    <jats:title>Background:</jats:title>
    <jats:p>The primary etiology of Little League shoulder (LLS) is rotational torque caused by repetitive throwing motion. However, there are few reports on the assessment of rotational torque during physical examination.</jats:p>
    </jats:sec>
    <jats:sec>
    <jats:title>Purpose:</jats:title>
    <jats:p>To investigate the usefulness of the resisted external rotation test (RERT) and the resisted internal rotation test (RIRT) in diagnosing LLS and determining the time to return to sports (RTS).</jats:p>
    </jats:sec>
    <jats:sec>
    <jats:title>Study Design:</jats:title>

    </jats:sec>
    <jats:sec>
    <jats:title>Methods:</jats:title>
    <jats:p>In total, 101 patients were diagnosed with LLS by proximal humeral physeal widening on radiography and tenderness on palpation over the lateral aspect of the proximal humerus, and the RERT and the RIRT were performed. During the 2 tests, the examiner lifted the patient’s elbow joint with one hand toward the humeral shaft and held the patient’s wrist joint with the other hand. In the RERT/RIRT, the patient was instructed to apply a maximal force of external/internal rotation from neutral alignment. The examiner resisted the force of external/internal rotation and maintained the setting position while holding the wrist joint. These test results were positive if either or both tests elicited shoulder pain. Positive test results and their association with radiographic findings were examined. In case of positive test results, the time to achieve negative test results and the time to RTS were investigated.</jats:p>

    <jats:title>Results:</jats:title>
    <jats:p>
    The RERT and RIRT were correlated with severity of LLS. The sensitivity of RERT/RIRT for LLS was 94.1%/36.6%, respectively. In RIRT, patients with advanced-stage LLS were more likely to have positive results than those with early-stage LLS (67.4% vs 10.9%;
    <jats:italic>P</jats:italic>
    &lt; .001). The mean time to achieve negative RERT and RIRT results was 6.7 and 4.7 weeks, respectively (

    = .01). Patients with advanced-stage LLS had a longer mean time to achieve negative RERT results than those with early-stage LLS (6.7 vs 4.7;

    &lt; .001). The time to RTS was 8.8 weeks in the group that resumed throwing after achieving negative RERT, while the group that resumed throwing before RERT became negative took 12.7 weeks (

    BACKGROUND: The purpose of this study was to examine two techniques for Carpal Tunnel Syndrome, mini-Open Carpal Tunnel Release (mini-OCTR) and Endoscopic Carpal Tunnel Release (ECTR), to compare their therapeutic efficacy. METHODS: Sixteen patients who underwent mini-OCTR in palmar incision and 17 patients who underwent ECTR in the wrist crease incision were included in the study. All patients presented preoperatively and at 1, 3, and 6 months postoperatively and were assessed with the Visual Analogue Scale (VAS) and the Disabilities of Arm, Shoulder and Hand Score (DASH). We also assessed the pain and cosmetic VAS of the entire affected hand or surgical wound, and the patient's satisfaction with the surgery. RESULTS: In the objective evaluation, both surgical techniques showed improvement at 6 months postoperatively. The DASH score was significantly lower in the ECTR group (average = 3 months: 13.6, 6 months: 11.9) than in the mini-OCTR group (average = 3 months: 27.3, 6 months: 20.6) at 3 and 6 months postoperatively. Also, the pain VAS score was significantly lower in the ECTR group (average = 17.1) than in the mini-OCTR group (average = 36.6) at 3 months postoperatively. The cosmetic VAS was significantly lower in the ECTR group (average = 1 month: 15.3, 3 months: 12.2, 6 months: 5.41) than in the mini-OCTR group (average = 1 month: 33.3, 3 months: 31.2, 6 months: 24.8) at all time points postoperatively. Patient satisfaction scores tended to be higher in the ECTR group (average = 3.3) compared to the mini-OCTR group (average = 2.7). CONCLUSIONS: ECTR in wrist increase incision resulted in better pain and cosmetic recovery in an early postoperative phase compared with mini-OCTR in palmar incision. Our findings suggest that ECTR is an effective technique for patient satisfaction.

    Background: It is not clear whether rehabilitation after surgery for trigger finger is effective. The aim of this study was to reveal its effectiveness for trigger finger. Methods: This study was a randomized, controlled trial that included patients who underwent operations for trigger fingers. The patients in the rehabilitation group had postoperative occupational therapy (OT) for 3 months, while the patients in the control group were not referred for rehabilitation but received advice for a range of motion exercises. We evaluated the severity of trigger finger, Disability of Arm-Shoulder-Hand (DASH) score, pain-visual analogue scale (VAS), grip strength, whether they gained a full range of motion (ROM), and complications before and after surgery. Results: Finally, 29 and 28 patients were included in the control and rehabilitation groups, respectively. At final follow-up, the DASH score, grip strength, and ROM were significantly improved in the rehabilitation group compared to that preoperatively. At final follow-up, pain was significantly improved in both groups from that preoperatively. There were no significant differences in the results, including the DASH score, grip strength, ROM and pain-VAS between the control and rehabilitation groups at the final follow-up. Subgroup analysis showed that there is a significant difference in the DASH score of patients doing housework or light work and those with a duration of symptoms &gt;12 months between the control and rehabilitation groups at the final follow-up.

    Rupture of the extensor pollicis longus (EPL) tendon is a known complication after undisplaced distal radius fracture (DRF). However, no report has revealed the relationship between EPL tendon rupture and the fracture pattern. Thus, this study aimed to investigate the characteristics of fractures at risk of EPL tendon rupture using fracture line mapping of undisplaced DRFs. This study used computed tomography imaging data of undisplaced DRFs with (n=18) and without EPL tendon rupture (n=52). Fracture lines obtained from 3D reconstruction data were drawn manually after matching with a 2D template wrist model. Fracture maps represented the fracture line distribution by superimposing the fracture lines of all 70 patients. Heat maps showed the relative frequency of the fracture lines as a gradual color change. Fracture lines of cases with EPL tendon rupture were concentrated in the proximal border of Lister's tubercle. By contrast, fracture lines of cases without EPL tendon rupture were relatively dispersed.

    AIMS: There are concerns regarding complications and longevity of total elbow arthroplasty (TEA) in young patients, and the few previous publications are mainly limited to reports on linked elbow devices. We investigated the clinical outcome of unlinked TEA for patients aged less than 50 years with rheumatoid arthritis (RA). METHODS: We retrospectively reviewed the records of 26 elbows of 21 patients with RA who were aged less than 50 years who underwent primary TEA with an unlinked elbow prosthesis. The mean patient age was 46 years (35 to 49), and the mean follow-up period was 13.6 years (6 to 27). Outcome measures included pain, range of motion, Mayo Elbow Performance Score (MEPS), radiological evaluation for radiolucent line and loosening, complications, and revision surgery with or without implant removal. RESULTS: The mean MEPS significantly improved from 47 (15 to 70) points preoperatively to 95 (70 to 100) points at final follow-up (p < 0.001). Complications were noted in six elbows (23%) in six patients, and of these, four with an ulnar neuropathy and one elbow with postoperative traumatic fracture required additional surgeries. There was no revision with implant removal, and there was no radiological evidence of loosening around the components. With any revision surgery as the endpoint, the survival rates up to 25 years were 78.1% (95% confidence interval 52.8 to 90.6) as determined by Kaplan-Meier analysis. CONCLUSION: The clinical outcome of primary unlinked TEA for young patients with RA was satisfactory and comparable with that for elderly patients. A favourable survival rate without implant removal might support the use of unlinked devices for young patients with this disease entity, with a caution of a relatively high complication rate regarding ulnar neuropathy.Level of Evidence: Therapeutic Level IVCite this article: Bone Jt Open 2023;4(1):19-26.

    人工肩関節置換術を行った15例16肩(全例女性、平均70.6歳)の治療成績について検討した。疾患は変形性肩関節症5肩、関節リウマチ11肩、観察期間は平均3.0年であった。平均値でJOAスコアは39.9点→80.3点、上肢障害評価表(DASH)は70.1点→56.0点、肩関節可動域は屈曲が68.7度→104度、伸展が37度→69度、外転が52度→82度、外旋が15度→36度、内旋が高位臀部→L3高位に改善した。X線学的評価では弛みやコンポーネントの脱転をきたした症例はなかった。腱板の状態はintact:5肩、thin:8肩、torn:3肩であり、腱板機能残存群(intact)と腱板機能不全群(thin・torn)に分けた成績は、JOAスコアの各項目の改善度に有意差はなく、可動域では残存群で屈曲、外転、外旋が不全群に比べ有意に改善度が高かった。Acromio-head distanceは不全群で術後に狭小化する傾向であった。術前DASHと術後JOAスコアとの間に強い負の相関を認めた。

    関節リウマチに対しSwansonおよびAVANTAインプラントを用いてMP人工指関節置換術を行い、半年以上経過観察し得た34例118指について治療成績を検討した。Swanson群は48指、AVANTA群は70指であった。治療成績として、MP関節可動域、握力、ピンチ力、health assessment questionnaire、Hand20、Disabilities of the Arm, Shoulder and Handを調査した。また、画像評価として、インプラント周囲の骨透亮像と硬化像を計測し、更に尺側偏位、破損等についても検討を行なった。その結果、術後の臨床成績においてAVANTA群が優れる傾向にあったが、明らかな有意差はなかった。インプラント破損をSwanson群の3指に認めたが、シリコンインプラントの耐久性として経過観察期間(Swanson群60.7±20.5ヵ月、AVANTA群21.3±9.1ヵ月)の影響が考えられた。

    脛骨遠位部骨折に対し一時的創外固定を併用してstaged operationを行った33例の治療成績について検討した。平均年齢は59歳で、骨折型はAO分類で43のAタイプが9例、Bタイプが6例、Cタイプが18例で、うち開放骨折が10例であった。その結果、受傷〜最終固定までの平均期間は13.3日、最終固定までに要した軟部組織再建は有茎皮弁が2例、遊離皮弁が1例、植皮が1例であった。6ヵ月以内に31例で骨癒合が得られたが、1例は遷延癒合、1例は深部感染から足関節固定を要した。尚、staged operation終了後の合併症は術後皮膚壊死2例、minor infectionを含む感染が3例であった。

    症例1(52歳男性)。作業中に鉄板が左足部に落ちて受傷した。X線所見より第一楔状骨粉砕を伴うリスフラン関節脱臼骨折と診断され、受傷当日に緊急手術を行い、創外固定術を施行した。だが、受傷後1ヵ月で創壊死と感染を生じたため、骨欠損部にセメントビーズを使用し感染を鎮静化した後、骨移植と有茎皮弁を行い、最終手術後6ヵ月で骨癒合が得られた。症例2(24歳男性)。交通事故にて受傷し、X線所見より第一楔状骨粉砕を伴うリスフラン関節脱臼骨折と診断され、整復徒手とシーネ固定を行い、受傷後10日で手術を施行した。術中に創外固定を併用して整復後、ロッキングプレートによる架橋プレート固定を行なったところ、術後4ヵ月で骨癒合が得られた。

    症例1は33歳男で、圧挫の強い右下腿骨折に対し、創外固定術を施行した。受傷後2週に髄内釘へconversionを行ったが、受傷後2ヵ月で感染拡大により髄内釘抜去し、創外固定術を再施行した。さらに2回抗菌薬含有セメントビーズ交換を行うも感染は沈静化せず、脛骨遠位部は骨欠損状態となった。受傷後3ヵ月でデブリドマンおよび抗菌薬含有セメント髄内釘挿入、イリザロフ創外固定にて再固定した。感染が沈静化したため骨延長を行い、ドッキング部に骨移植を行った。術後2年6ヵ月、感染の再発はなく、独歩可能である。症例2は18歳女で、交通事故にて骨欠損を伴う右下腿開放骨折を受傷し、抗菌薬含有セメントビーズ充填と創外固定術を施行した。その後、軟部欠損部に対してsural flapにて被覆した。受傷後2ヵ月時、骨欠損部に腸骨より骨移植、イリザロフ創外固定を施行したところ、術後7ヵ月で骨癒合を認めた。

    63歳女性。患者は数年以上前より両小趾の変形に気付くも放置していた。だが今回、両前足部の痛みおよび足趾の胼胝形成、歩行時の疼痛を主訴に受診となった。所見では立位X-PにてMTP関節で第5中足骨頭の外側への亜脱臼が認められ、内反小趾角は左右とも30°、第4-5中足骨間角(IMA)は右15°、左17°で、JSSF scoreは右32点、左42点であった。内反小趾に対して装具による保存治療が4ヵ月行われたが、症状の改善はみられず、中足骨骨幹部骨切り術が施行された結果、最終調査時(術後右側12ヵ月、左側9ヵ月)の骨癒合は良好で、内反小趾角は左右とも0°、IMAは右4°、左5°であった。また、JSSF scoreは右90点、左95点であった。尚、術前より認められたIP関節およびMTP関節の可動域制限は残存したが、疼痛や有痛性胼胝は消失し、良好な経過が得られた。

    症例1:63歳女。橈骨遠位端骨折(AO分類21-A3、背側転位型)に対する掌側プレート固定後2年10ヵ月で母指の屈曲障害が出現した。X線では骨折部のアライメントは良好で、プレートの浮き上がり、スクリューの突出もなかった。抜釘時、遠位列外側2穴目のscrew hole部分の露出とFPLの完全断裂を確認した。症例2:86歳女。橈骨遠位端骨折プレート固定後3年10ヵ月で示指DIP屈曲困難が出現した。CTでプレートはwatershedlineを超えない位置であった。術中所見でプレートは遠位列が露出し、FPLとII指FDSが部分断裂、II指FDPは完全断裂であった。症例3:74歳女。橈骨遠位端骨折プレート固定後7年6ヵ月で母指の屈曲障害が出現した。X線ではプレートの浮き上がりやスクリューの突出は認めなかった。抜釘時、FPLの完全断裂とII指FDPの部分断裂、断裂部直下でのプレート、screw holeの露出を認めた。全例にPLを使用した腱移植術を行った。

    骨端線障害後の下肢遺残変形に対し、Ilizarov創外固定器による複数回の矯正術を行った2症例について検討した。症例1(10歳男児)は1歳時の化膿性膝関節炎、症例2(14歳男児)は6歳時の交通事故により骨端線障害が生じていた。6歳時と9歳時に、Ilizarov創外固定器を用いて矯正骨切りと脚延長を行った。しかし変形は再発し、2度目の手術を9歳時と13歳時に行った。2例とも、Ilizarov創外固定器を用いて良好なアライメントと脚長差の補正が得られたが、若年での骨端線障害における遺残変形に対する骨端線閉鎖術、もしくは解除術なしでの変形矯正術は、複数回の手術を要すると思われた。

    小児の化膿性膝関節炎では、骨端線に障害が及ぶと下肢のアライメント異常を生じる。今回著者等は、生後1ヵ月時に化膿性膝関節炎を起こし、治療により関節炎は軽快したものの、大腿骨遠位の骨端線が高度に障害され、成長とともに外反膝変形が進行して膝蓋骨脱臼をきたした症例を経験した。Ilizarov創外固定器による複数回の矯正手術と観血的脱臼整復術を行い、良好なアライメントを獲得することができた。

    NCB distal femurプレートを用いて治療を行った大腿骨遠位部骨折5例の治療成績について検討した。AO分類はA1が2例、A2、A3、C2が各1例であった。その結果、1)骨癒合期間は平均22週、関節可動域は伸展平均3°、屈曲平均119°であった。2)Neerの評価基準による治療成績評価ではexcellentが1例、satisfactoryが2例、unsatisfactryが2例であった。3)術後合併症は遷延癒合を1例に認められた。尚、Femoral angleは術直後平均83.0°、最終観察時平均82.8°、Lindahl's angleは術直後平均39.4°、最終観察時平均39.8°と術後アライメントは良好に維持されていた。

    1997〜2007年にhemipulp flap(HPF)またはwrap around flap(WAF)を用いて母指を再建した、各6症例6指を対象に比較検討した。更に独自の質問項目により再建母指で困っていることを調査した。1)HPF群はIP Crease付近までの指腹欠損で、爪母が温存されており、IP関節は温存されていた。WAF群はIP関節から基節レベルの切断例であった。2)WAF群はHPF群と比較して年齢が高く、45歳以上の例が4例あったが、Semmes-Weinstein testでredの例はなかった。HPF群は20歳代の2例がredであった。3)Disabilities of the arm, shoulder and hand scoreの平均値は両群間に差はなかった。4)HPF群の機能評価は知覚や合併傷等の因子に対して一定の傾向がなく、WAF群と比較してばらついている印象があった。5)深腓骨神経を含めてHPF群は2本、WAF群は3本の神経を再建しており、より多くの神経を再建することでより良い知覚が得られる可能性が示唆された。

    Neurofibromatosis type 1に合併した脊柱側彎症5例(全例女性、dystrophic type4例、non-dystrophic type1、平均年齢18歳)に対し後方固定術を行い、その治療成績について検討した。術後平均観察期間41ヵ月の結果、術後の矯正率は平均49%、術中出血量は平均1710mlで、全例で自己血を使用、神経損傷や偽関節などの合併症は認められなかった。

    骨接合術を行った上腕骨通顆骨折23例23骨折(男19例・女4例・平均80歳)について検討した。受傷原因は転落17例、落下5例、その他1例であった。Perryらによる分類ではA(無転位)が3例、B(転位あり)18例、C(T字型)2例であった。骨接合術の内訳は鋼線2例、tension band wiring(TBW)10例、screw 1例、ONI plate 8例、MAYO plate 2例であった。これらの接合法別に代表例の経過を提示し、術式の特徴を解説した。全体の治療結果(経過観察期間平均13ヵ月)は、Jupiterらの評価基準を修正した肘屈曲可動域による評価で、骨折型Bの17例中12例がExcellent(15〜130°以上) およびGood(30〜120°以上)であった。また、TBWの8例中1例に遷延治癒を認めたが、他の7例はGood以上であった。経過観察できた20例のうち、Good以上は15例であった。

    13歳男児。スライディングで受傷し他院でのシーネ固定後、翌日に当院紹介受診した。X線で脛骨遠位部にわずかな骨折線を認め、CTで脛骨遠位骨端線前外側に関節面に及ぶ骨折を認めjuvenile Tillaux骨折と診断した。骨折の転位は最大5.3mmであり受傷後4日目に手術を施行した。前外側より小皮切を加え徒手的整復を試みたが十分な整復が得られないため、20mmのKirschner鋼線を挿入してガイドとし、脛骨プラトー骨折用cannulated bone tampを用いてハンマーで叩いて圧着整復した。その後、径4mmのcannulated cancellous screwで固定した。整復は良好で術後7週に抜釘、術後4ヵ月には歩行時痛、足関節可動域制限もなく、CTで良好な骨癒合を確認した。術後2年7ヵ月現在、早期骨端線閉鎖や変形性関節症性変化は認めなかった。

    症例1:6歳女児。自転車で転倒し、左肘を打撲した。Gartland分類タイプ2の屈曲型上腕骨顆上骨折と診断し、3本のK-鋼線による経皮ピンニングを行い、整復位、骨癒合とも良好であった。術後33ヵ月の肘関節可動域は屈曲148°、伸展13°で、軽度外反肘はあるがADLに大きな支障はない。症例2:10ヵ月男児。母親に抱きかかえられた状態で1mの高さから落下した。診断および治療は症例1と同様で、術後23ヵ月の可動域は屈曲150°、伸展0°であった。症例3:12歳男児。自転車で転倒した際、肘伸展位で左手をついて受傷した。診断は症例1と同様で、外顆より2本のK-鋼線でピンニングを行い、整復位、骨癒合とも良好であった。術後7ヵ月の肘関節可動域は屈曲150°、伸展0°であった。

    経皮ピンニングを施行した小児上腕骨顆上骨折326例(男児212例・女児114例)のピン配列と成績について報告した。骨折側は左196例、右130例で、伸展型320例、屈曲型6例であった。ピン配列は、服部法による外顆からの2本法195例、3本法73例、交差法44例、その他14例で、安部分類による型はII型92例、III型62例、IV型166例、不詳6例であった。2本法が多数を占めたが、III型、IV型は3型法が増加した。交差法は当初のみ行った。合併症は、ピン刺入による神経損傷は全例で認めなかった。固定不足による再手術が16例で、回旋変形8例、屈曲変形6例、尺屈変形2例であった。

    正常な靭帯や腱の骨への付着部の組織は“enthesis”と呼ばれており、近年力学的負荷と生物学的因子がenthesisの発生と発達に寄与していることが明らかになってきている。腱や靭帯を元の骨の付着部へ再固定し、その付着部の状態を検討した研究は散見されるが、移植腱を用いた修復モデルによる研究は乏しく、移植腱におけるenthesis形成のメカニズムはまだ不明である。我々はラビットを用いて、独自の移植腱モデルを作成し、enthesis形成のための至適条件を探索した。
    肘関節内側不安定症に対する移植腱による内側副靭帯の再建では、アンカーによる移植腱の固定、もしくは骨孔を作成し腱を骨孔内へ引き抜いて固定する方法が行われる。本研究では骨孔モデルとアンカーモデルを作成、組織学的にenthesisの発生状況を比較した。どちらも腱の骨付着部にはenthesis様の軟骨組織がわずかに形成したが、アンカーモデルでは有意に移植腱の萎縮が見られた。
    またenthesisの発生には、骨髄由来の間葉系細胞が重要といわれており、growth differentiation factor 5（GDF5）や、Basic fibroblast growth factor（bFGF）などの成長因子がenthesisの分化、増殖に関与するといわれている。本検討では移植腱モデルにおいて腱固定部位周囲のenthesisの発現がみられた部位の皮質骨の一部を削り、骨髄内の間葉系細胞が腱付着部周囲に誘導されやすいモデルを作成した。その結果さらなるenthesis様組織の形成が見られた。

    腱骨接合部 = エンテーシスの損傷に対する現在の治療法では正常構造の再構築は困難であり、再生医療の開発が望まれている。そして、その実現のためには組織発生や形成、変性メカニズムについての理解が必要である。
    我々が独自に開発した“ex vivo mechanical stress model”のデバイスを改良した上で、エンテーシス部に弱負荷、強負荷をかけたモデル、負荷をかけないコントロール群を作成し、エンテーシス部の組織変化について検証を行った。具体的な評価方法としてH-E染色、サフラニンO染色などの組織染色を行い、弱負荷群でエンテーシス構造、軟骨組織が維持されていることを確認した。
    またエンテーシス、軟骨の発現に関与するRNA、タンパクを評価するために、qRT-PCR、免疫染色を用いて、Sox9, Scx, Collagen type X, MMP13の発現評価を評価した。弱負荷群ではSox9, Scx, MMP3の発現が、強負荷群では、X型Collagenの発現がコントロール群に比べ強くみられた。細胞のアポトーシスの評価目的にTUNEL染色も行い、弱負荷群では他群に比べアポトーシスを起こしている細胞の割合がすくなかった。
    さらにエンテーシスの構造の細部の評価をする目的で電子顕微鏡にてそれぞれのエンテーシスの構造を確認した。弱負荷群ではアポトーシスを起こしている細胞が他群に比べ少なく、また強負荷群においては、軟骨細胞層の石灰化が他群に比べ強くみられた。
