MR imaging in the assessment of a solitary bone cyst

Introduction: It might be difficult to differentiate solitary bone cysts (SBCs) from other cysts and tumors when the lesion occurs at an unusual location and/or shows atypical radiographic features. Magnetic resonance imaging (MRI) is the most useful modality for evaluating the internal structure of lesions and may have the ability to distinguish SBCs from other lesions. However, few studies exist regarding MRI of SBCs in the oral region. The present study reports on the MR imaging of nine patients with SBCs and describes the diagnostic value of the MRI findings. Materials and methods: Nine patients with SBCs in the mandible were examined using MRI. In all patients, T1- and T2-weighted images (T1-WI, T2-WI) were obtained and contrast enhanced images and dynamic MRI were performed in four cases. Results: MR findings revealed homogeneous intermediate signal intensities (SI) on T1-WI and high SI on T2-WI in all cases. These SI indicated that the SBCs included homogeneous liquid. On the dynamic MRIs in all cases, the SBCs were enhanced from the margin to the inner part, indicating a gradual exudation of the contrast agent from the surrounding medullae to the inner part. These phenomena were not observed on the dynamic MRIs of the other true cysts with epithelial lining. Conclusion: MRI, especially dynamic MRI, can provide useful information for distinguishing SBCs from other cysts or tumors. Furthermore, dynamic MRI findings suggested that the liquid components of SBCs might have infiltrated from the surrounding medullae. © 2002 Elsevier Science Ireland Ltd. All rights reserved.

Signal intensity changes in T2-weighted MR image of the human trapezius muscle upon cold pressor stimulation

Objective: The ability to measure hemodynamics of skeletal muscle proper is one of the major goals for muscle pain researchers. The aim of the present study was to evaluate the ability of signal intensity (SI) in T2-weighted trapezius muscle magnetic resonance imaging (MRI) to detect intramuscular hemodynamic changes during cold pressor stimulation (CPS).
Materials and methods: Fifteen healthy volunteers (mean age, 25.9 +/- 2.1 years) participated in this study. T2-weighted MRI was acquired using a 1.5 tesla MR unit with a body array coil. The slice level was set perpendicular to the muscle long axis at the mid-point of the horizontal portion of the right trapezius muscle. Cold pressor stimulation (4degreesC) was applied to each subject's right foot and ankle for 2 min. The SI changes were recorded continuously for 7 min before, 2 min during, and 6 min after withdrawal of cold pressor stimulation. Six of these subjects also underwent a mock-CPS trial.
Results: The mean SI level in T2-weighted trapezius muscle MRI significantly increased during CPS (P<0.0001, one way repeated measure ANOVA) and returned to the baseline level after cold pressor withdrawal. No statistically significant signal changes were observed across the mock-CPS trial subjects. These findings are identical to the cold pressor-induced hemodynamic changes documented in the trapezius muscle by near-infrared spectroscopy evaluation.
Conclusions: Sl measurement in T2-weighted trapezius muscle MRI is sufficiently sensitive to detect intramuscular hemodynamic changes during CPS.

Assessment of carcinoma in the sublingual region based on magnetic resonance imaging

In the present study, we attempted to diagnose and detect the extent of tumors in the sublingual region using magnetic resonance imaging (MRI) and dynamic MRI. MRI with or without gadopentetate dimeglumine (Gd-DTPA)enhancement in seven lesions of the sublingual regions was performed. The seven lesions included four cases of mucoepidermoid carcinoma (mucoepidermoid Ca), two cases of adenoid cystic carcinoma (ACC), and one case of squamous cell carcinoma (SCC). Whether the tumor was malignant or benign, as well as the differential diagnosis, could not be determined on the basis of the MR signals, even when enhancement was performed. Dynamic MRI was performed in five cases, two cases of ACC, two cases of mucoepidermoid Ca, and one case of SCC. The dynamic MRI showed a rapid enhancement at 30-45 sec in all five cases before the normal sublingual gland began to be enhanced. The early phases at 30-45 sec of the dynamic MRI in five cases showed marked enhancement before the normal sublingual glands were enhanced, and therefore could clearly show the extent of the lesions. In conclusion, the dynamic MRI may be useful in differentiating malignant from benign tumors, and in detecting the extent of the tumors in the sublingual carcinomas.

A study of hypoxic cell radiosensitizer applied to Ehrlich ascite tumour: a comparison of FC43 emulsion and pentoxyfilline

In this study, we examined the effects of various combinations of treatments involving radiation, injections of perfluorochemicals (FC-43 emulsion) and injections of pentoxifylline on the growth delay of Ehrlich ascite tumours. Ehrlich ascite tumour cells were transplanted into the legs of ddY-strain mice. Tumour-bearing mice were divided into seven groups: group 1, no treatment; group 2, irradiated only; group 3, injected with FC-43 emulsion and kept in a carbogen atmosphere; group 4, injected with pentoxifylline and nicotinamide; group 5, injected with FC-43 emulsion, kept in a carbogen atmosphere and irradiated; group 6, injected with pentoxifylline and nicotinamide and irradiated; and group 7, injected with FC-43 emulsion, pentoxifylline and nicotinamide, kept in a carbogen atmosphere and irradiated. When 20 Gy irradiation was applied, tumour growth delay was 11 days in group 2, 20 days in group 5, 22 days in group 6, and 24 days in group 7. For a growth delay of 20 days, the dose modifying factor was 1.95+/-0.04 (standard deviations) in group 5, 1.97+/-0.09 standard deviations in group 6, and 2.01+/-0.07 standard deviations in group 7. It was concluded that FC-43 emulsion and pentoxifylline did not have an interactive effect.

Adenomatoid odontogenic tumor is a rare benign and odontogenic tumor that is frequently misdiagnosed as other odontogenic cysts and tumors on radiographic examination. To acquire additional information of adenomatoid odontogenic tumor, we performed magnetic resonance imagings (MRI) at a case of adenomatoid odontogenic tumor. The lesion was divided between the peripheral portion with a thick circular shape and the central portion with a round shape on the basis of the signal intensity (SI) of MRI. The peripheral portion showed intermediate SI contained multifocal no SI on T1WI, high SI contained multifocal no SI on T2WI, and heterogeneous enhancement on CE-T1WI. These multifocal areas corresponded to the numerous punctate radiopaque foci shown on computed tomography. The central portion showed homogeneous low SI on T1WI, homogeneous very high SI on T2WI, and no enhancement on CE-T1WI. Macroscopic examination revealed the round shaped lesion included one large cystic space correspondent to the central portion with a clear cystic wall correspondent to the peripheral portion on MRI. The MRI features corresponded to the macroscopic findings of the histopathological examination.

A case of 59-year-old male patient presenting with metastasis to the mandible from malignant pheochromocytoma is described. The conventional radiographs and CT images suggested that the lesion was malignant osteogenic tumors or metastatic tumors due to the existence of calcification and widespread periosteal sunburst spiculation. On MRI, an expansive mass was clearly depicted and the signal intensities of the lesion were low to intermediate on T1 weighted image with intermediate to high signal intensity on T2 weighted image. A strong enhancement of the lesion was also observed on contrast enhanced T1 weighted image. On maximum intensity projection image in the arterial phase, the mass showed exceedingly early enhancement and excessively dislocated adjacent vessels. The diagnosis of a pheochromcytoma was difficult on the basis of these imagings. The final diagnosis was based on a biopsy of the mandible and I-131 Meta-iodobenzylguanidine scintigraphy (MIBG) scintigraphy. A primary lesion of the right adrenal showed low uptake due to wide centric necrosis and metastatic lesions of liver, lumber vertebrae, ribs and sacroiliac joint showed high uptake on the I-131 MIBG scintigraphy. The final diagnosis was nonfunctioning malignant pheochromocytoma due to the absence of elevation of catecholamine or its metabolite.

Assessment of dynamic MRI of Warthin's tumors arising as multiple lesions in the parotid glands

We examined three patients with multiple synchronous Warthin's tumors in the bilateral parotid and described the value of using dynamic MRI. The time course of the contrast index (CI curves) was calculated from a dynamic series. Warthin's tumors showed intermediate signal intensity on T1WI, heterogeneous high and intermediate signal intensity on T2WI and a slight enhancement on Gd-T1WI. Warthin's tumors of CI curves showed specific findings. CI curves in each lesion showed the same pattern. It was difficult to diagnose masses as Warthin's tumors using only MR images on T1, T2 and Gd-T1WI. Dynamic MRI can distinguish Warthin's tumors from other possible tumors except for oncocytoma. Therefore, the use of dynamic MRI is recommended as a diagnostic method for Warthin's tumors in multiple synchronous lesions of the parotid gland. (C) 2002 Elsevier Science Ltd. All rights reserved.

76歳男.上顎義歯の接触痛を主訴とした.左側上顎切歯から大臼歯の口蓋歯肉に外向性の腫瘤がみられ,大臼歯部および上顎第二臼歯部には色素沈着を認めた.又,右側頸部リンパ節と左側顎下リンパ節を触知した.MRI T2強調像では左側口蓋部腫瘤および両側頸部リンパ節が中〜高信号を示し,同部はダイナミックMRIで造影剤注入後45〜60秒後より急激に信号強度の増加を認めた.又,MR血管造影法では左側上顎腫瘤へ流入する血管が描出された.上顎部分切除術,両側全頸部郭清術と化学療法を施行したが,その後肺転移,右側頬部転移を経て,1年後には多発性肺転移を認めた.病理組織学的に腫瘍細胞は著明な多形性と異型性を示し,メラニン産生細胞を認め,免疫組織学的にS-100蛋白およびHMB45陽性であったため,悪性黒色腫と診断した
