    BACKGROUND: The novel coronavirus disease 2019 (COVID-19) pandemic restricted nursing practicums, which limited students' clinical experience. In many programs, hands-on training was replaced with simulations, potentially affecting new nurses' practical skills and decision-making. Consequently, preceptors guiding inexperienced nurses faced increased instructional burdens and challenges in ensuring safe and competent practice. OBJECTIVES: This study aimed to examine how disruptions to clinical practicums due to the COVID-19 pandemic impacted the role performance of preceptors supporting newly graduated nurses in 2022 over time. METHODS: A longitudinal survey involving 426 preceptors from 39 hospitals in Hyogo Prefecture, Japan, was conducted three times in 2022 (June, September, and December). The main outcome assessed was preceptor role performance, which was measured using the 35-item Preceptor Role Self-Evaluation Scale. Secondary outcomes included participant demographics and perceived difficulties in guiding new nurses under COVID-19-related practicum restrictions, and they were assessed on a 5-point scale. Ethical and institutional approval was obtained, and the surveys were distributed online via QR codes and email. Participation was voluntary and anonymous, with responses linked over time using the participants' email addresses. RESULTS AND CONCLUSION: Seventy-six preceptors (six males and 70 females; 17.8% response rate) completed all three surveys. In June, those reporting higher instructional difficulty demonstrated significantly higher role performance, particularly in planning, evaluation, accident prevention, and cooperation between ward staff and patients. By September and December, their ability to cooperate was significantly better. These findings indicate that preceptors who perceived greater difficulty initially improved their performance, which highlights the heightened instructional demands caused by COVID-19 disruptions to clinical training.

    Objective

    This study aimed to longitudinally examine how the cancellation or modification of the clinical practicum due to COVID-19 affected the nursing practice competence of newly graduated nurses in Japan.

    Methods

    A survey was conducted across three periods—June, September, and December 2022—to seek responses from participants. A total of 73 participants (two men and 71 women) who responded in all periods were included in the analysis. The survey included questions on the impact of the integrated and specific clinical practicums, including a self-assessment scale for nursing problem-solving behaviors.

    Results

    During all response periods, those whose practicums were entirely clinical scored significantly higher in nursing problem-solving behaviors. Furthermore, by December, “psychological support to patients for overcoming challenges” improved alongside the clinical experience of newly graduated nurses. However, their practical skills related to aggregating information, identifying improvements in nursing problems, facilitating smooth patient interactions, and individualizing assistance were challenging to improve along with clinical experience.

    Conclusion

    This study revealed that canceling or modifying the clinical practicum owing to COVID-19 impacted the problem-solving behaviors of newly graduated nurses.

    兵庫大学の現代ビジネス学科、健康システム学科、栄養マネジメント学科、看護学科、社会福祉学科、こども福祉学科の3年生、保育科2年生の各学科5名の計35名を対象に、演習で行ったシミュレーションを用いた多職種連携教育の効果を検証した。演習に出席していた学生に、介入前と介入後のマッチングを行い、2回ともに回答が得られた32名を分析対象とした。多職種連携演習の目標に関する評価では、全ての目標で有意な向上が確認できた。地域貢献に関する項目では、「今回のメンバーと将来働きたいと思う」「多職種連携が必要になった時に積極的に行動ができますか」「自分が地域包括ケアが関係あると思いますか」などに有意な向上が見られた。多職種連携教育の効果を測定するRIPLSでは「チームワークとコラボレーション」「専門性」「RIPLS合計」が有意に上昇し、社会的スキルの尺度であるKiss-18も有意に得点が上昇した。

    本研究では, 病棟に勤務する看護師(男性101人, 女性101人)を対象に, 生物学的性差とジェンダー・タイプのどちらがストレッサーとバーンアウトの知覚に影響するかを検討することを目的に, BSRI (Bem Sex Role Inventory), NJSS (Nursing Job Stressor Scale), 日本版MBI (Maslach Burnout Inventory)を用いた質問紙調査を行った。その結果, 性差とジェンダー・タイプはそれぞれ独立しており, 性別による特徴は認めず, ストレッサーとバーンアウトとの間には, 部分的に低い水準の相関が確認できた。ストレッサーの知覚では, 性差とジェンダー・タイプの交互作用は有意ではなく, 「役割葛藤」「医師関係」「死」「質的負担」において男性よりも女性の方が有意に高得点で, 「役割葛藤」と「医師関係」においてはジェンダー・タイプの両性具有型の方が女性性型・男性性型よりも高かった。バーンアウトの知覚では, 「情緒的消耗感」は男性よりも女性の方が有意に高く, 「個人的達成感」では両性具有型が他のジェンダー・タイプよりも有意に高かった。この結果から, ストレッサーの知覚には女性であることと両性具有型であることが関与するが, 両性具有型の人は何らかのストレス対処要因を持ち合わせており, これがバーンアウトを防ぐ「個人的達成感」につながっていると推察された。The purpose of this study was to examine whether biological sex and/or gender type most affected perception of stressors and burnout in nurses. Self-rating questionnaires of BSRI (Bem Sex Role Inventory), NJSS (Nursing Job Stressor Scale), and MBI (Maslach Burnout Inventory-Japanese version) were administered to nurses comprising males 101, females 101 who work in three general hospitals. Results showed that sex differences and the gender type were independent of each other, and the characteristic of sex differences was not recognized. Weak correlations were found between stressors and burnout. Interaction effects between sex differences and the gender type were not significant. Perception of stressors in "nursing role conflict" "conflict with physicians/autonomy" "dealing with death and dying" "qualitative workload" were significantly higher in the female (sex) than in the male (sex). Perception of stressors in "nursing role conflict" "conflict with physicians/autonomy" was significantly higher in the psychologically androgynous (gender type) than in the masculine or feminine (gender type). Perception of the burnout in "emotional exhaustion" was significantly higher in females than in males, and "personal accomplishment" was significantly higher in the psychologically androgyny (gender type) than in other gender types. Considering these results, it can be deduced that although both females and the psychologically androgynous gender types are associated with perception of stressors, nurses in the second category may have some stress-coping factors, associated with "personal accomplishment" and prevent burnout as a result.

    本研究は、教授する人材が不足の中、わが国の特徴を踏まえ、かつ国際社会のニーズに合った人材を教育できるプログラムが必要との考えのもと、全国の各看護師養成機関における教授上の課題を明らかにし、講義を補完する支援システムを構築することを目的としている。本研究は、(第1段階)全国の看護系大学と看護専門学校の国際看護教育の実態調査、(第2段階)補完の必要性の高い講義・演習案の作成及び講義映像の作成、(第3段階)作成した講義映像、演習内容の試行及び評価、(第4段階)クラウド型国際看護学講義補完配信システムの構築の4段階で研究実施を計画している。
    第一段階の実態調査を行うために、2020年度は①日本国内の国際看護教育に関するもの、②海外における国際看護教育に関するもの、③日本の国際看護教育で使用されている教科書の構成要素の3つの視点で、文献検討を行い、それぞれ学会発表及び論文の投稿を行った。
    2021年度は第一段階の実態調査として、全国の看護師養成機関に対し国際看護教育の実態調査を行った。調査結果は日本国際看護学会の学術集会にて発表済みであり、現在論文の投稿作業に向けて準備を行っている。また実態調査により明らかとなった教授上の課題をもとに今後は第二段階である「補完の必要性の高い講義・演習案の作成及び講義映像の作成」を行う予定となっている。

    今年度は、認知行動療法を用いたメンター制度の構築し有効性を確認する。トレーニングを受けた看護師(メンター)により、認知行動療法を臨床経験1から5年目(メンティー)の看護師に対し実施し、バーンアウトや離職の意思が低下するか否かを明らかにし、看護師のメンタル面をサポートするメンター制度を構築するすることを目的に研究した。メンターによる認知行動療法は、メンティにも効果があることが示された。

    本研究の目的は、「女性看護師の転職による職場適応過程のプロセスと再適応の関連要因の解明」である。
    2020年度は、中途採用者、および中途採用者を受け入れた看護管理者を対象にした質的研究結果を分析し、日本看護科学学会学術集会にてポスター、および口頭でそれぞれ1編ずつ発表を行った。
    ポスター発表のタイトルは、「看護管理者による中途採用看護師への職場移行支援のプロセス」で、以下のことが示唆された。①看護管理者は中途採用者の入職までの経験や入職後の成長支援によって、自部署の発展を描いていたことが示唆された。②看護管理者による中途採用者の職場移行支援は、中途採用者の学習ニーズや不安・期待の理解と、職場順応のサインを基にトランジションの段階を判断し、職場の状況と照合しながら職場になじむように柔軟に対応していた。③看護管理者は中途採用者が望む職場を目指した移行支援をPDCAによって推進していることが伺えた。④中途採用者への職場移行支援には、採用者の仕事上の経験や職業観・生活スタイル・学習ニーズの理解のもとに、根拠に基づいた人材育成方法の知見を活用して、移行の段階に応じた柔軟な成長支援が望まれた。
    口頭発表のタイトルは、「20歳代女性看護師の転職による職場適応過程の解明」で、以下のことがわかった。転職者は、前職の経験より自分なりの仕事への定義を獲得していた。転職先では、もともと持っていた定義が新しい経験の遭遇により変化せざるを得ない状況となった。定義が変化する過程では,ネガティブな感情が出現するが、より良い職場環境と自らの経験や学習能力により乗り越えることで、新たな看護や仕事への価値を生み出し順応していた。
    以上の結果から、転職を経験した女性看護師を対象に、「転職した若手女性看護師の職場適応の過程と関連要因の解明」を目指して、量的研究を計画した。

    This study aims to develop and understand the effectiveness of staff training program based on cognitive behavioral treatment, with the focus on burnout syndrome. The ultimate objective is to reduce the turnover rate in nursing. Firstly, “Study 1” created a checklist that measures the level of proficiency grained, regarding cognitive behavioral treatment, and analyzed its reliability and validity. In “Study 2”, we conducted a staff training on cognitive behavioral treatment. The training targeted nurses with over 10 years’ clinical experience. The training measured areas such as their communication skills, and the data was used to determine the effectiveness of the program. The results showed that the satisfaction level of the organizers themselves was high, and that they could facilitate the treatment well. The results also suggested that the nurses in their first to third year of clinical experience, who attended the sessions.

    The purpose of this research is to reduce a burnout and intention of resigning by developing the program of group cognitive behavior therapy which focused on the intervention in a belief and automatic thoughts with an nurse's irrational belief for the clinical years-of-experience 3 year nurse, and performing group cognitive behavior therapy. According to the results, the significant difference was checked by a "helplessness" of an irrational belief, and "evasion / escape type coping" of coping. That is, these cognitive factors decreased by cognitive behavior therapy. Moreover, it was suggested that cognitive behavioral approach appears to be effective for reducing nurse's burnout or intention of resigning from the job in nurses with 3 year of clinical experience.
