    It is reported that immunostaining of Myoglobin (Mb) is useful for forensic diagnosis. In this study, we investigated the condition of fixation of striated muscle in 10 % neutral-buffered formalin to obtain appropriate stationarity of Mb in immunostaining. Firstly, criteria for staining intensity and definition of the stainability of examined were determined for sheep muscle tissue. Sheep myocardial tissue was fixed using 10 % neutral-buffered formalin under the 21 different conditions based on combinations of the following: three ratios of volume of formalin (mL) to weight of myocardium (g) (RFM) of 1, 4 or 9, 7 durations of fixation (DF) of 0.5, 3 or 6 h, and 1, 2, 5 or 7 days. Secondly, detection of Mb diffused form skeletal muscle from autopsy cases into formalin during fixation were confirmed by ELISA. Finally, the evaluation of stainability of Mb of striated muscle in routine autopsy examinations was confirmed using sheep staining intensity standards. From this experimental investigation, it has been demonstrated that the most suitable formalin fixation condition for using Mb staining in forensic diagnosis is RFM4 with a fixation time of at least DF 3 h up to 1 day. It was evident that staining intensity decreases with fixation durations exceeding 2 days, irrespective of the RFM. Thus, the fixation time was deemed the most influential factor affecting the staining properties of Mb staining in skeletal muscle tissue. When conducting Mb staining using striated muscle as an evaluation sample, particular attention should be paid to the fixation time.

    The mouse Harderian gland (HG) is a secretory gland that covers the posterior portion of the eyeball, opening at the base of the nictitating membrane. The HG serves to protect the eye surface from infection with its secretions. Mice open their eyelids at about 2 weeks of age, and the development of the HG primordium mechanically opens the eye by pushing the eyeball from its rear. Therefore, when HG formation is disturbed, the eye exhibits enophthalmos (the slit-eye phenotype), and a line of Fgf10+/- heterozygous loss-of-function mice exhibits slit-eye due to the HG atrophy. However, it has not been clarified how and when HGs degenerate and atrophy in Fgf10+/- mice. In this study, we observed the HGs in embryonic (E13.5 to E19), postnatal (P0.5 to P18) and 74-week-old Fgf10+/- mice. We found that more than half of the Fgf10+/- mice had markedly degenerated HGs, often unilaterally. The degenerated HG tissue had a melanized appearance and was replaced by connective tissue, which was observed by P10. The development of HGs was delayed or disrupted in the similar proportion of Fgf10+/- embryos, as revealed via histology and the loss of HG-marker expression. In situ hybridization showed Fgf10 expression was observed in the Harderian mesenchyme in wild-type as well as in the HG-lacking heterozygote at E19. These results show that the Fgf10 haploinsufficiency causes delayed or defective HG development, often unilaterally from the unexpectedly early neonatal period.

    法医剖検例における尿中ミオグロビン濃度の測定は,急性心筋梗塞などの診断をするうえで意義があるとされているが,ミオグロビンは尿中で不安定な物質であり,測定に際しては専用容器での保存が望ましい.しかしながら,この専用容器は尿量4-8mlを必要とするため,尿中ミオグロビン濃度測定が必要な場合に必ずしもこれを使用できるとは限らない.そこで,この専用容器と,保存剤を含まないポリエチレン製試験管とにそれぞれ剖検で得られた尿を保存し,ミオグロビン濃度測定値の相関を調べた.剖検例34例において得られた尿中ミオグロビン濃度の平均値および中央値は,専用容器では392.4(±584.1)ng/mLおよび101.5ng/mL,ポリエチレン製試験管では,197.2(±303.9)ng/mLおよび60.5ng/mLとなり,ポリエチレン製試験管に保存した場合には有意に測定値が低下する(p<0.05)という結果となったが,両者の間にはy=1.7796x+41.46(R2:0.8573)の回帰式が算出された.剖検時に得られた尿について,保存剤の含まれないポリエチレン製試験管に保存しても,この回帰式を用いることで本来の尿中ミオグロビン濃度を推定することが可能であると思われた.剖検時に尿量が少ない場合など,専用容器を用いることができない場合であっても,尿中ミオグロビン濃度の測定を行う意義はあると考えられる.(著者抄録)

    As more families are acquiring pets and the opportunities for wild animals to appear in human neighborhoods are increasing, the number of cases and accidents involving animals is increasing. Hence, the need to identify animal species from blood left over at accident sites is increasing. Human hemoglobin is used as a marker for human blood. Although tandem mass spectrometry is the dominant methodology used in proteomics research, peptide mass fingerprinting, given its instant applicability, may be useful for screening animal species, as the amino acid sequences of hemoglobin from various animals differ. In this study, solutions that were easily purified─using hemoglobin reagents from humans, Japanese macaques, bears, cattle, goats, sheep, sika deer, pigs, wild boars, dogs, cats, and nutrias─were digested by trypsin, and subjected to database searched using Mascot. No candidate proteins were found in the blood of goats, sheep, sika deer, wild boars, pigs, or nutrias. However, bloodstains from all animal species except nutria (which is not registered in the database) yielded candidates, which were identified as the hemoglobin of origin or its relatives. This difference may be attributed to more contaminants being included in blood. Further narrowing was possible using the average mass obtained via infusion electrospray ionization mass spectrometry measurement of the undigested solution in Mascot results. Saliva, urine, semen, and sweat collected from humans were also examined and searched for mascots, but no hits were obtained. In conclusion, this method may be useful for estimating animal species and identifying blood in forensic science.

    Massive methanol exposure can lead to severe and detrimental effects that can result in death or brain death. As organs from patients with brain death after methanol ingestion are less likely to be recovered, these patients have been considered marginal donors. We present a case of successful multiple organ transplantation (heart, lungs, and kidneys) from a methanol-poisoned patient. Our experience illustrates that donor death from methanol intoxication does not preclude organ transplantation.

    Myosin light chain (MLC)の検出による骨格筋の同定法では、当初はマイクロプレートを用いたダブルサンドイッチ法によるin-house developed ELISAとする計画であったが、ヒト心室筋MLC1を検出する臨床検査用市販試薬(キット)の応用に切り替えた。本年度の検討では、骨格筋はその部位に拘わらず十分量のMLC1を含有していることが認められた。なおこの試薬ではヒト骨格筋MLC1との交差が明らかにされており、肉眼で骨格筋を疑いうると考えられる大きさ(重量で0.01g程度)の組織片からのMLC1の定量的検出は理論計算上は可能である。また法医学実務への市販試薬の応用は、機関や人員の制限がなくなるのが利点である。
    Tear specific protein (tear prealbumin, lipocalin 1, LCN1)に着目した涙液の鑑別法では、上述のような臨床検査試薬はなく、当初計画通りマイクロプレートを用いたダブルサンドイッチELISAを採用しつつ、検査法の汎用性の観点から、ラテックス凝集法で実用に耐える検出限界を得られるかも検討対象とした。市販の標準抗原と抗体から、検査法として確立できそうな反応性のある組み合わせを検討した。また、涙液は鼻涙管経由で鼻孔から体外に排出され得ることから、鼻汁との鑑別は特に重要になる可能性に配慮し、鼻汁特異的蛋白Odorant-binding proteinに着目した鼻汁鑑別法も同時に確立する方針として、市販の標準抗原と抗体との反応性を検討した。

    Our analytical findings on drugs in forensic autopsy cases demonstrated that: (1) basic drugs such as methamphetamine and ephedrine that are excreted into urine as unmetabolized form into urine at a large proportion appear at a higher concentration in urine than in the stomach when administered intravenously, and basic drugs that are mostly excreted into urine as metabolized form appear at a higher concentration in the stomach than in urine after intravenous administration; and (2) metabolites of a basic drug which increase gradually in blood are rapidly excreted into the stomach if they preserve basic character.
    An animal experiment using rabbits demonstrated that flecainide, a basic drug, is rapidly excreted into the stomach when it is intravenously administered. We established an animal model using rats for investigating excretion patterns of methamphetamine into the stomach.

    Unknown factors or diseases that were not recognized to cause sudden infant death were investigated. It was thought that infection of Cronobacter sakazaki as cause of sudden death could be simply diagnosed only by the growth of that pathogen in the culture. Fulminant type Ⅰ diabetes could also be diagnosed by the extreme low level of blood 1,5-AG concentration, whereas evaluation of stress as the factor of sudden death should be examined further using saliva chromogranin A concentration.
    According to the investigation of the ratios of area of the left ventricle to those of the right ventricle and the total left heart in the heart cross-section, it was suggested that the the left heart dysfunction could be another possible unknown factor/cause of infantile sudden death.

    This study is aimed at implementing the PDCA cycle (process improvement technique consisting of 4 steps:Plan,Do,Check, and Act) which has two main components, medical, long term care, and welfare care fieldsites and the university. We have planned to cooperate with the medical, long term care, welfare fieldsites by examining together the Tsukuba-city medical welfare case examination meetings, and providing feedback to Tsukuba City by analyzing their needs survey. We performed a scientific study using several types of data analysis, including big data from national long-term care benefit expenditures data and from the Comprehensive Survey of Living Conditions. We examined the regional comprehensive care promotion policy , including these findings, to clarify how to advance the cooperation between medical and long-term care, what kind of support require the persons in need of care, children with disabilities and the use situation of nursing care insurance services.
