely in the tongue. Thus, it is difficult to predict its prognosis. Although recurrence of ALT/WDL is thought to be unlikely after complete excision, long-term follow-up is necessary when considering the pathologic conditions of this tumor at other sites. Here, we report a case of an ALT of the tongue, with a review of the literature. A 68-year-old man was referred to our hospital because of a tumor on the left side of his tongue. Upon palpation, the tumor was 12mm in diameter, circumscribed, elastic and hard, well demarcated, movable, and painless. We diagnosed the lesion as a lipoma and extirpated the tumor under local anesthesia. Because the specimen was histopathologically diagnosed as an ALT, as a precaution, we excised an additional 5mm from the area surrounding the original tumor under general anesthesia. Three years after the operation, the tongue demonstrated good healing without paresthesia or dysfunction, and to date there has been no evidence of recurrence.

Herein, we report a very rare case of pilomatricoma in the perioral region. A 27-year old Japanese female was referred to our clinic with a subcutaneous tumor in the angle of the left side of the mouth. The tumor was observed as a subcutaneous well-circumscribed and bony hard mass, without any changes of the skin surface, while a soft radiographic examination revealed a weak calcifying rounded mass. The tumor was extirpated using an intraoral approach, because of patient request. During the operation, the tumor was found to be partly attached to the surface skin, though easily removed from the surrounding tissue. The tumor was a rounded calcifying mass, 13. mm in diameter, and histologically diagnosed as a pilomatricoma. © 2010 Asian Association of Oral and Maxillofacial Surgeons.

症例は45歳女性で、1年前より左側上顎臼歯部に時折違和感を自覚していたが放置した。歯科治療で撮影されたパノラマX線で左側上顎洞内のX線不透過像を指摘された。顔貌は左右対称で、左側上顎眼窩下部に圧痛等の異常所見は認めなかった。左側上顎歯肉部に膨隆や腫脹、発赤等の異常所見を認めなかった。左上6、左上7は電気歯髄診断で生活反応を認めなかった。パノラマX線では左上678相当部の上顎洞底より左側上顎洞内に突出した類円形の塊状のX線不透過像が認められた。CTでは左側上顎洞内に上顎洞底から外向性に突出し上顎洞前壁、側壁、後壁と接する病変を認め、病変の内部のdensityは不均一であった。また、病変内部に歯を認めた。左側上顎骨腫瘍と診断し全身麻酔下に摘出術を施行した。病理組織学的所見は病変の内部に埋伏歯を認め、埋伏歯を取り囲むように塊状の不規則な形態の硬組織が認められた。硬組織はセメント質様、或いは骨様で、線維組織は細胞成分が豊富であった。術後経過は良好で術後6ヵ月現在、再発を認めていない。

Epulis osteoplastica is a rare disease. We report a case of giant epulis osteoplastica containing a largesequestrum. A 63-year-old man was referred to our clinic because of a painless swelling on the left mandibularalveolus. Intraoral examination revealed an elastic hard tumor with a normal surface in the left lower molar region.Radiographic examination showed a large radiopaque mass separate from mandibular bone. Gingival tumor excisionwas performed with the patient under general anesthesia. The excised specimen measured 50 X 38 X 28mm and contained the bony mass. Histopathologically, the lesion was diagnosed as epulis osteoplastica with asequestrum. The patient was followed up for 4 years after the operation, without any recurrence.

Vertical alveolar augmentation by distraction osteogenesis has become a standard pretreatment of dental implants due to the main benefit of lack of donor site complications, and many reports have shown its benefit with various types of alveolar distractors. However, autogenous bone transplantation and/or guided bone regeneration techniques are still the standard treatment for correcting a narrow alveolar ridge because of few commercially available distractors for alveolar widening. In this article, we treated a case by a mesh-plate type of alveolar widening distractor for a narrow posterior mandibular alveolar ridge after extirpation of a large jaw cyst.

Nasolabial cysts are rare non-odontogenic cysts that occur beneath the ala nasi, and debate about their complicated, unique pathogenesis continues. It is widely accepted that these lesions originate from the anlage of the nasolacrimal duct; however, some still think that nasolabial cysts arise from fissural cysts. The authors report a patient with a nasolabial cyst who also had a unilateral cleft lip and palate. This unusual finding may indicate a different origin for nasolabial cysts than what has been accepted in the past.

摂食障害が誘因と考えられた口蓋壊死性唾液腺炎の1例

26歳女。口蓋正中部疼痛を主訴とした。既往としてパニック障害、うつ病があり、25歳時より摂食障害を認め、過食と自己誘発性嘔吐を繰り返していた。右側口蓋に周囲硬結を伴う腫脹を認め、その中央部に壊死を伴う潰瘍を認めて口蓋悪性腫瘍を疑った。MRIでは右側口蓋にびまん性の腫脹を伴う境界明瞭な病変を認め、T1強調像で低信号、T2強調像で高信号を示した。潰瘍の生検を行い壊死性唾液腺炎と診断し、潰瘍にエレース軟膏とプロスタンディン軟膏を混和したガーゼを塗布し、保護床を装着して経過観察を行った。潰瘍は治癒傾向を示したが、生検6日後に左側口蓋に新たな潰瘍が出現した。右側病変と類似していたため同様の処置を行い、1ヵ月後に潰瘍は消失し、また経過観察中に過食や自己誘発性嘔吐も改善した。病理組織学的所見で小唾液腺の広範囲な腺房壊死や導管由来の扁平上皮化生を認めたが、粘液細胞の扁平上皮化生は認めず、腺房細胞周囲での著しい肉芽形成や炎症細胞浸潤も認めなかった。

顎矯正手術により長期間咬合の安定が得られた末端肥大症の1例

41歳女。下垂体腺腫と診断され経蝶形骨洞下垂体腫瘍摘出術を受け、以降末端肥大症の改善目的に持続性ソマトスタチンアナログの継続的投与を受けていた。今回、末端肥大症に伴う不正咬合および下顎前歯部歯間離開の改善目的に当科紹介となった。顔貌所見は顔面高および幅ともに大きく、前額部は突出し、眉部、眼窩外側縁は隆起していた。口腔内所見は舌は肥大し、睡眠時にいびきを伴っていた。上顎中切歯間と下顎前歯部に歯間離開を認め、上下顎中切歯ではover jetが-5mm、over biteが1mmを示し、上下第一大臼歯はIII級の対向関係にあった。側面セファロX線規格写真ではトルコ鞍の風船様拡大を認め、セファロ分析では骨格性下顎前突症を示した。末端肥大症に伴う骨格性下顎前突症および巨舌症と診断し、術前矯正治療後、下顎枝矢状分割術による顎矯正手術および舌縮小術を施行した。術後経過は良好で、術後4年4ヵ月のセファロX線写真分析で良好な下顎位と咬合を認めた。

An Uncommon Cleft Subtype of Unilateral Cleft Lip and Palate

The finding that the vomer plays a crucial role in maxillary growth suggests that the bilateral cleft configuration of unilateral cleft lip and palate (UCLP), in which the vomer is detached from the non-cleft-side secondary hard palate, negatively influences palatal development, and this hypothesis was tested. Sixty persons with complete UCLP, including those with the vomer detached from (n = 30, b-UCLP) and attached to (n = 30, u-UCLP) the secondary hard palate, were analyzed morphologically, with the use of cast models taken at 10 days, 3 mos, and 12 mos of age. The anterio-posterior palatal length at 12 mos of age in those with b-UCLP was significantly shorter than that in those with u-UCLP, by 8.7% (p < 0.05). In addition, palatal width development in the first year in those with b-UCLP was also significantly retarded. These results suggest that the uncommon bilateral cleft subtype in UCLP should be included in the cleft classification.

Aim: Peripheral ameloblastoma (PA) is a rare variant of ameloblastoma occurring in the extraosseous region. With regard to the histogenesis of the tumor, two major sources of origin are considered: odontogenic epithelial remnants and the gingival epithelium. In this study, we examined the immunohistochemical profiles of cytokeratins (CKs) and Ki-67 labeling index (LI) of PAs, and discuss the histogenesis and the biologic behavior of the PA. Materials and methods: Eight cases of PA were retrieved from the pathology files of 212 cases of ameloblastoma that had been registered at our hospital. Immunohistochemical staining was performed in seven cases using monoclonal antibodies of six CKs (7, 8, 13, 14, 18, and 19) and Ki-67. Results: All cases of PA expressed CK13, 14, and 19. CK18 was positive staining in six cases, and CK8 in five cases. This staining pattern was similar to that in intraosseous ameloblastomas (IAs). The mean of Ki-67 LI of PAs (1.91%) was significantly lower than that of IAs (4.82%) (P = 0.002). Conclusion: We consider that the PA originates from odontogenic epithelial remnants rather than from the gingival epithelium, and the Ki-67 LI of the tumor is a good prognostic indicator. © 2007 The Authors.

口蓋裂を伴う両側性斜顔面裂の1例

1歳2ヵ月、メキシコ人男児。術前所見では右側にTessier No.4顔面裂、左側にNo.5顔面裂(それぞれNo.4 cleft、No.5 cleft)、左側に顎裂部に連続する索状組織を認めた。右眼は開眼不能であり、角膜は肥厚、混濁し、失明していた。生後8ヵ月時に右側斜顔面裂の形成手術、左側の索状組織切除術を施行されたが、審美的・機能的改善が得られなかった。当科入院時、右側披裂部に手術後瘢痕、上唇の披裂を認め、下眼瞼は下方に強く牽引され閉眼不能であった。CTでは右側小眼球症を認め、右側眼窩内容積は左側に比べ狭小であった。右側の骨披裂は乳犬歯相当部から眼窩下縁にかけて、左側の骨披裂は第二乳臼歯相当部から眼窩下縁にかけて存在した。口蓋裂を伴う両側性斜顔面裂、右側No.4 cleft、左側No.5 cleftと診断し、No.4 cleftには連続Z形成術を応用し、No.5 cleftには口角形成術(松矢法)とZ形成術を応用した形成術を行った。術後経過は良好で、白唇、口角、頬部の良好な形態を確認した。

低位骨性癒着大臼歯症例に対するSingle-tooth dento-osseous osteotomyならびにCorticotomy 多数歯骨性癒着症例の治療経験

20歳男。上下左右の大臼歯の骨性癒着を主訴とした。口腔内所見では両側上下顎第2小臼歯、第1大臼歯が低位であり、同部で咬合を認めなかった。X線で両側下顎第2小臼歯、第1大臼歯に歯根膜腔を認めず、骨性癒着を疑った。両側上下顎第1大臼歯、第1小臼歯の低位骨性癒着歯と診断し、右側上下顎第1大臼歯と左側上顎第1大臼歯に対しCorticotomyを施行し、顎間ゴムの矯正装置により咬合誘導を行った。しかし、初回の処置だけで移動を確認したのは右側上顎第1大臼歯のみであり、それ以外の2歯については再手術による海綿骨の離断を追加した。残りの左側下顎第1大臼歯に対しては、Single-tooth dento-osseous osteotomyを施行し、顎間ゴムの矯正装置により咬合誘導を行った。4歯とも移動を確認し、術後4ヵ月のX線で歯根膜スペースの拡大から歯牙の脱臼を認め、術後6ヵ月には歯槽骨の良好な治癒状況を認め、左右臼歯部の咬合関係が回復した。

両側に発生した乳臼歯歯根嚢胞の1例と乳歯歯根嚢胞の臨床的検討

11歳男。学校検診で齲蝕を指摘され、某歯科医院を受診したところパラノラマX線検査で両側下顎第二乳臼歯根尖部にX線透過像を認められ当科に紹介された。諸検査により後続永久歯が先天的に欠損した乳臼歯歯根嚢胞と診断した。局所麻酔下に抜歯+嚢胞摘出術を施行し、術後経過良好であった。当院で1985〜2005年に経験した乳歯歯根嚢胞14例について臨床的検討を行った。男女比は1.8:1、初診時年齢は6〜13歳(平均9.6歳)であった。診断契機は「パノラマX線で偶然発見」が最も多く8例(57.1%)、次いで「顎骨の腫脹」4例(28.6%)、「疼痛」2例(14.3%)の順であった。治療法は抜歯+嚢胞摘出が9例(64.3%)、抜歯+開窓術が5例(35.7%)であった。

ビスフォスフォネートにより発症した下顎骨壊死の1例

Bisphosphonates (BPs) are widely used for the management of hypercalcemia related to malignancy and bone metastases from breast cancer in Japan. We report a case of osteonecrosis of the jaw (ONJ) associated with a history of BPs therapy forthe management of bone metastases from breast cancer. A 69-year-old woman was referred to our department becaused of unhealed extraction sockets of the lower left premolar and first molar, which were removed at a dental clinic. CT scans of the mandible showed regions of mottled bone and sequestra, and a clinical diagnosis of ONJ was made. After conservative treatment with antibiotics and local minor curettage, the inflammatory symptoms almost completely disappeared. Dentists should be aware of this potentially serious complication in patients receiving long-term treatment with BPs.

Chickenpox is a common infection due to varicella-zoster virus (VZV), which generally occurs in childhood. However, the incidence of chickenpox in adults is increasing because of changes in the social environment. In this paper, we report a case of adult chickenpox with the first symptom in the oral cavity. A 32-year-old Japanese woman presented with palatal ulcerative lesions and many bullae on her skin. The first symptom was continuous spontaneous pain at the left upper molar, present for 15 days before her first visit. Intravenous and oral acyclovir was given for a clinical diagnosis of VZV infection. Although the serum antibody examination showed no evidence of VZV infection, the symptoms led to a diagnose of adult chickenpox.

Long-term outcome and factors influencing bridging plates for mandibular reconstruction 査読

Vascularized bone grafts have become the preferred method of mandibular reconstruction. However, the technique is considered to increase both the operating time and blood loss, which might be associated with an increased morbidity and mortality. We conducted a retrospective analysis of 100 consecutive patients who underwent immediate bridging plate reconstruction. The median follow-up duration was 70 months. The 5-year overall survival rate was 69.9%. Cox multivariate analysis revealed that red blood cell transfusion was an independent prognostic factor for the overall survival. The plate survival with no complications was 62.2% at 5 years. Anterolateral defects and preoperative radiotherapy emerged as an independent adverse factor for plate survival. Ameloblastoma is one of the well-known odontogenic tumours that can be associated with calcifying odontogenic cysts (COCs), but only a few reports include detailed clinical and radiographic features. In this paper we demonstrate a case of large ameloblastomatous COC in the mandible. The radiographic examination revealed the presence of a multilocular cystic lesion in the right posterior part of the mandible containing the impacted lower second molar with remarkable expansion toward both lingual and buccal side. This feature was different from the general findings of COC and rather resembled that of ameloblastomas.

Open vertical osteotomy in the mandibular ramus is the one surgical option for the correction of mandible deformity. It is easy to assume that this procedure is useful for open reduction of condylar fractures. In this paper we describe a new systematic procedure for open reduction of high condylar fractures. The procedure involves vertical subcondylotomy and a new retractor for subcondylotomy to protect the mandibular neurovascular bundle.<BR>The major points of this operation are the use of vertical condylotomy, removal of the posterior border of the ramus and plating the condylar head with it, and accurate replacement of the posterior border of the ramus with mini plates prepared before osteotomy.

In this study, we clinically observed 146 patients with sports-related oral and maxillofacial fractures treated at the First Department of Oral and Maxillofacial Surgery, Osaka University Faculty of Dentistry during the past 15 years. The results were as follows.<BR>Among 146 patients, 134 were male and 12 were female. Although 62.5% of the patients were 15 to 24 years of age, there was an annual trend toward a decrease in patients aged 15 to 19 years. The most common causative sport event was baseball (54 patients), followed by rugby football (26 patients) and soccer (15 patients). There was also a decrease in baseballrelated cases and an increase in soccer-related cases in recent years. Isolated mandibular fractures occurred in 92 patients, and the most common site was the mandibular angle, followed by the condyle and canine. Among 37 isolated midface fractures, the most common site was the zygomatic arch (14 patients).<BR>Baseball-related mandibular fractures had a high incidence of two-line fractures and fractures of the anterior portion of the mandibular body as compared with other events.

Eight patients with unilateral condylar fractures of the mandible who were treated at our clinic by Martin-Eckelt lag screw osteosynthesis are described.<BR>The patients were aged 14 to 78 years. There were 4 males and 4 females. Four patients had fractures of the base of the condylar process, and four had fractures of the condylar neck. Two patients had no dislocation, four had dislocation, and two had luxation. Clinical examinations 6 months after primary operation showed that mouth opening was not less than 40mm in any patient. Radiographic examinations revealed a correct position and normal form of the condylar head in six patients, but resorption of the condylar head was found in two patients. These two patients required extirpation of condylar head for reduction. Since the use of a lag screw decreases the supply of nutrients from the bone marrow of mandibular ramus to the condylar head, the condylar head should not be removed from the surrounding soft tissue during operation.

Sialolith usually occurs in the submandibular gland and its occurrence in the parotid gland is relatively rare. Especially the occurrence of sialoliths in the bilateral parotid gland is very rare and only three cases of such a case have been reported. In this paper, a case of sialolithiasis occurring in the bilateral parotid gland was reported.<BR>The patient, a 65-year-old Japanese male, visited our clinic complaining of swelling of the right buccal mucosa including the orifice of the parotid duct. Roentogen examination revealed the presense of sialoliths in the right parotid duct and the deep lobe of the left parotid gland. Extirpation of the sialoliths of the right parotid duct was performed under local anesthesia, and 6 sialoliths of 2 mm or less in diameter were observed. Though extirpation of the left sialolith was not performed because of lack of objective and subjective symptoms, long-term follow up is considered nessesary.

We report six cases of unilateral transverse facial cleft. Five patients were male and one was female. One case was diagnosed as solitary macrostomia, and other cases had accessory ears. Among latter cases, one also suffered from microtia and was diagnosed as the first and second branchial syndrome. Another case was diagnosed as Goldenhar syndrome because of the presence of epibulbar dermoid. The other cases were diagnosed as the first branchial syndrome.

Pharyngeal flap surgery is used in the prevention of hypernasality and the devel-opment of articulation disorders that are associated with velopharyngeal incompetence after primary palatal closure. However, surgery may result in failure when the level of the pharyngeal flap base is not determined properly with relation to that of velopharyngeal move-ment or when the flap shrinks because of scar contraction through prolonged exposure of its raw surface. This report describes one patient who had suffered from recurrence of velopharyngeal incompetence after pharyngeal flap surgery. Lateral cephalographic and nasopharyngeal fiberscopic examinations identified the cause of flap atrophy to be marked scar contraction, and the flap base was remarkably lower than the level of velopharyngeal closure in the patient. Unified velopharyngoplasty was newly performed on the basis of find-ings obtained from lateral cephalographic and nasopharyngeal fiberscopic examinations to correct the recurrence of velopharyngeal incompetence in the patient. One year postoperatively, complete closure of the velopharynx in coordination of the flap during speech was confirmed by nasopharyngeal fiberscopy.

Hemifacial microsomia is defined as a condition affecting primarily aural, oral, and mandibular development and its clinical features may vary. In this paper we report a case of hemifacial microsomia with cleft palate. A Japanese girl 8 months of age was referred to our clinic for the treatment of cleft palate. Clinically, she had hypoplasia and inferior displacement of the left auricle, stenosis of the external auditory canal, a hypoplastic mandible, and facial palsy at the left side of the lower lip. There was no significant asymmetry of the maxillary dental arch or soft palate, and the left side of the soft palate showed mild hypokinesia. Palatoplasty with a push-back method was performed under general anesthesis at the age of 1 year 5 months. A few reports have shown a trend toward velopharyngeal insufficiency in hemifacial microsomia due to hypoplastic palatal muscles and poor neural control of these muscles. In the future, comprehensive speech therapy will be required in this patient.

We reported two cases of calcifying epithelioma arising in subcutaneous tissue around the auricle. One case was in a 11-year-old Japanese girl who visited our clinic complaining of a mass at the left preauricular region, which had been present since the age of 3. Radiographic examination revealed the presence of calcification measuring 10×8 mm in diameter. Enucleation was performed, and a histopathological diagnosis of a calcifying epithelioma was made. Histopathologically, the tumor was composed mainly of shadow cells, and bone formation was also observed. The other case was in a 52-year old Japanese man, who visited our clinic complaining of a mass at the left side of the neck. The tumor was located at the subauricular region, and the surface of the tumor had undergone partial necrosis. The tumor was enucleated with the surrounding granulation tissue. The histopathological diagnosis was a calcifying epithelioma. Histopathological examination revealed the tumor to be composed of basophilic cells and shadow cells; calcifications were also observed.<BR>The postoperative course of both patients was uneventful, with no evidence of recurrence.

We clinically observed patients with sialolithiasis of the submandibular gland who visited our department during the past 10 years (January 1984 to December 1993). The following results were obtained.<BR>One hundred forty-six cases of sialolithiasis of the submandibular gland were diagnosed. The highest proportion of patients were in their 3rd decade of life, and the ratio of men to women was 1 to 1.4. The most common site of occurrence was within the submandibular duct (84.2%), and larger sialoliths tended to occur in the gland and the transitional portion. The main clinical symptoms of submandibular sialolithiasis were swelling and/or tenderness of the gland, and these symptoms tended to occur on the gland side. Infection of the oral cavity tended to occur on the orifice side.

Sialolithiasis mainly occurs in the submandiblar gland and its occurrence in minor salivary glands is very rare. In this paper we report three cases of sialolithiasis occurring in the minor salivary glands of the lip. Two cases occurred in the upper lip and one in the lower lip. Histopathologically, the case in the lower lip consisted of more than six calcified masses in the minor salivary gland tissue.

Benign fibrous histocytoma is a well-established soft tissue neoplasm, and the occurrence of this lesion in the oral cavity is rare. Only 3 cases of this lesion arising centrally in the mandible have been reported in the literature. In this paper, we reported a patient with benign fibrous histiocytoma of the mandible.<BR>A 42-year-old Japanese woman was referred to our department complaining of swelling on the lingual side of the right mandibular body in November 1987. Radiographic examination demonstrated a illdefined radiolucent lesion in the right body of the mandible extending from 7 to the ramus. We performed segmental resection of the mandible in March 1988. A solid yellowish lesion was observed in the mandible body, and partial destruction of the lingual palate was found. Histopathologically, the lesion was composed of spindle-shaped figroblastic cells and histiocytic cells. The histiocytic cells showed negative staining for S-100 protein and positive staining for a-1 antitrypsin. Based on these clinical and histopathological findings, the lesion was diagnosed as benign fibrous histiocytoma. The postoperative course was good and no signs of recurrence have been seen almost 6 years and 4 months after operation.

This study investigated the correlation between clinical and histopathological findings in 171 cases clinically diagnosed as oral leukoplakia. Histopathological examination revealed squamous cell carcinoma in 14 cases, leukoplakia with epithelial dysplasia in 68 cases, and hyperkeratosis without epithelial dysplasia in 89 cases. According to the grade of epithelial dysplasia, the leukoplakias were classified into mild, moderate, severe epithelial dysplasia and carcinoma in situ, which included 49, 14, 3 and 2 cases, respectively. High grades of epithelial dysplasia were mainly observed on the lateral edge of the tongue or the oral floor. There was a close correlation between the clinical appearance of lesions and a high grade of dysplasia; the majority of the cases with high grades of dysplasia or with early invasive carcinoma clinically showed erosive-type lesions.

Four cases of pituitary hormone deficiency with cleft defects of the lip and palate were reported. In all cases, we noticed the small premaxilla, but there was no cerebral anomaly such as holoprosencephaly.The present report suggests the small premaxilla is a sign of the pituitary hormone deficiency in cleft lip and palate children.This clinical feature would be useful for diagnosis of the pituitary hormone deficiency.

Conservative treatment for ameloblastoma frequently produces recurrence. But even after radical treatment for this lesion, recurrence may occur. Recurrence in such cases is most likely due to involvement of adjacent soft tissue structures by tumor tissue.<BR>In such cases of recurrence after conservative treatment, ameloblastoma proliferate immediately in soft tissue and gingiva. We performed segmental resection of mandible bone involving adjacent soft tissue and one-stage microsurgical reconstruction using radial forearm skin flap.

As cleidocranial dysplasia is one of the systemic bone disorders characterised by anomalies of clavicle, cranium, maxilla and mandible, complications are rarely found in other than the bone system.<BR>We have examined a boy, 8 years and 3 months old when first seen, of this disease accompanied with pseudohypoparathyroidism I, who is perhaps the second reported case in Japan. He was diagnosed to be pseudohypoparathyroidism I based on the attacks of tetany, calcified bodies in the brain found by CT, low Ca and high P levels in the blood, and on the Ellsworth-Howard examination. He was diagnosed to have cleidocranial dysplasia because of the specific motion of approaching shoulders, late existence of the anterior fontanell, and teeth anomalies as written below.<BR>At the examination of 9 years and 4 months, D+D/D+D and cuspids of E/E/E/E erupted, but no permanent tooth had erupted, all remaining in the jaws ascertained by roentgenography. Several serious carious deciduous teeth were extracted at this time. At the examination of 12 years and 3 months, however, no permanent tooth was seen except for the incisor edges of the lower central incisors.<BR>Cephalometric x-ray analysis at 9 years and 4 months revealed maxillary maldevelopment to anterior and down directions, progenia, and maldevelopment of the mandibular angle. These were also found at the 12 years 3 months examination, but the amount of the maxillary and mandibular developments during these periods were nearly the same as that of normal boys.<BR>The etiology of cleidocrnial dysplasia was discussed based on the present case and on some literatures reporting the complications that accompanied this disease.

This study was intended to clarify the mechanical contraction properties of the tensor veli palatini muscle in dogs. Twenty adult dogs were used under an intraperitoneum anesthesia of the pentobarbital sodium. The tension of the tensor veli paltini muscle was recorded by electrical stimulation to the motor nerve.<BR>Results were as follows:<BR>1. The contraction time and half relaxation time of maximal isometric twitch of the muscle were calculated as 13.7±0.7 msec and 10.4±1.2 msec, respectively.<BR>2. A clear summation of muscle contractions was obtained with stimulation at 20 Hz and maximal tetanic tension was at 90 Hz. The tetanus twitch tension ratio wasfound to be 10.7±1.1.<BR>3. The maximum rate of tension rise was observed at 150 Hz and value was 2.5±0.5 times as long as the rate of a twitch contraction.<BR>4. The contraction rate of an isotonic twitch was 3.2±0.7% and the maximum contraction rate was obtained at 80 Hz which was 26.3±1.9%.
