    BACKGROUND: The efficacy and safety of 42 °C hot saline irrigation in haemostasis remain unclear. AIMS/OBJECTIVES: This study aimed to investigate the effects of hot saline irrigation at 42 °C during endoscopic sinus surgery (ESS) to assess its efficacy in reducing intraoperative bleeding. MATERIAL AND METHODS: In this randomized, double-blind, within-person trial, 13 patients with bilateral chronic rhinosinusitis with nasal polyps undergoing ESS were randomly assigned to receive 42 °C hot saline irrigation on one side and room-temperature saline irrigation as a control on the contralateral side. The primary outcome was the Boezaart surgical field score, while secondary outcomes included operative time, histopathological changes, and postoperative complications. The Wilcoxon signed-rank test was used for statistical analyses. RESULTS: The Boezaart surgical field score significantly improved with hot saline irrigation compared with the control during anterior ethmoid surgery (p = 0.046); however, no significant difference was observed during posterior ethmoid and sphenoid surgeries. Operative times were comparable between groups. No adverse tissue changes or postoperative complications were observed. CONCLUSIONS AND SIGNIFICANCE: Hot saline irrigation at 42 °C improved surgical field visibility during ESS without adverse effects, demonstrating its potential as a haemostatic adjunct. This cost-effective and safe technique warrants further investigation for broader clinical application. TRIAL REGISTRATION: UMIN Clinical Trials Registry: UMIN000052864.

    This study investigated the effects of intranasal administration of a semaphorin 3A inhibitor (Sema3A-I) in a mouse model of olfactory disorder, where olfactory sensory neuron (OSN) axons had been severely damaged. We performed axotomy (transection of OSN axons) of the OSNs in mice and administered Sema3A‑I intranasally to seven mice and saline to another seven mice. Following treatment, we assessed the thickness of the olfactory epithelium and the regeneration ratio of OSN axons. Intranasal administration of Sema3A-I did not significantly promote OSN regeneration, axonal outgrowth, or improve axonal projection compared to saline administration. Although Sema3A-I administration showed some promotion of axonal outgrowth, the difference was not statistically significant. Continuous subcutaneous administration of Sema3A-I in rats after axotomy promotes OSN regeneration and axonal outgrowth. Given that intranasal administration is minimally invasive, we believe that it may still be a feasible route when combined with additional treatment strategies. Further investigation into administration methods and therapeutic combinations is warranted.

    A 76-year-old man presented with right eyelid swelling and deteriorated vision. Examination revealed anterior uveitis with hypopyon and a visual acuity of 20/2,000 in the right eye, with no abnormalities in the left. Computed tomography revealed enlargement of the right maxillary sinus and internal fluid accumulation, suggesting a postoperative maxillary cyst (POMC). Nasal endoscopic surgery drained the pus by opening the lower wall of the maxillary cyst. Following the procedure, intraocular inflammation resolved, and visual acuity in the right eye improved to 24/20. This is the first reported case of uveitis secondary to POMC.

  • Endoscopic surgical strategy for highly vascular pediatric sinonasal and pterygopalatine fossa tumors. International journal

    Pediatric sinonasal tumors are rare, and minimally invasive approaches are preferred because of unknown long-term effects on development. We report three cases of highly vascular pediatric sinonasal and pterygopalatine fossa tumors that were successfully removed using an endonasal endoscopic approach with transcatheter arterial embolization (TAE). The patients (9-11 years old) had contrast-enhanced tumors in the left ethmoid sinus near the sphenopalatine foramen in case 1, in the right nasal cavity near the sphenopalatine foramen in case 2, and in the right pterygopalatine fossa in case 3, with diameters of 32, 50, and 61 mm, respectively. Computed tomography and magnetic resonance imaging demonstrated tumors with strong contrast enhancement and flow voids in all cases. TAE was performed in all cases on the day of surgery, followed by endoscopic sinus surgery with nasal septal modification. Pathology revealed pyogenic granuloma, nasal polyps, and juvenile angiofibroma. After TAE, tumor reduction improved visualization in case 1. Nasal septum manipulation enabled medial traction and tumor base identification in cases 2 and 3. The combination of preoperative TAE and septal modification proved effective for achieving complete endoscopic tumor removal. No complications occurred, and follow-up at 1-1.5 years showed no tumor recurrence or developmental abnormalities.

    BACKGROUND: Advanced surgical interventions are required to treat malignancies in the anterior skull base (ASB). This study investigates the utility of endoscopic endonasal and transcranial surgery (EETS) using a high-definition three-dimensional exoscope as an alternative to traditional microscopy. METHODS: Six patients with carcinomas of varying histopathologies underwent surgery employing the EETS maneuver, which synchronized three distinct surgical modalities: harvesting of the anterolateral thigh flap, initiation of the transnasal technique, and initiation of the transcranial procedure. RESULTS: The innovative strategy enabled successful tumor resection and skull base reconstruction without postoperative local neoplastic recurrence, cerebrospinal fluid leakage, or neurological deficits. CONCLUSION: The integration of the exoscope and EETS is a novel therapeutic approach for ASB malignancies. This strategy demonstrates the potential of the exoscope in augmenting surgical visualization, enhancing ergonomics, and achieving seamless alignment of multiple surgical interventions. This technique represents a progressive shift in the management of these complex oncological challenges.

    Objectives This retrospective study aimed to investigate the relationships between the Keros classification, the Gera classification, the vertical height of the posterior ethmoid roof (ER), and anterior ethmoidal artery (AEA) types in Japanese patients. Methods We investigated the computed tomography (CT) slices of paranasal sinuses (120 sides) of 60 patients; measured the cribriform plate (CP) depth, lateral lamella CP angle (LLCPA), and vertical height of the lateral ER from the hard palate (LERHP) at the coronal plane of the posterior ethmoidal artery (PEA); and reviewed the AEA types, whether floating or non-floating. Results CP depth was positively correlated with LLCPA (r=0.63; p<0.01) and the height of LERHP (r=0.19; p<0.05). The height of the LERHP in females was significantly lower than that in males. With increased CP depth, floating AEAs became prevalent (p<0.001). Conclusion In females, low height of the posterior ethmoid sinus roof, where cerebrospinal fluid (CSF) leaks occurred while penetrating the basal lamella, often existed; the heights positively correlated with the Keros classification in Japanese patients. The Keros and Gera classifications, AEA type, and posterior ER height do not individually constitute a complete risk assessment but may correlate, preventing major complications, such as CSF leak and orbital hemorrhage.

    OBJECTIVE: Combination intranasal drugs with a corticosteroid and antihistamine are available in several countries with better effect than treatments with single agents. However, it remains unclear whether this effect is also seen in Japanese cedar pollinosis (JCP), the most prevalent seasonal allergic rhinitis in Japan. We investigated the effect of an add-on intranasal antihistamine with an intranasal corticosteroid in JCP during the pollen dispersal period. (UMIN000025508) METHODS: We performed a double-blinded, randomized, placebo-controlled trial from March 1 to 14, 2017. Patients (n = 20 per group) received either a mometasone furoate nasal spray (MFNS) plus a levocabastine nasal spray (levocabastine group) or MFNS plus a placebo nasal spray (placebo group). The primary endpoint was the difference in the total nasal symptom score (TNSS) after treatment between the two groups. Differences in the total ocular symptom score, total symptom score, total medication score, total symptom-medication score, and five individual symptoms as well as safety were the secondary endpoints. RESULTS: The change in the TNSS from baseline was significantly greater in the levocabastine group than in the placebo group. A significant reduction in the TNSS was observed more than 6 days earlier in the levocabastine group than in the placebo group. Such add-on effects were also seen in the secondary endpoints. Both treatments were well-tolerated. CONCLUSION: The intranasal antihistamine provided better control of not only nasal symptoms, but also of ocular symptoms, and decreased the need for rescue medications when added to intranasal corticosteroid treatment in JCP patients.

    <p>症例は65歳男性。糖尿病性腎症による末期腎不全に対する腎移植術前CTにて右蝶形骨洞に真菌を疑う軟部影を認めたが、無症候性であった。しかしながら、移植後の免疫抑制状態で劇症型になる可能性を考慮し、移植3カ月前に内視鏡下副鼻腔手術ESSにて糸状菌塊を除去(粘膜への真菌浸潤なし)、追加治療することなく経過は安定していた。 生体腎移植については妻をドナーとしたABO不適合(術前減感作療法としてRituximab 200mg/body×1、DFPP×2、PE×1を実施、免疫抑制剤はTac-ER、MMF、PSL、Basiliximabを使用)、経過は良好でPOD18に退院となった( Cr 1.28mg/dl )。 移植後2カ月が経過し、右顔面痛・頭痛が出現、近医で副鼻腔洗浄や抗菌薬投与を受けるも改善せず。β-Dグルカン 53pg/mlと異常高値、副鼻腔MRIでは一部骨への浸潤を疑う軟部影を認め、再度ESSを施行、紙様板は腐骨化を認めたが真菌塊を可及的に除去した。VRCZも併用・継続し、術後1カ月でβ-Dグルカンも正常化、現在、再燃を認めていない。 浸潤性副鼻腔真菌症は副鼻腔から眼窩,海綿状静脈洞,頭蓋内へ浸潤し致死的となり、全生存率は概ね50%と、予後不良とされる。今回、予防的にESSを先行したが、腎移植後、急性浸潤性に再燃した症例を経験したので、若干の文献的考察を加えて報告する。 </p>

    The glomerular map in the olfactory bulb (OB) is the basis for odor recognition. Once established during development, the glomerular map is stably maintained throughout the life of an animal despite the continuous turnover of olfactory sensory neurons (OSNs). However, traumatic damage to OSN axons in the adult often leads to dysosmia, a qualitative and quantitative change in olfaction in humans. A mouse model of dysosmia has previously indicated that there is an altered glomerular map in the OB after the OSN axon injury; however, the underlying mechanisms that cause the map distortion remain unknown. In this study, we examined how the glomerular map is disturbed and how the odor information processing in the OB is affected in the dysosmia model mice. We found that the anterior-posterior coarse targeting of OSN axons is disrupted after OSN axon injury, while the local axon sorting mechanisms remained. We also found that the connectivity of mitral/tufted cell dendrites is reduced after injury, leading to attenuated odor responses in mitral/tufted cells. These results suggest that existing OSN axons are an essential scaffold for maintaining the integrity of the olfactory circuit, both OSN axons and mitral/tufted cell dendrites, in the adult.

    BACKGROUND: Statistically significant results of medical intervention trials are not always clinically meaningful. We sought to estimate the minimal clinically important difference (MCID) (the smallest change in a given endpoint that is meaningful to a patient) during seasonal alteration of Japanese cedar/cypress pollinosis (JCCP). METHODS: Results of a double-blinded, placebo-controlled trial of JCCP patients conducted between 2008 and 2010 were analyzed using an anchor-based method in which a face scale for Japanese rhinoconjunctivitis quality-of-life questionnaire (JRQLQ) was set as an anchor. MICDs were calculated as changes of average scores, including those for naso-ocular symptoms with 5 items in diary cards (T5SS), naso-ocular symptoms with 6 items (T6SS) and QOL with 17 items on the JRQLQ when face scale scores either improved or deteriorated by one point. RESULTS: In 2009 and 2010, 3,698 and 374, respectively, grains/cm(2) of pollens were dispersed. The MCIDs for T5SS in 2009 and 2010 were 1.426 (0.285 per item) and 1.441 (0.288), respectively. The MCIDs for T6SS were 4.115 (0.686) and 3.183 (0.531) in 2009 and 2010, respectively. The MCIDs for QOL were 10.469 (0.616) and 6.026 (0.354) in 2009 and 2010, respectively. CONCLUSIONS: For T5SS in the diary, T6SS and QOL in JRQLQ, unit differences of 1.5 (0.3 per item), 3.6 (0.6) and 8.2 (0.5), respectively, were considered clinically meaningful by JCCP patients. The MCID for symptoms recorded in the diary was stable irrespective of the dispersed pollen level.

    Facial nerve edema is an important finding in Bell's palsy patients. Inflammation may cause facial nerve edema, and mechanical compression and ischemic change of the facial nerve may occur in the facial nerve canal. A few studies have reported the dimensions of the facial nerve canal using conventional computed tomography or human temporal bone sections. However, the cross-sectional area of the facial nerve canal has not been fully understood. Therefore, the cross-sectional area of the facial nerve canal was measured in patients with unilateral Bell's palsy by computer tomography with multiplanar reconstruction. Sixteen patients with unilateral Bell's palsy were enrolled. Computed tomography of the temporal bone was performed, and perpendicular images to the facial nerve canal were reconstructed by the multiplanar reconstruction technique. The cross-sectional area of the facial nerve canal on the affected and unaffected sides was measured at the labyrinthine segment, the horizontal segment, and the mastoid segment. Both in the labyrinthine and horizontal segments, the mean cross-sectional area of the facial nerve canal was significantly smaller on the affected side than on the unaffected side. There was no significant difference between the affected and unaffected sides in the cross-sectional area of the facial nerve canal in the mastoid segment. The labyrinthine segment was the narrowest segment in the facial nerve canal. These findings suggest that the facial nerve is vulnerable, especially in the labyrinthine segment of the facial nerve canal, and a narrow facial nerve canal may be one of the risk factors for Bell's palsy.

    嗅覚受容体MOR29AとMOR29Bに蛍光標識した遺伝子組み換えマウス(MOR29ABマウス)に対し、嗅細胞の軸索を物理的に切断(軸索切断)を行い嗅覚障害モデルマウスを作成した。投射異常のおきた嗅細胞軸索は時間経過とともに軸索伸長の距離を伸ばし、本来の投射部位に近づいていく。そこで臨床的に効果の認められている嗅覚刺激療法をマウスに施行し、嗅細胞の回復を促進することが考えられた。しかしながら発生期に確立した嗅球の投射地図上のあるべき投射位置に戻るのは完全には困難である。そこで今軸索伸長を阻害する神経忌避因子Semaphorin3Aに着目した。Semaphorin3Aは発生期に受容体であるNeuropirin1と共同し、嗅細胞軸索の投射地図を形成するのに重要な役割を果たすが、発生期の終了とともにSemaphorin3Aは消失する。しかし、外傷をはじめとする炎症が引き起こされるとSemaphorin3Aは再度放出され、神経再生を阻害する。Semaphorin3A阻害薬を用いて、再生を阻害する炎症因子を取り除き、嗅覚再生にどのような影響を及ぼすか検討を行った。軸索切断後にSemaphorin3A阻害薬を3週間点鼻投与し、嗅細胞の再生、軸索伸長の過程を検討した。生理食塩水を同期間点鼻した対照群と比較して、嗅上皮の厚みは変わらなかった。Semaphorin3A阻害薬は軸索伸長を促進するが嗅細胞のターンオーバー促進にはつながらなかった。今後軸索投射の変化を経時的に検討を行う。

    We have been studying sensorineural hearing loss for many years, but correct olfactory function requires the restoration of olfactory maps in the olfactory bulb by regenerated olfactory sensory neurons (OSNs). Our previous report showed that after severe damage to OSN, such as axotomy, the olfactory map was distorted and synaptic connections between OSNs and secondary neurons were reduced. To normalize the olfactory map, we attempted sensory training in mice after axotomy. The sensory training group sniffed five odors, and the control mouse sniffed saline. The thickness of the olfactory epithelium makes a significant difference between the olfactory training group and the control group. We believed that olfactory training aids in the regeneration of OSN after axotomy, and that promoting OSN regeneration can normalize the distorted olfactory map of the olfactory bulb.

    We research olfactory disorder and therapy using the mouse injured the olfactory nerve axon. The normal smell sense is required correct olfactory nerve network. But,the olfactory sensory neurons(OSN) damaged severely, the olfactory nerve formed incorrect network even the olfactory sensory neurons recover. We tried the several research, for example, the axons of OSN maintain for guidance of the new axon and administration of axon guidance. But these researches are not going well. So, we observe change over time of OSN axons in the olfactory bulb, the regenerated network is not perfect. But the OSN axons 19 weeks after injury are more promoted than 6 weeks.

    To investigate the effectiveness of intranasal drug administration, the numbers of mitosis in the subventricular region where neural stem cells are present and of new neurons in the olfactory bulb where new neurons born in the subventricular region distribute through the rostral migration pathway were evaluated immunologically using Ki 67 (a marker of cell division) and DCX (a marker of new neurons), respectively.
    The nasal administration of prolactin or GDF (growth differentiation factor) 11 showed significant increases in cell division in the subventricular region and in neonatal neurons in the olfactory bulb by statistical analysis, respectively. However, as a transfer route of the nasal administration drug, the active evidence of direct transfer from the nasal cavity to brain tissue was not obtained.
    To elucidate brain tissue migration routes of intranasal medicine, further research such as analysis of the blood concentration of drug administered nasally should be necessary.
