要介護高齢者の増大に対して，要介護状態の発症を遅らせ，健康寿命を延伸することが強く望まれている。近年，フレイルが高齢者の自立喪失の有意なリスク因子であると報告された。フレイルサイクルの一端に口腔機能，特に咀嚼嚥下機能の低下による低栄養がある。そこで，口腔機能低下に関するリスク因子を早期に発見し，早期に口腔機能，栄養状態の改善を図ることができれば，高齢者の要介護状態への転落を遅延できると考えている。
実際の食事時の筋電図を音声波形解析技術の応用と機械学習により，早期の咀嚼機能低下を類推する試みを開始したが，教師データ獲得が困難であったため，他のリスク因子を検討する中でACTN3遺伝子多型が抽出された。そこで遺伝子多型との関連を検討することとした。
咀嚼嚥下機能の維持，賦活を目的とし行われる筋機能訓練により，オーラルディアドコキネシスに加え，舌圧も改善することが知られている。この舌圧は骨格筋量よりも体幹筋量の影響を受け，舌機能や舌骨上筋群は速筋繊維が優位な筋と言われている。一方，サルコペニア発症時には速筋線維優位な筋線維の萎縮が生じ，速筋はサルコペニアの影響を大きく受ける。このサルコペニア発症のリスク因子の一つに，ACTN3遺伝子R577X多型による筋線維の萎縮が明らかとなっているが，これらと舌機能低下並びに嚥下障害を呈するリスクについての関連は明らかでない。
そこで本年度は，ACTN3遺伝子R577X多型を評価するための予備的検討を行った。特に，高齢者を対象にDNAを採取する必要があることから，被検者負担の少ない採取方法並びにその結果の妥当性について検討した。その結果，唾液中，頬粘膜の擦過（10回，20回）を行い，R577X多型を評価したところ，同一の結果が得られた。これより，頬粘膜の10回擦過によってR577X多型を評価するのに十分なDNA採取ができることが明らかとなった。 -

顎関節症の中でも，関節円板転位症例は関節円板後部結合組織の適応により症状・兆候が比較的短期間に軽減を示し，その大部分で良好な予後が期待できると言われている。その一方で，症状の持続あるいは悪化を示す症例も少なからず存在しており,痛みは長期化すると中枢の感作が生じることから，難治性へと移行すると治療はより複雑となる。そこで，本申請研究では顎関節症症状の難治化のリスク因子を解明することを目的として，顎関節症が多因子疾患であることを考慮し，精神心理学的評価や睡眠障害，睡眠時／起床時悪習癖などの要因を包括的に検証する解析モデルを用いた前向きコホート研究を行い，予後リスク因子を検討することとした。
そのなかで顎関節症状の難治化に関与する候補因子として，解剖学的要因だけでなく，行動要因，咬合要因，心理的要因の評価項目を含めて包括的に検討する予定であるが，候補因子の数が多いためにあらかじめ絞り込みを行う必要があった。そこで，予備研究として健常者を対象に過去に実施した顎関節症症状と諸因子に関する横断研究について質問紙による追跡調査を新規に計画し，顎関節候補因子間の相互相関性を明らかにすることとした。令和２年度は，研究を開始するにあたり，臨床研究実施計画書などを作成し，岡山大学臨床研究審査専門委員会への申請準備を行った。今後は予備研究を実施し，絞り込んだ候補因子を用いて，岡山大学病院顎関節症・口腔顔面痛み外来を受診した顎関節症患者を対象に初期治療の奏功/非奏功に関わるリスク因子を検討する予定である。 -
オーラルフレイル回避を目指した筋機能訓練の有効性検討と新規個別スキームの開発

現在，要介護高齢者の増大が社会的問題となっている。そこで要介護状態の発症を遅らせ，健康寿命の延伸が強く望まれている。近年，フレイルが本邦高齢者の自立喪失の有意なリスク因子であると報告された。フレイルサイクルの一端に口腔機能，特に咀嚼嚥下機能の低下による低栄養がある。そこで，口腔機能の低下を早期に発見し，早期に口腔機能，栄養状態の改善を図ることができれば，高齢者の要介護状態への転落を遅延できるかもしれない。
咀嚼嚥下機能のうち，咀嚼機能の評価には咬合力測定，グミゼリーやチューインガムといった試験食を実際に咀嚼させることによる評価方法やアンケートによる主観評価方法が用いられている。しかし咬合力測定や試験食による評価には高価な機材が必要であり簡便に広く臨床応用するのは難しい。一方，アンケート調査では被検者の食品の嗜好性や認知機能の多寡による影響を強く受けるといった問題点がある(富永ら，2007)。そのため，簡便かつ精度高く咀嚼機能を評価できるシステムの開発が強く望まれている。
令和2年度には健常者を被検者として，咬合時，嚥下時の筋電図学的検討を行った。その結果，嚥下時には閉口筋に加え，顎下筋群の筋活動亢進が認められた(岡山大学臨床研究審査専門委員会承認 臨2008-003)。さらに，口腔機能が低下した高齢者の筋電図を正常高齢者と比較したところ，低下高齢者では筋活動量の低下といった閉口筋(咬筋)機能の低下とともに顎下筋群の筋電図パターンに差異が認められた。 -

67 subjects (15 males, 52 females, average age at first visit 39.6 ± 16.4 years) of the all 338 subjects agreed to this study and underwent follow-up. The average elapsed period since the first visit was 12.2 +/- 1.1 years. 7 subjects with incomplete data were excluded, and we analyzed 60 subjects in this experiment. Comparing the mouth opening range at the time of the first visit to the follow-up survey, the mouth opening range increased respectively. There was no significant difference in the mouth opening range between the pathological classifications. Regarding the degree of pain and associated symptoms, the pain in the temporomandibular joint was significantly reduced at rest time and during functioning. On the other hand, there was no significant difference in the degree of headache, neck pain, and stiff shoulders.

The Sleep bruxism (SB) etiological mechanisms remain still unclear yet, thus, it is an important issue to elucidate the detail mechanisms of SB occurrence. There might be possible associations among the SB, above mentioned systemic movements and the micro-arousal, and the consideration of these associations would be helpful to clarify the SB mechanisms. Thus, we examined the detail associations among the occurrence of SB, body motion and sleep arousal. The mechanism such as nerve activities associated with leg movement and swallowing might be related to the aetiology of SB incidence.
