Facial frontal morphological changes related to mandibular setback osteotomy using cephalograms. Reviewed International journal

AIM: The purpose of this study was to examine morphological changes of the mandibular region using frontal cephalograms following bilateral sagittal split osteotomy (BSSO) for mandibular setback. Furthermore, we compared the stability between screw and plate fixation. METHODS: Pre and postoperative frontal cephalograms were taken of 26 patients. Parameters, Angle "A" which is formed by constructed lines along the mandibular ramus, and mandibular width at the angle were calculated. The subjects were analyzed according to the types of bony fixation, whether by screws or plates. RESULTS: Angle "A" and mandibular angular width of total numbers did not widely change at early period following operation. Late postoperatively, mandibular width of total numbers decreased in conjunction with increasing Angle "A". When comparing the parameters between screw and plate groups, the plate group showed a decrease of Angle "A" as an early change. Late postoperatively, the plate group showed an increase of Angle "A" and a decrease of mandibular width, while the screw group had smaller changes. CONCLUSION: We demonstrate postoperative inclination of mandibular lower ramus towards the facial midline. We suggest that the types of rigid fixation have an influence on frontal skeletal morphology, namely that screw fixation has higher stability.

A case of giant epulis osteoplastica with a sequestrum

Epulis osteoplastica is a rare disease. We report a case of giant epulis osteoplastica containing a largesequestrum. A 63-year-old man was referred to our clinic because of a painless swelling on the left mandibularalveolus. Intraoral examination revealed an elastic hard tumor with a normal surface in the left lower molar region.Radiographic examination showed a large radiopaque mass separate from mandibular bone. Gingival tumor excisionwas performed with the patient under general anesthesia. The excised specimen measured 50 X 38 X 28mm and contained the bony mass. Histopathologically, the lesion was diagnosed as epulis osteoplastica with asequestrum. The patient was followed up for 4 years after the operation, without any recurrence.

Vertical alveolar augmentation by distraction osteogenesis has become a standard pretreatment of dental implants due to the main benefit of lack of donor site complications, and many reports have shown its benefit with various types of alveolar distractors. However, autogenous bone transplantation and/or guided bone regeneration techniques are still the standard treatment for correcting a narrow alveolar ridge because of few commercially available distractors for alveolar widening. In this article, we treated a case by a mesh-plate type of alveolar widening distractor for a narrow posterior mandibular alveolar ridge after extirpation of a large jaw cyst.

Nasolabial cysts are rare non-odontogenic cysts that occur beneath the ala nasi, and debate about their complicated, unique pathogenesis continues. It is widely accepted that these lesions originate from the anlage of the nasolacrimal duct; however, some still think that nasolabial cysts arise from fissural cysts. The authors report a patient with a nasolabial cyst who also had a unilateral cleft lip and palate. This unusual finding may indicate a different origin for nasolabial cysts than what has been accepted in the past.

摂食障害が誘因と考えられた口蓋壊死性唾液腺炎の1例

26歳女。口蓋正中部疼痛を主訴とした。既往としてパニック障害、うつ病があり、25歳時より摂食障害を認め、過食と自己誘発性嘔吐を繰り返していた。右側口蓋に周囲硬結を伴う腫脹を認め、その中央部に壊死を伴う潰瘍を認めて口蓋悪性腫瘍を疑った。MRIでは右側口蓋にびまん性の腫脹を伴う境界明瞭な病変を認め、T1強調像で低信号、T2強調像で高信号を示した。潰瘍の生検を行い壊死性唾液腺炎と診断し、潰瘍にエレース軟膏とプロスタンディン軟膏を混和したガーゼを塗布し、保護床を装着して経過観察を行った。潰瘍は治癒傾向を示したが、生検6日後に左側口蓋に新たな潰瘍が出現した。右側病変と類似していたため同様の処置を行い、1ヵ月後に潰瘍は消失し、また経過観察中に過食や自己誘発性嘔吐も改善した。病理組織学的所見で小唾液腺の広範囲な腺房壊死や導管由来の扁平上皮化生を認めたが、粘液細胞の扁平上皮化生は認めず、腺房細胞周囲での著しい肉芽形成や炎症細胞浸潤も認めなかった。

顎矯正手術により長期間咬合の安定が得られた末端肥大症の1例

41歳女。下垂体腺腫と診断され経蝶形骨洞下垂体腫瘍摘出術を受け、以降末端肥大症の改善目的に持続性ソマトスタチンアナログの継続的投与を受けていた。今回、末端肥大症に伴う不正咬合および下顎前歯部歯間離開の改善目的に当科紹介となった。顔貌所見は顔面高および幅ともに大きく、前額部は突出し、眉部、眼窩外側縁は隆起していた。口腔内所見は舌は肥大し、睡眠時にいびきを伴っていた。上顎中切歯間と下顎前歯部に歯間離開を認め、上下顎中切歯ではover jetが-5mm、over biteが1mmを示し、上下第一大臼歯はIII級の対向関係にあった。側面セファロX線規格写真ではトルコ鞍の風船様拡大を認め、セファロ分析では骨格性下顎前突症を示した。末端肥大症に伴う骨格性下顎前突症および巨舌症と診断し、術前矯正治療後、下顎枝矢状分割術による顎矯正手術および舌縮小術を施行した。術後経過は良好で、術後4年4ヵ月のセファロX線写真分析で良好な下顎位と咬合を認めた。

An Uncommon Cleft Subtype of Unilateral Cleft Lip and Palate

The finding that the vomer plays a crucial role in maxillary growth suggests that the bilateral cleft configuration of unilateral cleft lip and palate (UCLP), in which the vomer is detached from the non-cleft-side secondary hard palate, negatively influences palatal development, and this hypothesis was tested. Sixty persons with complete UCLP, including those with the vomer detached from (n = 30, b-UCLP) and attached to (n = 30, u-UCLP) the secondary hard palate, were analyzed morphologically, with the use of cast models taken at 10 days, 3 mos, and 12 mos of age. The anterio-posterior palatal length at 12 mos of age in those with b-UCLP was significantly shorter than that in those with u-UCLP, by 8.7% (p < 0.05). In addition, palatal width development in the first year in those with b-UCLP was also significantly retarded. These results suggest that the uncommon bilateral cleft subtype in UCLP should be included in the cleft classification.

Aim: Peripheral ameloblastoma (PA) is a rare variant of ameloblastoma occurring in the extraosseous region. With regard to the histogenesis of the tumor, two major sources of origin are considered: odontogenic epithelial remnants and the gingival epithelium. In this study, we examined the immunohistochemical profiles of cytokeratins (CKs) and Ki-67 labeling index (LI) of PAs, and discuss the histogenesis and the biologic behavior of the PA. Materials and methods: Eight cases of PA were retrieved from the pathology files of 212 cases of ameloblastoma that had been registered at our hospital. Immunohistochemical staining was performed in seven cases using monoclonal antibodies of six CKs (7, 8, 13, 14, 18, and 19) and Ki-67. Results: All cases of PA expressed CK13, 14, and 19. CK18 was positive staining in six cases, and CK8 in five cases. This staining pattern was similar to that in intraosseous ameloblastomas (IAs). The mean of Ki-67 LI of PAs (1.91%) was significantly lower than that of IAs (4.82%) (P = 0.002). Conclusion: We consider that the PA originates from odontogenic epithelial remnants rather than from the gingival epithelium, and the Ki-67 LI of the tumor is a good prognostic indicator. © 2007 The Authors.

口蓋裂を伴う両側性斜顔面裂の1例

1歳2ヵ月、メキシコ人男児。術前所見では右側にTessier No.4顔面裂、左側にNo.5顔面裂(それぞれNo.4 cleft、No.5 cleft)、左側に顎裂部に連続する索状組織を認めた。右眼は開眼不能であり、角膜は肥厚、混濁し、失明していた。生後8ヵ月時に右側斜顔面裂の形成手術、左側の索状組織切除術を施行されたが、審美的・機能的改善が得られなかった。当科入院時、右側披裂部に手術後瘢痕、上唇の披裂を認め、下眼瞼は下方に強く牽引され閉眼不能であった。CTでは右側小眼球症を認め、右側眼窩内容積は左側に比べ狭小であった。右側の骨披裂は乳犬歯相当部から眼窩下縁にかけて、左側の骨披裂は第二乳臼歯相当部から眼窩下縁にかけて存在した。口蓋裂を伴う両側性斜顔面裂、右側No.4 cleft、左側No.5 cleftと診断し、No.4 cleftには連続Z形成術を応用し、No.5 cleftには口角形成術(松矢法)とZ形成術を応用した形成術を行った。術後経過は良好で、白唇、口角、頬部の良好な形態を確認した。

低位骨性癒着大臼歯症例に対するSingle-tooth dento-osseous osteotomyならびにCorticotomy 多数歯骨性癒着症例の治療経験

20歳男。上下左右の大臼歯の骨性癒着を主訴とした。口腔内所見では両側上下顎第2小臼歯、第1大臼歯が低位であり、同部で咬合を認めなかった。X線で両側下顎第2小臼歯、第1大臼歯に歯根膜腔を認めず、骨性癒着を疑った。両側上下顎第1大臼歯、第1小臼歯の低位骨性癒着歯と診断し、右側上下顎第1大臼歯と左側上顎第1大臼歯に対しCorticotomyを施行し、顎間ゴムの矯正装置により咬合誘導を行った。しかし、初回の処置だけで移動を確認したのは右側上顎第1大臼歯のみであり、それ以外の2歯については再手術による海綿骨の離断を追加した。残りの左側下顎第1大臼歯に対しては、Single-tooth dento-osseous osteotomyを施行し、顎間ゴムの矯正装置により咬合誘導を行った。4歯とも移動を確認し、術後4ヵ月のX線で歯根膜スペースの拡大から歯牙の脱臼を認め、術後6ヵ月には歯槽骨の良好な治癒状況を認め、左右臼歯部の咬合関係が回復した。

両側に発生した乳臼歯歯根嚢胞の1例と乳歯歯根嚢胞の臨床的検討

11歳男。学校検診で齲蝕を指摘され、某歯科医院を受診したところパラノラマX線検査で両側下顎第二乳臼歯根尖部にX線透過像を認められ当科に紹介された。諸検査により後続永久歯が先天的に欠損した乳臼歯歯根嚢胞と診断した。局所麻酔下に抜歯+嚢胞摘出術を施行し、術後経過良好であった。当院で1985〜2005年に経験した乳歯歯根嚢胞14例について臨床的検討を行った。男女比は1.8:1、初診時年齢は6〜13歳(平均9.6歳)であった。診断契機は「パノラマX線で偶然発見」が最も多く8例(57.1%)、次いで「顎骨の腫脹」4例(28.6%)、「疼痛」2例(14.3%)の順であった。治療法は抜歯+嚢胞摘出が9例(64.3%)、抜歯+開窓術が5例(35.7%)であった。

A case of osteonecrosis of the mandible caused by bisphosphonates

Bisphosphonates (BPs) are widely used for the management of hypercalcemia related to malignancy and bone metastases from breast cancer in Japan. We report a case of osteonecrosis of the jaw (ONJ) associated with a history of BPs therapy forthe management of bone metastases from breast cancer. A 69-year-old woman was referred to our department becaused of unhealed extraction sockets of the lower left premolar and first molar, which were removed at a dental clinic. CT scans of the mandible showed regions of mottled bone and sequestra, and a clinical diagnosis of ONJ was made. After conservative treatment with antibiotics and local minor curettage, the inflammatory symptoms almost completely disappeared. Dentists should be aware of this potentially serious complication in patients receiving long-term treatment with BPs.

Sialolipoma is a new variant of salivary gland lipoma, which was first proposed by Nagao et al. (Histopathology 2001; 38: 30) in 2001. We report this rare case of sialolipoma in the hard palate. A 60-year-old Japanese woman was referred to our department complaining of a painless swelling on the right side of the hard palate. Intra-oral examination revealed a soft, elastic, dome-shaped mass with I cm in diameter located in the posterior part of the hard palate. Magnetic resonance imaging examination revealed high intensity on T-1-weighted image and iso-intensity on T-2-weighted image. Incisional biopsy revealed that the tumor was encapsulated by fibrous tissue, consisted of adipose tissue, and also contained normal salivary gland tissue peripherally. First diagnosed as an ordinary lipoma of the hard palate, the tumor was excised. According to the recent criteria of histologic findings of sialolipoma, we rediagnosed the tumor as sialolipoma of the hard palate.

A new attachment for proximal segment positioning for bimaxillary dysgnathic surgery--technical note. Reviewed International journal

BACKGROUND: The frequent insertion of self-tapping screws for securing the proximal positioning plate in bimaxillary osteotomy often cause loosening of screws and plate, which produces incorrect repositioning. In this article, a newly developed base-attachment for a T-shaped proximal positioning plate for bimaxillary osteotomy is presented. MATERIAL: The base-attachment set consists of a titanium base-attachment and two titanium tapped screws. The attachment has a shallow depression having the same shape as the basal part of the positioning plate. METHOD: The base-attachment is secured by self-tapping screws until the end of the procedure, and the T-shaped proximal positioning plate is placed and secured by two titanium-tapped screws. RESULT: By using this base-attachment, the T-shaped plate can be easily placed repeatedly without difficulty. CONCLUSION: This base-attachment set may be useful for the frequently repositioned proximal plate in bimaxillary osteotomy.

A case of adult chickenpox with the first symptom in the oral cavity

Chickenpox is a common infection due to varicella-zoster virus (VZV), which generally occurs in childhood. However, the incidence of chickenpox in adults is increasing because of changes in the social environment. In this paper, we report a case of adult chickenpox with the first symptom in the oral cavity. A 32-year-old Japanese woman presented with palatal ulcerative lesions and many bullae on her skin. The first symptom was continuous spontaneous pain at the left upper molar, present for 15 days before her first visit. Intravenous and oral acyclovir was given for a clinical diagnosis of VZV infection. Although the serum antibody examination showed no evidence of VZV infection, the symptoms led to a diagnose of adult chickenpox.

Long-term outcome and factors influencing bridging plates for mandibular reconstruction Reviewed

Vascularized bone grafts have become the preferred method of mandibular reconstruction. However, the technique is considered to increase both the operating time and blood loss, which might be associated with an increased morbidity and mortality. We conducted a retrospective analysis of 100 consecutive patients who underwent immediate bridging plate reconstruction. The median follow-up duration was 70 months. The 5-year overall survival rate was 69.9%. Cox multivariate analysis revealed that red blood cell transfusion was an independent prognostic factor for the overall survival. The plate survival with no complications was 62.2% at 5 years. Anterolateral defects and preoperative radiotherapy emerged as an independent adverse factor for plate survival. The use of bridging plates is an option for lateral mandibular reconstruction with no preoperative irradiation to avoid the risk from blood transfusion. (C) 2005 Elsevier Ltd. All rights reserved.

Ameloblastoma is one of the well-known odontogenic tumours that can be associated with calcifying odontogenic cysts (COCs), but only a few reports include detailed clinical and radiographic features. In this paper we demonstrate a case of large ameloblastomatous COC in the mandible. The radiographic examination revealed the presence of a multilocular cystic lesion in the right posterior part of the mandible containing the impacted lower second molar with remarkable expansion toward both lingual and buccal side. This feature was different from the general findings of COC and rather resembled that of ameloblastomas.
