    本症例では、顎関節症状と下顎骨の左方偏位を伴う顔面非対称症例に対して、下顎枝垂直骨切り術を伴う外科的矯正治療を行い、顎関節症状の改善と偏位側への側方滑走運動の改善を認めたので報告する。患者は19歳女性で、下顎の左方偏位と右側顎関節の開閉口時の疼痛を主訴に来院した。上顎骨は標準範囲内で後方位傾向、下顎骨は標準範囲内で前方位傾向にあり、骨格性III級を示した。大臼歯関係はAngle III級で、上顎前歯歯軸は唇側傾斜、下顎前歯歯軸は標準範囲内で舌側傾斜傾向を示し、上下顎前歯部に叢生を認めた。顔面正中に対して上顎歯列正中は一致、下顎歯列正中は5.0mm左方に偏位していた。初診時の顎口腔機能検査所見では、左方への側方滑走運動の制限と左右下顎頭の開閉口路の不一致を認めた。以上から、上下顎前歯部叢生、下顎骨の左方偏位と偏位側への側方滑走運動の制限を伴う顔面非対称の骨格性下顎前突症例と診断し、下顎枝垂直骨切り術により遠位骨片を右側で6.0mm、左側で1.0mm後方移動し、顔面非対称が改善された。治療終了時の顎口腔機能検査所見では、右側下顎頭の疼痛が消退し、左右下顎頭と下顎前歯の運動経路のばらつきが減少するとともに、偏位側への側方滑走運動量が増加した。以上から、偏位側への側方滑走運動の制限を伴う顔面非対称症例において、下顎枝垂直骨切り術を伴う外科的矯正治療を行い、正貌・側貌の形態的改善だけでなく、顎関節症状や偏位側への側方滑走運動の改善等、機能的な改善が得られることが示された。(著者抄録)

    The superior salivatory nucleus (SSN) contains parasympathetic preganglionic neurons innervating the sub mandibular and sublingual salivary glands. Cevimeline, a muscarinic acetylcholine receptor (mAChR) agonist, is a sialogogue that possibly stimulates SSN neurons in addition to the salivary glands themselves because it can cross the blood-brain barrier (BBB). In the present study, we examined immunoreactivities for mAChR subtypes in SSN neurons retrogradely labeled with a fluorescent tracer in neonatal rats. Additionally, we examined the effects of cevimeline in labeled SSN neurons of brainstem slices using a whole-cell patch-clamp technique. Mainly M1 and M3 receptors were detected by immunohistochemical staining, with low-level detection of M4 and M5 receptors and absence of M2 receptors. Most (110 of 129) SSN neurons exhibited excitatory responses to application of cevimeline. In responding neurons, voltage-clamp recordings showed that 84% (101/120) of the neurons exhibited inward currents. In the neurons displaying inward currents, the effects of the mAChR antagonists were examined. A mixture of M1 and M3 receptor antagonists most effectively reduced the peak amplitude of inward currents, suggesting that the excitatory effects of cevimeline on SSN neurons were mainly mediated by M1 and M3 receptors. Current-clamp recordings showed that application of cevimeline induced membrane depolarization (9/9 neurons). These results suggest that most SSN neurons are excited by cevimeline via M1 and M3 muscarinic receptors.

    The aim of this study was to clarify the effect of vagal afferent activation on salivation and swallowing-like events. Salivation is part of a reflex induced by stimulation of the oral area during feeding or chewing. Recently, we reported that nausea induced by gastroesophageal reflux (GER) activation produced salivation and swallowing in humans. Here, we investigated the ability of visceral sensation to enhance salivation and swallowing in rodents in order to inform the mechanism of GER-mediated stomatognathic activation. First, we administered LiCl to anesthetized male rats to induce nausea. LiCl significantly increased salivation and increased the activity of the vagal afferent nerve. Next, we simultaneously recorded salivation and swallowing using an electrode attached to the mylohyoid muscle during vagal afferent stimulation in a physiological range of frequencies. Vagal afferent stimulation significantly increased salivation and swallowing-like events in a frequency-dependent manner. A muscle relaxant, vecuronium bromide, diminished the swallowing-like response but did not affect salivation. These results indicate that visceral sensation induces salivation and swallowing-like events in anesthetized rodents through vagal afferent activation.

    This article reports successful use of orthodontic anchoring screws to treat bimaxillary protrusion and mandibular inclined third molars in a 25-year-old woman with complaints of crowded anterior teeth and polishing difficulties of third molars. She was diagnosed with skeletal Class II malocclusion with bimaxillary protrusion and mesial inclination of mandibular bilateral third molars. The maxillary first and mandibular second premolars were extracted, and two orthodontic anchoring screws were implanted in the buccal area of the maxillary molars. The total active treatment period was 24 months. After treatment, the facial profile and occlusion, including third molars, successfully improved, and stomatognathic functions were maintained after orthodontic treatment. Over a 24-month retention period, the satisfactory facial profile and adequate occlusion were maintained.

    Obesity has a prevalence of 15-30% among European and American populations. It is an incurable chronic disease associated with considerable mortality and co-morbidity. The co-morbidity risk can be reduced substantially by a moderate weight loss of 5-15%. Notably, additional weight gain exacerbates the morbidity of any concurrent disease. Obesity is also recognized as the basis for metabolic syndrome. Recent research has shown that adipocytes secrete various hormones and cytokines that contribute to obesity. Leptin is an adipostatic hormone that acts on receptors in the hypothalamus to suppress food intake and increase energy consumption. Reduced sensitivity to this molecule can trigger the onset of obesity. Neuropeptides such as leptin also affect salivary secretion. Various neuropeptides have been identified in saliva; the associated receptors are located in the salivary glands or in the nerves innervating the salivary glands. Obesity is associated with hyposalivation and thereby related to several aspects of oral health, such as caries and periodontitis. Hyposalivation is a severe morbidity that can lead to a precipitous decline in oral hygiene, which further leads to multifocal dental caries and periodontitis, or even cardiac disorders.     Functional dyspepsia (FD) is a functional gastrointestinal disorder (FGID). According to the Rome III consensus, FD is divided into 2 subgroups: epigastric pain syndrome (EPS) and postprandial distress syndrome (PDS). Although multiple mechanisms of FD pathogenesis have been suggested, its underlying etiology and pharmacological therapy remain unclear. Ghrelin is a gut-derived peptide found in the stomach. It plays a role in the regulation of gastric motility and appetite. The ghrelin gene encodes 3 molecular forms, acyl ghrelin, des-acyl ghrelin and obestatin. Acyl ghrelin acts as an endogenous ligand for growth hormone secretagogue receptor; furthermore, it is orexigenic, with effects on food intake, energy homeostasis and gastrointestinal motility. Des-acyl ghrelin exerts an opposite effect to acyl ghrelin. Obestatin exerts an inhibitory effect on the motor activity of the antrum and duodenum in fed animals. These peptides exert differential effects on gut motility and food intake. The therapeutic potential of ghrelin has attracted attention due to its varied bioactivities. Certain studies have shown that total ghrelin levels are significantly lower in patients with FD compared with healthy volunteers and that the acyl ghrelin levels of patients with FD are higher compared with healthy volunteers. However, a recent study demonstrated that acyl ghrelin levels in patients with PDS were lower compared with healthy volunteers; the association between FD and other ghrelin family gene products also remains unclear. Although certain studies have demonstrated the beneficial effects of acyl ghrelin administration and its agonist in patients with FD, only a few clinical reports exist. Further studies are required in order to examine the effects of ghrelin on FD.

    In the 1990s the number of patients diagnosed with taste disorders in the USA and Japan was over one million people each year, and the number is increasing annually. Taste disorders are caused by several factors such as genetic disease, head trauma, structural changes, glossodynia, cancer, change of lifestyle, and more. The role of zinc in the treatment of taste disorders has been studied since the oral administration of zinc by patients was reported to improve their taste disorders. Carbonic anhydrase (CA), a zinc metalloenzyme, has also been studied in association with taste disorders, since the regulation of serum CA levels was shown to influence the effect of orally administrated zinc in the treatment of taste disorders. Zinc is an essential trace element that contributes to the active center of approximately 300 enzymes. Studies have revealed that zinc is involved in various physiological functions. Moreover, some medications have been shown to induce a zinc deficiency, which has been associated with a variety of clinical conditions. Hence, since the relationship between taste disorder and serum zinc concentration has been discussed for long time, taste disorder may be useful in diagnosing zinc deficiency. Moreover, it appears that medicines of the zinc-containing supplement type contribute to the treatment of taste disorders caused by zinc deficiency. Orally administered zinc has been shown to directly stimulate food intake via neuropeptide in the hypothalamus. Therefore, zinc administration may potentially be used to treat taste disorders, as well as several other diseases by stimulating feeding. The article presents some promising patents on the role of zinc in the treatment of taste disorders.

    Eating disorders, including anorexia and bulimia nervosa, are potentially life-threatening syndromes characterized by severe disturbances in eating behavior. An effective treatment strategy for these conditions remains to be established, as patients with eating disorders tend to suffer from multiple relapses. Because ghrelin was originally discovered in the stomach mucosa, it has been widely studied over the past decade in an effort to uncover its potential roles; these studies have shed light on the mechanism by which ghrelin regulates food intake. Thus, studying ghrelin in the context of eating disorders could improve our understanding of the pathogenesis of eating disorders, possibly resulting in a promising new pharmacological treatment strategy for these patients. In addition, early detection and treatment of eating disorders are critical for ensuring recovery of young patients. Oral symptoms, including mucosal, dental, and saliva abnormalities, are typically observed in the early stages of eating disorders. Although oral care is not directly related to the treatment of eating disorders, knowledge of the oral manifestations of eating disorder patients may aid in early detection, resulting in earlier treatment; thus, oral care might contribute to overall patient management and prognosis. Moreover, ghrelin has also been found in saliva, which may be responsible for oral hygiene and digestion-related functions. This review discusses the pharmacological potential of ghrelin in regulating food-intake and the role of saliva and oral care in young patients with eating disorders.

    Ghrelin was recently identified as the first endogenous ligand for growth hormone secretagogue receptors (previously known as orphan receptors). Two major molecular forms of ghrelin are found in the stomach and plasma: acyl ghrelin with O-n-octanoylated serine at position 3 and des-acyl ghrelin. Interestingly, these distinct molecular forms play contrasting roles in ingestive behavior, upper gastrointestinal motility, and gastric acid secretion. Acyl ghrelin stimulates food intake both in fed and fasted rodents and induces adiposity, alleviates ingestive behavior, stimulates gastrointestinal motor activity and gastric acid secretion, and accelerates gastric emptying in several species. In contrast, des-acyl ghrelin has been demonstrated to disrupt gastric motility in rats and delay gastric emptying in mice and rats. Thus, acyl ghrelin is a potent stimulator of ingestive behavior, gastrointestinal motility, and gastric acid secretion, whereas des-acyl ghrelin exerts opposing effects on ingestive behavior and gastrointestinal motility but does not affect gastric acid secretion. Ghrelin is expected to be used in various applications such as therapeutic strategies for obesity, anorexia, cachexia, and cardiovascular disease in the clinical setting. In this article, we review the evidence on the roles of acyl ghrelin and des-acyl ghrelin in the regulation of ingestive behavior, upper gastrointestinal motility, and gastric acid secretion in mammals

    The superior salivatory nucleus (SSN) contains preganglionic parasympathetic neurons to the submandibular and sublingual salivary glands. Cevimeline, a muscarinic acetylcholine receptor agonist, stimulates the salivary glands and is presently used as sialogogue in the treatment of dry mouth. Since cevimeline passes through the blood-brain barrier, it is also able to act on muscarinic acetylcholine receptors in the central nervous system. Our preliminary experiment using the whole-cell patch-clamp technique has shown that cevimeline excites SSN neurons in rat brain slices, suggesting that SSN neurons have muscarinic acetylcholine receptors; however, it is unclear which subtypes of muscarinic acetylcholine receptors exist in SSN neurons. In the present study, we investigated immunohistochemically muscarinic acetylcholine receptor subtypes, M1 receptor (M1R), M2R, M3R, M4R, and M5R in SSN neurons. SSN neurons innervating the salivary glands, retrogradely labeled with a fluorescent tracer from the chorda-lingual nerve, mostly expressed M3R immunoreactivity (-ir) (92.3%) but not M1R-ir. About half of such SSN neurons also showed M2R- (40.1%), M4R- (54.0%) and M5R-ir (46.0%); therefore, it is probable that SSN neurons co-express M3R-ir with at least two of the other muscarinic receptor subtypes.     Cevimeline, a therapeutic drug for xerostomia, is an agonist of muscarinic acetylcholine receptors (mAChRs), and directly stimulates the peripheral mAChRs of the salivary glands. Since cevimeline is distributed in the brain after its oral administration, it is possible that it affects the central nervous system. However, it is unknown how cevimeline affects the superior salivatory (SS) neurons, which control submandibular salivation. In the present study, we examined the effects of cevimeline on the SS neurons using the whole-cell patch-clamp technique in brain slices. In Wistar rats (6-10 days), the SS neurons were retrogradely labeled by Texas Red applied to the chorda-lingual nerve. Two days after injection, whole-cell recordings were obtained from the labeled cells, and miniature excitatory postsynaptic currents (mEPSCs) were examined. Cevimeline induced the inward currents dose-dependently and increased the frequency of mEPSCs. Therefore, it is suggested that cevimeline enhances the excitability via post- and presynaptic muscarinic receptors in the rat SS neurons. In conclusion, cevimeline may enhance the excitability of the SS neurons.

    患者は初診時年齢21歳2ヵ月の女性で、前歯部が噛み合わないことを主訴に来院した。初診時に両側下顎頭の吸収が疑われたため、1年間の経過観察を行い、22歳2ヵ月時に下顎の位置が初診時と変化していないことを確認し、上顎前突を伴うアングルI級、骨格II級ハイアングル症例と診断した。クワドヘリックス装置による上顎の拡大と、バイヘリックスによる下顎の拡大後、歯科矯正用アンカースクリューによる上下顎大臼歯の圧下と上顎大臼歯の遠心移動を併用してマルチブラケット装置により前歯部被蓋を改善した。動的治療期間は4年10ヵ月であった。上顎臼歯部の遠心移動によって口唇の突出感が緩和され、側貌の改善が認められた。保定開始後5年1ヵ月が経過し、安定した咬合と下顎位が維持されている。(著者抄録)

    本研究の目的は、矯正治療中の口唇顎裂もしくは口唇口蓋裂を伴う患者の保護者における心理状態や関心事を明らかにすることである。鹿児島大学医学部・歯学部附属病院矯正歯科を受診した口唇顎裂もしくは口唇口蓋裂を伴う未成年患者の保護者86名を対象とし、歯科衛生士による質問紙調査を行った。VAS法を用いた質問では、保護者を患者の発達段階により5群(幼児期、学童期・小学校低学年、学童期・小学校高学年、青年前期・中学生、青年中期以降)に分類し、VAS値の差を統計学的に検討した。質問紙調査の結果、矯正治療の必要性を知った時は、「悲しみと怒りおよび不安」の心理状態を示した保護者が最も多く、そのほとんどが「矯正治療が長期に及ぶ」ことによる「不安」であった。また、保護者は「顔貌」などの審美的項目に対して特に高い関心を示し、「社会生活の不安」についても関心が高かった。一方、「矯正治療の期間」や「矯正歯科での1回の診療時間」において、青年中期以降群の保護者は、幼児期や学童期群の保護者と比較して、VAS値が有意に高く、保護者は長期間の矯正治療と長い診療時間に関して負担に感じていた。「矯正装置のトラブル」においては、学童期・小学校高学年群の保護者は、小学校低学年群と比較して、有意にVAS値が高く、トラブルが多いと回答した。使用中の矯正装置の種類は、ほぼ同じであることから、治療が長期化することで、保護者はトラブルの負担を感じやすいことが示唆された。以上から口唇顎裂もしくは口唇口蓋裂を伴う患者の保護者は、特に審美的な問題について関心が高く、矯正治療の期間や診療時間が長いことに対する不安や負担の程度は、患者の発育段階によって異なることが明らかとなった。(著者抄録)

    従来の固定式大臼歯遠心移動装置とスケレタルアンカレッジ装置の臼歯部と前歯部への治療効果について検討した。日英4つの電子的データベースを用いて、包含条件を含む文献を検索し、セファロメトリックな方法で調査された治療前後のデータを含む各排除項目を満たす文献に選別し、さらに9項目に及ぶ質的な条件を満たしたものをメタ・アナリシスに供した。スクリーニングで45件の文献を抽出し、さらに質的解析によって8文献を抽出した。大臼歯の遠心移動に対して、歯科矯正期アンカースクリューを用いたスケレタルアンカレッジ装置は、従来の固定式大臼歯遠心移動装置と比較して、前歯部への影響がなく効果的にかつ効率的に歯を動かしたが、ペンデュラム装置は、大臼歯の遠心傾斜を誘発した。約1年間の大臼歯遠心移動で、従来の固定式大臼歯遠心移動装置では3〜4mm、スケレタルアンカレッジ装置で4〜5mm程度の遠心移動が期待できた。

    ラットを使用し、顎下腺・舌下腺の唾液分泌の副交感神経の起始核である上唾液核ニューロンに対するセビメリンの影響を電気生理学的に検討した。さらに、免疫組織化学的実験により上唾液核ニューロンに存在するムスカリン様アセチルコリン受容体のサブタイプを検討した。セビメリンは上唾液核ニューロンの興奮性を促進した。全ての上唾液核ニューロンのシナプス後膜にムスカリン様アセチルコリン受容体が存在し、約60%のニューロンにおいては、シナプス前膜においてもムスカリン様アセチルコリン受容体が存在した。上唾液核ニューロンに存在するムスカリン様アセチルコリン受容体はM1およびM3受容体で、M1受容体を介した反応が大きかった。顎下腺・舌下腺の唾液分泌の中枢である上唾液核ニューロンにはM1受容体およびM3受容体が存在し、そのうち、セビメリンは主にM1受容体を介して上唾液核ニューロンの興奮性を促進する可能性が示唆された。

    Gap junctional intercellular communication among osteocytes in chick calvaria, their natural 3D environment, was examined using FRAP analysis. Cell-cell communication among osteocytes in chick calvaria was mediated by Cx43 and was regulated by extracellular pH(o) extracellular calcium ion concentration, and PTH.
    Introduction: The intercellular network of communication among osteocytes is mediated by gap junctions. Gap junctional intercellular communication (GJIC) is thought to play an important role in integration and synchronization of bone remodeling. We hypothesized that extracellular pH (pH.) and extracellular calcium ion concentration ([Ca2+](e)), both of which are dynamically altered by osteoclasts during bone remodeling, affect GJIC among osteocytes. Using fluorescence replacement after photobleaching (FRAP) analysis, we examined the effect of changes in pH(o) and [Ca2+](e) and addition of PTH on GJIC in osteocytes in chick calvaria. Additionally, we examined the role of intracellular calcium on the regulation of GJIC among osteocytes.
    Materials and Methods: Anti-Connexin43 (Cx43) immunolabeling was used to localize gap junctions in chick calvaria. GJIC among osteocytes in chick calvariae was assessed using FRAP.
    Results: Cx43 immunoreactivity was detected in most of the osteocyte processes. FRAP analysis showed dye-coupling among osteocytes in chick calvariae. In untreated osteocytes, fluorescence intensity recovered 43.7 +/- 2.2% within 5 min after photobleaching. Pretreatment of osteocytes with 18 alpha-GA, a reversible inhibitor of GJIC, significantly decreased fluorescence recovery to 10.7 +/- 2.2%. When pH(o) was decreased from 7.4 to 6.9, fluorescence recovery significantly decreased from 43.3 +/- 2.9% to 19.7 +/- 2.3%. Conversely, when pH(o) was increased from 7.4 to 8.0, fluorescence recovery was significantly increased to 61.9 +/- 4.5%. When [Ca2+](e) was increased from 1 to 25 mM, fluorescence recovery was significantly decreased from 47.0 +/- 6.1% to 16.1 +/- 2.1%. In bone fragments exposed to 1.0-10 nM rPTH for 3 h, replacement of fluorescence was significantly increased to 60.7 +/- 7.2%. Chelating intracellular calcium ions affected GJIC regulation by [Ca2+](e) and PTH.
    Conclusions: Our study of cell-cell communication between osteocytes in chick calvaria showed for the first time that GJIC among osteocytes is regulated by the extracellular environment and by hormonal stimulation during bone remodeling. This method may be more biologically relevant to living bone than current methods.

    本年度は、骨細管内の構造解析から得られた流体-構造連成解析を進めて流体応力が骨細胞の表面にどのように変形を与えているかを、共同研究者である亀尾佳貴先生を中心としたグループが国際誌Biomechanics and Modeling in Mechanobiologyに発表した。本研究では、骨細管壁と骨細胞突起を結ぶテザリングエレメントが骨細管内を流れる体液の移動で引っ張られ、細胞突起に付着する膜部位で10%を超える細胞膜変形を生じることが確認できた。この変形はメカニカルストレスにより細胞内カルシウム上昇などを生じるには十分な細胞膜変形量であることから、これら細胞突起周辺の微細構造が骨細胞の細胞突起をメカノセンサーとして重要な役割を果たしていることが示唆された。また、新たにFIB-SEM(Focused Ion Beam-Scanning Electron Microscopy)で取得した高解像度連続断層写真を用いて、これまでより広域の細胞突起の3次元形態解析を行った。得られた断層画像から細胞突起、骨細管壁を自動抽出するために機械学習を初めて応用することができた。結果、DSC(dice similarity coefficient)は、83%となり、人が手動で抽出する同等の程度まで精度を上げることができた。これにより抽出した3次元画像から形態計測を行い骨細管の微細構造を数値化することにも成功した。以上の結果をJournal of Bone and Mineral Metabolismに報告した。また、これまでの研究の成果を第80回日本矯正歯科学会学術大会で行われた日本学術会議歯学委員会臨床系歯学分科会主催の公開シンポジウム「進化・発生・メカニカルストレスから探る顎顔面形成・維持機構最先端」でシンポジストとして発表した。

    It remains unclear whether the AhR signaling affects the TMJ. The aim of this study was to investigate possible mechanisms of bone loss in the TMJ due to the epigenetic alteration such as smoking. In particular, whether benzo[a]pyrene (B[a]P) induces expression of Cyp1a1 in the mandible. Possible functions of an endogenous ligand FICZ were investigated in a TMJ-OA mouse model. B[a]P was administered orally to mice and bone metabolism was examined. TMJ-OA was induced in WT mice with forceful opening of the mouth, and therapeutic functions of FICZ were detected. Exposure to B[a]P accelerated bone loss in the mandible. This bone loss manifested with osteoclastic bone resorption. In a TMJ-OA model, FICZ exhibited a dose-dependent rescue of subchondral bone loss by repressing osteoclast activity. Pretreatment with FICZ reduced RANKL-mediated osteoclastogenesis. B[a]P regulates subchondral bone metabolism via the Cyp1a1. FICZ can prevent TMJ-OA by regulating osteoclast differentiation.

    In this study, we conducted experiments to clarify the effects of the trigeminal nerve and facial nerve on the morphology of the maxillofacial region during growth. As a result of nerve activity blocking experiments and nerve activity activation experiments of the trigeminal nerve and facial nerve of rats, a decrease in the formation rate of cut teeth, calcification of the pulp, and disorder of the arrangement of ameloblasts and odontoblasts were observed. It was. In addition, as a result of analyzing Sonic hedgehog (Shh) gene-deficient mice and Shh gene-expressing mice, changes in bone and tooth morphology that are thought to be the effect of Shh on long-term maxillofacial morphogenesis were observed. As a result of conducting an experiment using fluorescence bioimaging, no characteristic stem cell dynamics were observed with or without nerve activity blocking / activation.

    Recent studies have focused on the relationship between masticatory function such as MOF and body function such handgrip strength (HS). On the other hand, maximum occlusal force (MOF) is associated with maxillofacial morphology, such as high mandibular plane (Mp) angle, and with malocclusion such as open bite, posterior crossbite and scissor bite. However, the relationships between maxillofacial morphology/malocclusion and HS remain unclear. Furthermore, there is no one investigating simultaneously the relation between maxillofacial morphology, malocclusion, MOF, and HS. In this study, we found that the subjects with a large gonial angle and a small overbite or a crossbite/scissor bite in the molar section had low HS, which is related with MOF.

    Vagal afferent stimulation significantly increased salivation and swallowing-like events in a frequency-dependent manner. A muscle relaxant, vecuronium bromide, diminished the swallowing-like response but did not affect salivation. Moreover, the tissue blood flow rate of the body surface increased according to the frequency of the vagus nerve stimulation. These results indicate that visceral sensation induces salivation and swallowing-like events in anesthetized rodents through vagal afferent activation.
    These findings might contribute a new application of vagus nerve stimulation as to enhancing oral function.

    In order to elucidate the relationship between stress and the stomatognathic function, this study investigated the effect of the stomatognathic function caused by modulating the hypothalamus - pituitary - adrenal axis (HPA axis). As a result, the activity of the masticatory muscle electromyogram remarkably decreased from several hours immediately after administering a stress factor, adrenocorticotropin releasing factor (CRF: 1.0 nM (10 μL)) into the lateral cerebral ventricle during the light period. After this response, EMG activity showed a tendency of strong hyperactivity on the contrary. And it showed that the total EMG activity for 24 hours after administration increased relative to control group administrated the artificial cerebrospinal fluid. It also suggested that the administration of CRF into the lateral cerebral ventricle could have affected the sleep-wake rhythm.

    Clinical research: we evaluated the relationship between a stomatognathic function, which closely relate with gastrointestinal function, and a malocclusion. In the patients with scissors-bite, the chewing pattern and activities of both the temporalis and the masseter muscles were significantly narrower and lower, respectively. In the patients with molar cross-bite, the reverse chewing pattern during deliberate unilateral mastication was significantly more frequently observed. Those results suggest that gastrointestinal function in patients with scissors-bite or cross-bite malocclusion may be low.
    Basic research: we focused on the vagal nerve dominating the visceral sensation. As a result of cervical vagal stimulation, it was observed the rhythmic muscle activity of mylohyoid which means swallowing with salivary secretion. From this study, it was suggested that visceral sensation is involved in activation of stomatognathic functions including salivation and swallowing.

    Gastroesophageal Reflux (GER) is a disease featured by reflux of stomach contents into the esophagus, causing chest pain and nausea by stimulation to the esophagus. Its therapy is mainly performed by acid reducer. We focused on the vagal afferent pathway mediating visceral sensation to brain. First, we investigated whether experimental stimulation affect oral function involving salivation. Next, we investigated whether acid reducer affect salivation. The present study suggested that vagal afferent stimulation evoked salivation, but also a kind of acid reducer increased salivation evoked by vagal afferent stimulation. Therefore, it was suggested that visceral sensation related to oral function involving salivation.

    This study focused on the relationship of stress and the stomatognathic function. As peripheral stimulation, we performed the acid stimulation to upper part gastrointestinal tract and, as central stimulation, we used corticotropin release factor where related stress. Recording procedure was mainly electromyogram (EMG: masseter muscle activity), electroencephalogram (EEG), and electrocardiogram (ECG). The result is that EMG increased after peripheral and central stimulation in comparison with before stimulation. In addition, quality of the sleep decreased, and it was suggested that the stimulation influenced the autonomic nervous system. On the other hand, in the gene expression of the stimulation on the upper gastrointestinal tract, ASIC3 and TRPV1 gained expression. It was suggested that peripheral stimulation was transmitted through the sense of pain related tract.
    Therefore, stress has close to the relations with the stomatognathic dysfunction.

    In the present study, we hypothesized that there is a strong correlation between stmatognathic function and the motility function of the stomach and esophagus. (1) Initial gastric emptying is suppressed by mastication, and that the suppression is caused by mastication-induced inhibition of gastric activity. (2) Esophageal-derived autonomic nervous system modulation induces masseter muscle activity, irrespective of the presence of subjective gastrointestinal symptoms. (3) By administering proton pump inhibitor to the patients with sleep bruxism, masseter muscle activity during sleep significantly decreased. (4) Involuntary teeth clenching is related to regional brain activity in the insula and anterior cingulate cortex. These studies suggest that the function and sensation of the stomach and esophagus may be closely related with a stmatognathic function via brain activity.

    本年度は、小中学生を対象に、以下の調査。解析を行った。概要は次の通り。研究背景:小・中学生における問題行動が学校および社会における大きな問題と言われている。一方、小・中学生の睡眠に関する様々な問題も、学校生活や日常生活に影響を及ぼすとされ、問題視されてきている。睡眠障害を引き起こす原因のひとつに睡眠時の微小覚醒を生じさせる胃食道逆流症(GERD)があると言われている。本研究では、問題行動に関し、睡眠の動態とGERDの症状がどのように関連しているのかを検討するため、小・中学生を対象に問題行動と睡眠動態およびGERD症状を調査・解析したので報告する。対象は鹿児島県内在住の小・中学校の生徒30名(男子16名、女子14名、年齢10歳‐15歳)とその保護者である。方法:生徒の基本的属性と生活および睡眠に関する質問項目について、記入式で回答を得た。問題行動についてはPediatric Symptoms Checklist(PSC、日本語版)を用いて、GERD症状についてはFrequency Scale for the Symptoms of GERD(FSSG)を用いて回答を得た。睡眠動態についてはActiwatch2(米国フィリップスレシプロニクス社)を用いて、一夜の睡眠時間、入眠潜時、REM睡眠時間、non-REM睡眠時間、REM睡眠時間/睡眠時間、中途覚醒を記録し、解析した。PSCを基準に問題行動群と正常行動群に分け、生活および睡眠に関する事項、GERD症状ならびに睡眠動態を比較した。
    結果:小・中学生では、全体の15%に問題行動が見られた。問題行動を呈する者では食事習慣と睡眠に特徴がみられ、GERD症状が多く、睡眠に関して睡眠中の動態に乱れが見られた。問題行動とGERD症状、睡眠の乱れには関連があることが示唆された。以上の解析結果を英文にまとめ、現在、国際誌に投稿中である。

    We ivestigated the sleep disturbances, problem behavior, and symptoms of the gastroesophageal reflux of primary school children and junior high school children(total : 6087, boys : 3017, girls : 3070, age : 6-15 years old). In the problem behavior group, the symptoms of GERD and sleep disturbances were significantly more observed. Also, skipping breakfast, taking supper with part of their family, and shorter conversation at or after the supper and bruxism and clenching during sleep were significantly more observed than in the normal behavior group. It was suggested that the problem behavior has associations with dietary habit, sleep bruxism and clenching, and GERD symptoms.

    In the present study, we hypothesized that good occlusion increases the acid clearance in the oral cavity, pharynx and esophagus, leading to the prohibition of the gastroesophageal reflux disease(GERD).
    (1) The results of the investigation of masticatory functions in patients with skeletal Class III malocclusion suggested that GERD symptoms were observed more often than in normal subjects.
    (2) The results of the investigation of the effects of intra-esophageal infusions of 5-mL acidic solution(0.1 N HCl) or saline to the healthy adults suggested that sleep bruxism episodes were induced by esophageal acidification.
    (3) Bruxism patients(n=7) had high frequency of GERD with a mild esophagitis.
    These researches show good occlusion has associations with esophageal acidification, suppression of upper gastrointestinal disease.

    唾液は脳幹(下位中枢)を介し反射性に分泌されるだけでなく、前脳(上位中枢)からの入力によっても制御されている。しかし、その制御機構は基礎および臨床研究においても、未だ具体的に明らかにされていない。本研究課題では、免疫組織学および電気生理学的手法を用い、唾液分泌に対する上位および下位中枢からの抑制性制御機構について調ベた。
    その結果、全ての上唾液核細胞は上位中枢から抑制性入力を受けていることが明らかとなった。このことから、唾液分泌は興奮性入力のみならず、上位中枢からの下行性抑制性入力によっても調節されていることが示唆された。
