    In this study, we conducted semi-structured interviews of family members of mentally ill individuals who underwent an Assertive Community Treatment program (ACT). We used a qualitative, descriptive study design to codify and categorize data with continuous comparative analysis, which provided an overall picture regarding the thoughts of family members. Our results revealed a transition process in family members mindset to "I want to leave things as we are, and I become self-congruent" because "I feel like there are people to support us." Although they continued to have "unforgettable suffering" and "anxiety about the mentally ill individual and I getting old", specific changes in the thoughts of family members included a transition to thoughts of empathic understanding such as "I feel the sadness they hold in without telling anyone" from "I feel like there are people to support us". In addition, by thinking "I understand that it is so because they have a mental illness" and "I am accepting that it is so because they have a mental illness", family member-s progressed to think "I want them to continue medications to maintain their lifestyle" and "I exist with people with mental illnesses".

    The objective of this study is to show the actual situations of cases in which nurses feel difficulty in taking care of psychiatric patients and their families at home-visit nursing stations and explore the support needs of visiting nurses. The objects were 116 home-visit nursing stations in A prefecture, and 48 stations answered to our questionnaire. We implemented a questionnaire research about the experiences of difficult cases by mailing method and interviewed 10 visiting nurses of 6 cooperative stations about their 13 difficult cases and their support needs. The difficult cases were reported by 14 stations (29.2%) for patients and by 12 stations (25.0%) for families. Also, it was reported that visiting nurses have "the anxiety for the difficulty in understanding psychiatric patients" and their support needs were "the knowledge of the effective way to respond according to each situation" and "nursing advice of psychiatry experts". Detailed countermeasures, consultation services by psychiatric specialists, and network as the support for the relationship of visiting nurses are considered to be required.

    Abstract

    As a countermeasure to the increased loneliness induced by the COVID‐19 pandemic‐related university closures, universities provided students with online interaction opportunities. However, whether these opportunities contributed to reducing loneliness during the university closures remains unclear, as previous studies have produced contradictory findings. We conducted a nationwide cross‐sectional survey. Data were collected on demographics, social environment, social support, interactions, health and loneliness from 4949 students from 60 universities across Japan. We used psychological network and Directed Acyclic Graphs (DAGs) to examine the effect of online interactions on loneliness during university closures during COVID‐19. The results showed that the frequency of online interactions with friends did not exert a significant influence on loneliness during university closures. A comparative examination of the DAGs further illuminated that the social environment exhibited fewer pathways for interpersonal interactions and social support during these closure periods. The psychosocial pathways influencing young adults' loneliness show variations contingent on the university's closure status. Notably, the impact of heightened online interactions with friends on loneliness appears to be less pronounced among young adults in the context of university closure.

    本研究は,デイケアに通所し地域で生活する統合失調症患者83名を対象に,居場所感とQOLの2変数の関連を明らかにすることを目的とした横断研究である。調査項目は,日本語版WHOQOL-26,精神障害者の居場所感尺度(3下位尺度からなる2次因子モデル),個人特性で構成した。想定した居場所感とQOLの因果関係モデルを共分散構造分析を用いて検討した結果,適合度指標は統計学的な許容水準を満たし,モデルはデータに適合した。居場所感が大きいことは,より良いQOLであることが明らかになり,その寄与率は44%であった。このことから,在宅生活をする統合失調症患者がより良いQOLを獲得するためには,彼らの居場所感を高める支援が有効であることが示唆された。(著者抄録)

    The relationship of family acceptance, stress coping behavior, distress in daily life to familial negative attitude was investigated using causal analysis in 150 family members living in the Chugoku and Shikoku districts returned completed questionnaires. Family acceptance and stress coping behavior were indirectly determines familial negative attitude via distress in daily life while family acceptance was directly determines. The determination coefficients were 0.34 for distress in daily life and 0.47 for familial negative attitude which indicate 34% of the variance in distress in daily life was explained by family acceptance and stress coping behavior, 47% of the variance in familial negative attitude was explained by distress in daily life based on family acceptance and stress coping behavior. These results suggest potential clues to nursing support for family members based on statistical evidence.

    This study aimed to implement an experience-based program to promote the understanding of the concept of recovery, which is defined as a meaningful life and valued sense of integrity based on subjective and individual viewpoints, among psychiatric nurses working in hospitals in Japan and to evaluate this program. Methods: We conducted a preliminary survey of nurses at six facilities with psychiatric wards. The experience-based program was conducted for 12 nurses who expressed a wish to participate. The program spanned three days and consisted of lectures, assertive community treatment (ACT) staff visits for recovery orientation, and group work. Results: The mean Recovery Knowledge Inventory of nine participants ultimately included was 3.41 points (SD, 0.28) before the program and 3.69 points (SD, 0.24) after the program, indicating a significant difference (p =.004). Categories obtained from ACT support experiences included the following: “Continuing to attend to the need to live in one’s community/home regardless of how bad psychiatric symptoms become without the use of medicines,” “Viewing the person living their life in a place where they belong and in their own individual style,” “Valuing the patient’s wishes is the slow but sure way to a fruitful relationship,” and “Become familiar to the patient and their family’s lifestyle by carefully listening to the family’s feelings”. Discussion: We believe that this program can help establish the concept of recovery in hospital, which tend to rely on the medical model, and enable daily nursing practice to be implemented from a strength-based viewpoint.

    The purpose of this cross-sectional study was to evaluate relationships among environmental provision and quality of life. Subjects were 83 schizophrenic patients attending day care at mental hospitals. The assessment was carried out using the Environmental Provision Scale, WHOQOL-26 Scale and included personal characteristics. The results of the application of structural equation modeling showed that the model was almost adequate to our date. This means that more greater environmental has more good quality of life. The contribution rate for quality of life was 44%. These results suggested that, in order to raise the quality of life in schizophrenic outpatients, nurses must provide nursing intervention to raise patients environmental provision.

    本研究は,精神疾患患者を支える家族を対象に,生活困難を抱える家族のストレス反応の軽減と患者の病状悪化を予防するための看護実践の基礎資料を得ることを目的として,患者受容,生活困難度,対処行動,批判的態度の因果関係を検討した。結果,中国四国圏内に住む150名の家族から質問紙調査の回答が得られ,患者受容とストレス対処行動(情緒優先対処行動)は,生活困難度を介して間接的に患者への批判的態度を規定するとともに,患者受容は直接的に批判的態度へ影響する因果モデルが支持された。決定係数は,生活困難度に対し0.34,批判的態度に対し0.47であり,生活困難度の分散の34%が患者受容と情緒優先対処行動によって説明でき,批判的態度の分散の47%が患者受容,情緒優先対処行動,生活困難度によって説明できることが示された。以上のことから,統計学的な根拠に基づいた家族へのストレス支援の手がかりが示唆された。(著者抄録)

    This study aimed to implement an experience-based program to promote the understanding of the concept of recovery, which is defined as a meaningful life and valued sense of integrity based on subjective and individual viewpoints, among psychiatric nurses working in hospitals in Japan and to evaluate this program.
    The results indicated recovery orientation significantly increased in the participants, revealed multiple instances of the concept of recovery in categories obtained from participant Assertive Community Treatment support experiences, helped nurses to realize the current limitations of nursing support in hospitals that tend to rely on the medical care model.

    A program with the aim of working toward the recovery of in-patients in psychiatric hospitals was implemented. The results showed that the average values after implementing the program were statistically significantly higher for all items. Moreover, all the sub-factors showed a large change in the effect size that indicated the change’s strength. Specific connections with families were observed in all the participants’ practice reports, interpreted as indications of 11 effects of family support that were interpreted from the self-reflections. The results suggest that specific methods for support aimed at the recovery of in-patients in psychiatric hospitals were demonstrated. The results also indicated the effectiveness of listening to the thoughts of the families as a form of support that included the families during patient hospitalizations, accepting them, and having an empathetic understanding.

    I conducted individual interviews with 8 families who have a family member with psychiatric disability and use Assertive Community Treatment. I made it clear that how each family is working for well-being of the disabled family member in daily life. It is inevitable to understand families'thoughts and needs in order to support them properly.
