＜文献概要＞70歳,女性。3年前より下口唇にびらんが出現した。1年半前にC型慢性肝炎に対して,レジパスビル/ソホスブビル内服による直接作用型抗ウイルス薬治療を行いHCV RNAは陰性化したが,下口唇の症状は残存していた。初診時,下口唇にびらんを認め,病理検査では真皮内に多数の形質細胞浸潤がみられた。下口唇のびらんは0.1%タクロリムス軟膏を外用して治癒した。形質細胞性口唇炎と肝疾患との関連について述べた報告があるが,自験例ではHCVのウイルス量と口唇炎の病勢との間に直接的な関連は認められなかった。今後は症例の蓄積により,HCVとの関連が示唆される皮膚疾患に対する,直接作用型抗ウイルス薬の効果の有無が明らかにされることを期待したい。

Generalized pustular psoriasis (GPP) is the most severe psoriasis variant. Mutations in the IL-36 antagonist IL36RN, in CARD14 or AP1S3 provide genetic evidence for autoinflammatory etiology but cannot explain its pathogenesis completely. Here we demonstrate that unopposed IL-36 signaling promotes antigen-driven and likely pathogenic T-helper type 17 (Th17) responses in GPP. We observed that CD4+ T cells in blood and skin lesions of GPP patients were characterized by intense hyperproliferation, production of the GPP key mediator, IL-17A, and highly restricted TCR repertoires with identical T-cell clones in blood and skin lesions, indicating antigen-driven T-cell expansions. The clonally expanded CD4+ T cells were major producers of IL-17A. IL-36 signaling substantially enhanced TCR-mediated proliferation of CD4+ T cells. Moreover, GPP patients showed preferences for HLA-DRB1∗14, HLA-DQB1∗05, and HLA-DQB1∗03. We conclude that in GPP unopposed IL-36 signaling and certain HLA-class II alleles may cooperate in promoting antigen-driven Th17 responses, which in the obvious absence of exogenous triggers may reflect autoimmune reactions. This study reveals a pathogenic pathway where innate immune dysregulation promotes T-cell-mediated inflammation in GPP.

A 79-year-old female who had suffered from asymptomatic, annular, hyperkeratotic, brownish macules on the extremities and trunk for 7 years spontaneously developed pruritic erythematous changes of the pre-existing lesions as well as newly formed erythematous papules. A histopathological examination showed typical cornoid lamella and a perivascular infiltration of lymphocytes and eosinophils in the upper dermis. Laboratory investigations revealed eosinophilia. After an inflammatory period lasting for 16 months, the pruritic erythematous changes and the eosinophilia gradually improved, leaving annular, brownish, mildly hyperkeratotic macules. Our case was unusual for its transient inflammatory condition of disseminated superficial porokeratosis (DSP), which was consistent with the previously reported "Inflammatory stage of DSP".

A Case of Streptobacillus moniliformis Infection with Cutaneous Leukocytoclastic Vasculitis.

A 71-year-old man presented with a high fever, polyarthralgia, petechiae and palpable purpura accompanied by livedoid change on his legs and feet. Histopathological findings of the purpura revealed perivascular infiltration of neutrophils, mononuclear cells, and nuclear debris, and extravasation of red cells mainly in the upper dermis: all signs consistent with leukocytoclastic vasculitis. Small vessel thrombi, which are characteristic features of septic vasculopathy, were also observed. Direct immunofluorescence showed negative results. Blood culture revealed the growth of gram-negative bacilli. Subsequently, 16S rRNA sequencing of DNA confirmed the organism as Streptobacillus moniliformis, which is the causative pathogen of rat-bite fever. He had frequently encountered wild rats in his house although there was no evidence of rat bite on his body. Empiric therapy with intravenous administration of ceftriaxone in combination with azithromycin hydrate led to a prompt resolution of the symptoms. Precise history-taking related to contact with rats and detection of skin eruptions suggestive of leukocytoclastic vasculitis on the extremities, especially on the feet, can be clues to Streptobacillus moniliformis infection. Familiarity with its cutaneous features is important for early diagnosis; the evidence herein may also help in understanding its underlying pathogenesis.

A 77-year-old female with a 19-year history of rheumatoid arthritis was admitted to our hospital because of the simultaneous appearance of oral mucositis and erosions on her back accompanied by appetite loss.She had been treated with increasing doses of oral methotrexate (MTX), which had started 3 years and 9 months previously, up to 8 mg/week combined with oral prednisolone 5 mg/day.Laboratory investigation revealed leukocytopenia and thrombocytopenia, followed by anemia 3 days later.MTX was stopped and the patient was successfully treated with folic acid 24 mg/day and granulocytecolony stimulating factor 75 μg/day for 3 days.Although oral mucositis is a well-known side effect of MTX, the initial diagnosis of our case was difficult because of the unusual symptom of erosions on her back, which were reminiscent of pemphigus.It is important to recognize that oral mucositis is a predictive sign of bone marrow suppression and to be aware of the appearance of skin erosions and/or ulcerations as side effects of MTX therapy.
