    BACKGROUND: Previous studies have assessed the upper airway using various examination methods, such as cephalometric imaging and magnetic resonance imaging (MRI). However, there is a significant gap in the research regarding the relationship between these different imaging modalities. This study compares airway assessments using kinetic MRI and cephalometric scans, examining their correlation with three dimensional (3D) MRI data. MATERIALS AND METHODS: Kinetic MRI, cephalometric scans, and 3D MRI of forty-seven participants were used in the present study. Airway areas and widths were measured at the retropalatal, retroglossal, and hypopharyngeal levels in both kinetic MRI and cephalometric scans. Airway volumes were calculated from 3D MRI data. Statistical analyses, including the Wilcoxon Signed Rank test, Spearman correlation, and multiple linear regression, were performed to evaluate the data and identify significant differences, correlations, and prediction models, respectively. RESULTS: Significant differences were found between kinetic MRI and cephalometric scans. Cephalometric data showed larger airway areas and widths compared to kinetic MRI measurements. Although both cephalometric and kinetic MRI showed a correlation with 3D MRI, kinetic MRI demonstrated stronger correlations with 3D MRI airway volumes than cephalometric scans. According to our linear regression model equations, RPA-Max (maximum retropalatal airway area) and RPA (retropalatal airway area) can elucidate variations in RPV (retropalatal airway volume). RGA-Med (median retroglossal airway area) and RGA-Min (minimum retroglossal airway area) can explain variations in RGV (retroglossal airway volume). HPA (hypopharyngeal airway area) and ULHPAW-Max (maximum upper limit hypopharyngeal airway width) account for variations in HPV (hypopharyngeal airway volume). Additionally, TA-Max (maximum total airway area) can account for variations in TPV (total pharyngeal airway volume). CONCLUSION: Both cephalometric data and kinetic MRI data showed correlations with 3D MRI data. The shared posture of kinetic MRI and 3D MRI led to stronger correlations between these two modalities. Although cephalometric data had fewer correlations with 3D MRI and predictors for 3D airway volume, they were still significant. Our study highlights the complementary nature of kinetic MRI and cephalometric imaging, as both provide valuable information for airway assessment and exhibit significant correlations with 3D MRI data.

    OBJECTIVES: To determine the intricate associations between soft palate dimensions, pharyngeal airway lumen area, the minimal constricted area of the airway (MinAx), and pharyngeal airway volume in subjects with diverse vertical skeletal patterns, and to develop prediction models that could help clinicians predict upper airway volume using soft palate dimensions, airway lumen dimensions, and MinAx. MATERIALS AND METHODS: Pre-treatment lateral cephalometric radiographs and magnetic resonance imaging (MRI) scans of 80 women were classified into three vertical skeletal groups based on Frankfort-mandibular plane angle (FMA angle): hypodivergent (FMA < 26.9°, n = 26), normodivergent (26.9°≤ FMA ≤ 34.1°, n = 29), and hyperdivergent (FMA > 34.1°, n = 25). Soft palate dimensions, pharyngeal airway lumen data, MinAx, retropalatal airway volume (RPV), and retroglossal airway volume (RGV) data were measured using MRI. Forward multiple linear regression was used to predict pharyngeal airway volumes. RESULTS: Among the eight predictive models developed, six exhibited strong performance, explaining 50%-77% of the variability in airway volumes. MinAx, RPV, and total pharyngeal airway volume (TPV) were considerably higher in hypodivergent subjects than in hyperdivergent subjects. Hyperdivergent subjects had a longer soft palate length (SPL) than normodivergent and hypodivergent subjects. CONCLUSIONS: The present study highlights the necessity of considering soft palate dimensions and airway characteristics in orthodontic treatment planning, especially for patients with varying vertical skeletal patterns. Understanding these relationships can help in predicting potential airway issues and customising treatment plans accordingly.

    Retinoic acid (RA) signaling plays multiple essential roles in development of the head and face. Animal models with mutations in genes involved in RA signaling have enabled understanding of craniofacial morphogenic processes that are regulated by the retinoid pathway. During craniofacial morphogenesis RA signaling is active in spatially restricted domains defined by the expression of genes involved in RA production and RA breakdown. The spatial distribution of RA signaling changes with progressive development, corresponding to a multiplicity of craniofacial developmental processes that are regulated by RA. One important role of RA signaling occurs in the hindbrain. There RA contributes to specification of the anterior-posterior (AP) axis of the developing CNS and to the neural crest cells (NCC) which form the bones and nerves of the face and pharyngeal region. In the optic vesicles and frontonasal process RA orchestrates development of the midface, eyes, and nasal airway. Additional roles for RA in craniofacial development include regulation of submandibular salivary gland development and maintaining patency in the sutures of the cranial vault.

    BACKGROUND: Few prospective investigations have compared direct and indirect techniques through comprehensive and detailed clinical evaluations, considering the impact of all factors. OBJECTIVES: This study aimed to compare and evaluate direct and indirect bonding methods at a single institution and to clarify the selection criteria for the bonding method. MATERIALS AND METHODS: This single-centre, quasi-randomized controlled clinical trial included 153 patients who required fixed orthodontic treatment. They were randomly divided into indirect and direct binding groups by the project lead (K.K.), who was blinded to all clinical data, and performed the allocation using medical record numbers. The chair time for bracket bonding, discomfort during bracket bonding, oral hygiene after bonding, number of bracket failures, number of intentional bracket reattachments, post-treatment occlusal index, and total treatment time were assessed. Outcomes were compared using a two-sample t-test or Mann-Whitney U test (P < .05). RESULTS: Fifty-eight patients were included in the indirect bonding group (20 male, 38 female; mean age: 20.63 ± 5.69 years) and 66 (14 male, 52 female; mean age: 23.17 ± 8.83 years) in the direct bonding group. Compared to the direct bonding group, the indirect bonding group had shorter chair time (P < .001), a shorter total treatment period (P < .01), and a better final occlusal relationship (P < .001). The number of bracket detachments was higher (P < .001) in the indirect bonding group, but the number of intentional reattachments was lower (P < .001). CONCLUSION: Indirect bonding may improve the efficiency of orthodontic treatment. HARMS: No harm was observed during the study. TRIAL REGISTRATION NUMBER: This trial was approved by the Ethics Review Committee of Okayama University (approval number: d10001), UMIN registration number 000022182.

    Abstract

    Objective

    The objective of the study was to elucidate the association between cranial base (Bjork‐Jarabak analysis), midsagittal cross‐sectional area of the airway in the pharyngeal region (MCSA‐PR) data and pharyngeal‐airway volume (PAV) and develop a model that could help clinicians predict PAV using two‐dimensional (2D) data (Bjork polygon and MCSA‐PR).

    Materials and Methods

    Pre‐treatment lateral cephalometric radiographs and magnetic resonance imaging (MRI) scans of 82 women were categorized into three anteroposterior skeletal groups based on ANB angle: Class I (n = 29), 1.5° ≤ ANB≤5.1°; Class II (n = 26), ANB &gt;5.1°; Class III (n = 27), ANB &lt;1.5°. The Bjork polygon, MCSA‐PR data from cephalograms and PAV data from MRI scans were examined. Intergroup comparisons were performed using the Kruskal–Wallis test and one‐way analysis of variance (ANOVA), with pairwise comparisons conducted using the Bonferroni‐corrected Mann–Whitney U‐test for the Kruskal–Wallis test and Bonferroni‐corrected multiple comparison test for one‐way ANOVA. Forward multiple linear regression was used to create model equations for predicting PAV.

    Results

    MCSA‐PR and anterior (N‐S) and posterior (S‐Ar) cranial‐base lengths were positively correlated with the PAV. We developed four models; three operated at the group level, and one encompassed the entire sample. Notably, all models could effectively explain the variance in the PAV data. The model for the Class I group was the strongest (adjusted R2 = 0.77).

    Conclusion

    Our findings indicate the remarkable potential of the MCSA‐PR, N‐S and Bjork sum angles (BSA) as predictors of the PAV and the relevance of 2D cephalometric and cranial‐base parameters in predicting the three‐dimensional (3D) pharyngeal‐airway size.

    This report illustrates successful nonsurgical orthodontic treatment of a hypodivergent adult patient with bilateral posterior scissors bite (Brodie bite) and excessive overjet. A 26-year-old woman primarily reported maxillary incisor protrusion. She was diagnosed with Class ll division 1 malocclusion with skeletal Class I, short face, low mandibular plane angle and bilateral posterior scissors bite. A lingual arch with anterior bite block and posterior miniscrews with preadjusted edgewise appliances were used to improve the bilateral scissors bite. After achieving molar occlusion, the maxillary first premolars were extracted, and six miniscrews were used to improve the anterior-posterior and vertical discrepancies. After active treatment for 56 months, the convex facial profile with excessively protruded lips was improved and good interdigitation with ideal incisor relationship was achieved. Additionally, the irregular movements of the incisal path and the bilateral condyles during lateral excursion were improved. At 13 months of retention, a satisfactory facial profile, occlusion, and jaw movements were maintained. The treatment results suggest that miniscrews and fixed bite blocks were effective and efficient to facilitate correction of the bilateral scissors bite, excessive overjet, and vertical relationship correction in this nonsurgical orthodontic treatment.

    INTRODUCTION: In recent years, patients with orthognathic surgery in middle-aged and elderly people have come to be a more frequent occurrence. Breast cancer is the most frequently diagnosed cancer in woman worldwide, and its prevalence rate is steadily increasing. PRESENTATION OF CASE: We report a case of a 47-year-old Japanese woman in whom left-side breast cancer (Stage 1) was unexpectedly found just before orthognathic surgery in April 2012. Breast-conserving surgery was performed (estrogen receptor+, progesterone receptor+, HER2 -, surgical margin+, sentinel lymph node +) that May. From June to August docetaxel (75mg/m2) and cyclophosphamide (600mg/m2) were administrated four times every 21days and thereafter radiotherapy (total 60Gy) was completed. The cancer surgeon declared the prognosis good and the patient had a strong desire to undergo orthognathic surgery, so in November we performed a bimaxillary osteotomy, and administration of tamoxifen began 6 weeks after the osteotomy. DISCUSSION: There are breast cancer cases in which the prognosis is sufficiently good for a planned orthognathic surgery to proceed. Good communication among surgeons and the patient is important. CONCLUSION: We experienced a case in which breast cancer was found just before the orthognathic surgery; we performed a bimaxillary osteotomy, including follow-up tamoxifen administration, during breast cancer treatment.

    骨格性下顎前突患者に対し安全に下顎骨後方移動術を施行するためには、術後の上気道の狭窄やそれに伴う睡眠呼吸状態への影響に配慮しなければならない。過去の研究においても、下顎骨後方移動術を行った患者の上気道形態の変化は調べられているが、これらは覚醒時に撮影したセファログラムやコンピューター断層撮影画像を解析した静態評価であった。そのため、本研究は、上気道の動態および三次元解析が可能であるMRI movieおよびvolumetric MRIを用いて下顎骨後方移動術前後の睡眠時における上気道形態の変化を観察し、下顎骨の後方移動量と睡眠時の上気道の容量との関連を解明し、睡眠時の上気道形態変化と睡眠時呼吸状態との関連を調べることで睡眠呼吸障害の発症リスクを明らかにすることを目的としている。
    研究を開始するにあたり、昨年度の研究実績として、臨床研究実施計画書および研究説明書、同意書等を作成し、岡山大学臨床研究審査専門委員会への申請を行った。臨床研究審査専門委員会の承認が得られたため、患者への同意説明を開始したが、新型コロナウイルス感染症拡大に伴う手術の延期や患者の同意拒否が続いている状態である。

    It is speculated that cell homeostasis is reduced because osteoblasts on the surface of the bone matrix are exposed to various pathological conditions when orthodontic force is applied to teeth. However, details of molecular mechanism for maintaining homeostasis is still unclear. As a result of this study, it was confirmed that osteoblasts on the surface of the alveolar bone during orthodontic tooth movement were in a hypoxic state and were exposed to endoplasmic reticulum stress. In addition, the expression of autophagy markers at the same site was also confirmed, and it was also confirmed that in culture osteoblasts, autophagy was enhanced with respect to hypoxic stress. These findings suggest that autophagy may be involved in maintaining cell homeostasis in osteoblasts exposed to hypoxic and endoplasmic reticulum stress during orthodontic tooth movement.

    In the present study, we investigated if the new pain mechanism with Transient Receptor Potential (TRP) channel exist at experimental orthodontic tooth movement. There were TRPV2, TRPC1 and C6 in the periodontal ligament (PDL) tissues compared with the pulp tissues. The TRPV2-positive free nerve endings were observed in the PDL. Moreover, c-fos-positive cell were decreased at trigeminal subnucleus caudalis by the antagonist to the TRPV2. From the above, it is suggested that TRPV2 channel is concerned in the pain stimulus reception at orthodontic tooth movement.
