    BACKGROUND: Alcohol consumption is a common risk factor for esophageal squamous cell carcinoma (ESCC) and hepatic fibrosis. Mac-2 binding protein glycosylated isomer (M2BPGi), a sensitive marker of fibrosis, may predict postoperative skeletal muscle loss by reflecting the metabolic link between liver function and muscle maintenance. METHODS: Eighty-nine patients with ESCC undergoing esophagectomy (June 2021-July 2024) with alcohol use and pretherapeutic M2BPGi levels were included; the cutoff was 0.66. Muscle mass was assessed by bioelectrical impedance analysis. RESULTS: The study population was stratified by M2BPGi levels into low (n = 59) and high (n = 30) groups. No significant differences were observed in sex, clinical stage, preoperative skeletal muscle mass, surgical approach, or intraoperative fluid balance. The high M2BPGi group showed lower preoperative albumin levels (4.2 vs. 4.0 g/dL, p = 0.01) and higher CRP (0.1 vs. 0.4 mg/dL, p < 0.01) and ICG R15 values (9.2% vs. 12.8%, p = 0.04). Although there was no difference in the incidence of complications, the decrease in skeletal muscle mass at 3 months was significantly greater in the high M2BPGi group (- 7.6% vs. - 12.0%, p = 0.02). In the propensity score-matched cohort (low n = 40; high n = 20), the high M2BPGi group showed significantly greater muscle loss at 3 months (- 7.5% vs. - 12.6%, p = 0.04) and a trend toward longer hospital stays (22 vs. 25.5 days, p = 0.06). CONCLUSION: High pretherapeutic M2BPGi levels are independently associated with postoperative skeletal muscle loss at 3 months.

    BACKGROUND: Esophagectomy confers high postoperative morbidity and mortality. To improve postoperative outcomes, this study investigated the effectiveness of a 2-week prehabilitation program on the preoperative cardiopulmonary function of patients undergoing surgery for esophageal cancer. METHODS: In this parallel-group, open-label, superiority randomized clinical trial, participants were randomly assigned (1:1) to the multimodal prehabilitation (m-prehab) group or the control group and received exercise instruction from a physical therapist. The m-prehab group received additional exercise instruction 1 week later and were administered additional oral nutritional supplements (430 kcal and 47 g protein 2 weeks before surgery). The primary outcome was change in the anaerobic threshold (AT) from baseline to immediately before surgery. RESULTS: Among the participants (42 men and 8 women; m-prehab [n = 25], control [n = 24]; mean age, 65.3 years), those in the m-prehab group consumed significantly more protein than those in the control group (76.8 ± 16.6 vs 61.6 ± 18.0 g/day, p < 0.01). The m-prehab and control groups showed no significant intergroup difference in preoperative daily step counts (7590 and 7910, respectively) or change in AT. Quadriceps strength increased significantly more in the m-prehab group than in the control group (3.7 ± 13.5 vs - 5.3 ± 16.0; p = 0.04), and the skeletal muscle index showed a nonsignificant but increasing trend (4.0 ± 6.5 vs 1.1 ± 4.8; p = 0.09). Estimated protein intake was unassociated, whereas preoperative step count was significantly correlated positively with change in AT (r = 0.412; p = 0.02). CONCLUSIONS: This prehabilitation program increased preoperative protein intake but did not contribute to an increase in physical activity, nor did it result in significant differences in preoperative changes in AT.     Background/Objective: Postoperative dysphagia is a common cause of postoperative pulmonary complications (PPCs) following esophagectomy for esophageal cancer. Although the reclining posture is effective for general dysphagia, its effectiveness after esophagectomy remains unclear. Therefore, we aimed to explore effective approaches in the management of dysphagia after esophagectomy. Methods: This study included patients who underwent esophagectomy at the Department of Surgery, Hamamatsu University School of Medicine Hospital between January 2018 and March 2021. For the postoperative swallowing evaluation, the patients underwent a videofluoroscopic swallowing study by drinking 30 mL of liquid in two postures, a 45-degree reclining position (45°R) and a 90-degree upright position (90°U), and assessments were performed using the Penetration-Aspiration Scale. Results: Eighty-seven patients participated in the study. Laryngeal penetration and aspiration were, respectively, observed in 19 (21.8%) and 21 patients (24.1%) at 90°U, and in 14 (16.1%) and five patients (5.7%) at 45°R (p < 0.05). PPCs occurred in 10 patients (11.5%), and vocal cord paralysis occurred in 22 patients (25.3%). However, after adjusting the feeding conditions based on the results of the swallowing assessment, PPCs after meal initiation occurred in only five of these 10 patients (5.7%). Conclusions: Although dysphagia after esophagectomy is observed in approximately half of the patients, including those with minor dysphagia, the risk of aspiration can be reduced by changing the patient's posture during food consumption. Thus, postural changes may be a useful approach for reducing the risk of PPCs.

    OBJECTIVES: Phase angle (PhA), derived from bioelectrical impedance analysis (BIA), is an indicator of cell membrane health. Low PhA values reflect poor cellular function and low muscle mass. However, consensus regarding the appropriate cutoff value of PhA remains insufficient, and its impact on outcomes after esophagectomy in patients with esophageal malignancies is not well studied. We aimed to investigate whether preoperative PhA is associated with postoperative complication risk and survival prognosis and whether PhA decrease during the surgical preparation period is a prognostic factor in patients with esophageal cancer. METHODS: This retrospective cohort study analyzed data from 194 patients who had undergone esophagectomy for esophageal malignancies. A PhA measured several days before surgery, with cutoff values of 5.0° for men and 4.2° for women, was used. The relationship between postoperative pneumonia and clinicopathological factors and between low PhA and postoperative outcomes and survival prognosis was investigated. The preoperative PhA decline and survival prognosis were analyzed in 134 patients whose PhA was measured twice before surgery. RESULTS: Overall, 93 and 101 patients were classified into the low and high PhA groups, respectively. A multivariate analysis showed that a serum albumin level lower than 3.5 g/dL and low PhA were independent risk factors for pneumonia (odds ratio [OR] = 3.40, P = 0.03; OR = 3.42, P = 0.03, respectively). The low PhA group exhibited significantly higher intraoperative fluid balance (6.7 versus 6.0 mL/kg/h, P = 0.01) and a higher proportion of patients who failed to achieve early mobilization on the first postoperative day (46 versus 32%, P = 0.04) than did the high PhA group. Multivariate analysis using a Cox proportional hazards model revealed that low PhA was a poor survival prognostic factor, independent of the clinical stage of esophageal cancer (hazard ratio = 2.61, P < 0.01). In patients whose PhA was measured twice preoperatively, a decrease in PhA during the preoperative period was a significant indicator of poor survival (hazard ratio = 2.59, P < 0.01). The group with a decrease in PhA during the preoperative period had significantly fewer steps than the group with an increase in PhA (6220 ± 2880 versus 8200 ± 2850, P < 0.01). CONCLUSIONS: Low PhA was a risk factor for postoperative pneumonia in patients who had undergone esophagectomy and was associated with poor survival prognosis. A decrease in PhA during the preoperative period was a significant poor prognostic factor. Increasing physical activity before surgery may lead to an increase in PhA. Thus, it is important to measure and evaluate PhA changes sequentially in patients with esophageal cancer.

    Background/Aim: Sarcopenia contributes to a poor prognosis in patients with esophageal cancer; thus, any clinical support that prevents loss of skeletal muscle mass preoperatively and postoperatively should be actively investigated. This study aimed to evaluate physical activity during the perioperative period and its impact on postoperative skeletal muscle mass. Patients and Methods: Sixty-two patients who underwent esophagostomy at the Hamamatsu University School of Medicine between 2019 and 2023 were evaluated. The physical activity (measured by the step count) of patients scheduled for esophagectomy was assessed preoperatively using a fitness tracker. The percentage change in skeletal muscle mass index (SMI) was calculated preoperatively and 6 months postoperatively. Factors associated with decreased SMI 6 months after esophagectomy were analyzed using multivariate analysis. Results: The median decrease in SMI was –6.2%. Multivariate analysis revealed that factors associated with the reduction of SMI were age &gt;69 years [odds ratio (OR)=7.21, 95% confidence interval (CI)=1.36-38.19, p=0.020], preoperative step count &lt;7,800 steps/day (OR=5.17, 95% CI=1.38-19.33, p=0.015), and postoperative step count &lt;2,400 steps/day (OR=3.55, 95% CI=1.01-12.45, p=0.048). Conclusion: A low perioperative step count and older age were significant risk factors for skeletal muscle loss in patients with esophageal cancer undergoing surgery. For patients with a low number of steps in the perioperative period or for older patients, interventions to increase the number of steps may prevent skeletal muscle loss.

    Abstract

    Aim

    Dysphagia often develops after esophagectomy. The geniohyoid muscle is involved in swallowing movements, but its significance in esophagectomy patients remains unclear. We investigated the relationship of preoperative geniohyoid muscle mass with post‐esophagectomy swallowing function.

    Methods

    We retrospectively analyzed 114 patients who underwent esophagectomy and gastric conduit reconstruction for esophageal malignancy. We evaluated preoperative geniohyoid muscle sagittal cross‐sectional areas (cm2) using computed tomography. Median values for each sex were considered as cutoff values. Dysphagia severity was assessed using the Penetration–Aspiration Scale (PAS) during video‐fluoroscopic swallowing studies performed 7–10 days postoperatively.

    Results

    The cross‐sectional area was significantly larger in males than in females (3.2 ± 0.7 vs. 2.4 ± 0.5, p &lt; 0.01: median in males: 3.2 cm2, and in females: 2.3 cm2). These values were used to define high and low cross‐sectional area groups. The cross‐sectional area correlated positively with grip strength (correlation coefficient (CC) = 0.530) and skeletal muscle index (CC = 0.541). Transthyretin levels (22.4 ± 6.8 vs. 25.4 ± 5.5, p = 0.03) and cross‐sectional area (2.6 ± 0.7 vs. 3.2 ± 0.8, p &lt; 0.01) were significantly lower in patients with (PAS score ≥6; 20%) than in those without aspiration during fluoroscopic swallowing studies. Recurrent laryngeal nerve palsy was significantly more frequent in those with than in those without aspiration during fluoroscopic studies (22% vs. 5%, p = 0.03). In the multivariate analysis, low cross‐sectional area and recurrent laryngeal nerve palsy were both independent risk factors for aspiration during swallowing studies (odds ratio = 3.6, p = 0.03 and odds ratio = 6.6, p = 0.02, respectively).

    Conclusion

    Preoperative geniohyoid muscle mass, evaluated using neck computed tomography, can predict dysphagia after esophagectomy.

    <title>Abstract</title><sec>
    <title>Background</title>
    Esophageal cancer surgery requires maintenance and enhancement of perioperative nutritional status and physical function to prevent postoperative complications. Therefore, awareness of the importance of preoperative patient support is increasing. This study examined the usefulness of using a diary in combination with a wearable fitness tracking device (WFT) in patients undergoing surgery for esophageal cancer.


    </sec><sec>
    <title>Methods</title>
    Ninety-four patients who underwent esophagectomy between February 2019 and April 2021 were included. Physicians, nurses, dietitians, and physical therapists provided diary-based education for the patients. In addition, a WFT was used by some patients. The perioperative outcomes of patients who used both the diary and WFT (WFT group) and those who used the diary alone (non-WFT group) were compared. In addition, propensity score matching was performed to improve comparability between the two groups.


    </sec><sec>
    <title>Results</title>
    After the propensity score matching, the rate of postoperative pneumonia was significantly lower in the WFT group (0% vs. 22.6%, <italic>P</italic> = 0.005). The postoperative hospital stay was shorter in the WFT group <italic>(P</italic> = 0.012). Nutritional status indices, such as the prognostic nutritional index, also improved significantly in the WFT group at 1 month after surgery <italic>(P</italic> = 0.034). The rate of diary entries was significantly higher in the WFT group (72.3% vs. 28.3%, <italic>P</italic> &lt; 0.001).


    </sec><sec>
    <title>Conclusion</title>
    The use of a WFT reduced the incidence of postoperative pneumonia and improved postoperative nutritional status and rates of diary entries after esophagectomy, suggesting that its use may be useful for promoting recovery after esophagectomy.

    BACKGROUND: The correlation between perioperative changes in nutritional status during esophagectomy and prognosis remains unclear. This study aimed to evaluate the impact of changes in prognostic nutritional index levels during the perioperative period on esophageal cancer patient survivals. METHODS: From January 2009 to May 2019, 158 patients with esophageal squamous cell carcinoma were enrolled. From the time-dependent ROC analysis, the cutoff values of preoperative and postoperative prognostic nutritional index levels were 46.9 and 40.9. Patients were divided into preoperative-high group (Group H) and preoperative-low group (Group L). Then, patients in Group L were divided into preoperative-low and postoperative-high group (Group L-H) and preoperative-low and postoperative-low group (Group L-L). Long-term outcomes and prognostic factors were evaluated. RESULTS: Patients in Group L had significantly worse overall survival than those in Group H (p = 0.001). Patients in Group L-L had significantly worse overall survival than those in Group L-H (p = 0.023). However, there was no significant difference in overall survival between Groups H and L-H (p = 0.224). In multivariable analysis, advanced pathological stage (hazard ratio 10.947, 95% confidence interval 2.590-46.268, p = 0.001) and Group L-L (hazard ratio 2.171, 95% confidence interval 1.249-3.775, p = 0.006) were independent predictors of poor overall survival. CONCLUSIONS: Patients in Group L-H had a good prognosis, similar to those in Group H. This result indicated that increasing the postoperative prognostic nutritional index level sufficiently using various intensive perioperative support methods could improve prognosis after esophagectomy in patients with poor preoperative nutritional status.

    本研究では、食道切除術を予定する食道癌患者を対象に、サルコペニア予防を目的とした手術前から術後長期間のモバイルヘルスプログラムの開発をめざしている。本研究は以下の3項を主旨とする。
    ①モバイルヘルスシステムの開発。これまで食道切除術を受ける患者に対して用いていた治療日記をタブレット端末専用のアプリとし、ウェアラブルデバイスから得られる身体データ（歩数、脈拍、睡眠状況等）と同期することで、患者の状況と身体状況が即時に総合的に検討できるシステムの開発。②モバイルヘルスシステムを用いた介入によるフレイル、サルコペニア予防の効果の検討。③モバイルヘルスプログラムを用いた介入による長期予後やQ O Lに与える影響の検討。
    2021年度はモバイルヘルスシステムの開発を進めた。治療日記のアプリはプロトタイプ版のアプリが完成し、ウェアラブルデバイスから得られたデータのクラウドへの移行、治療日記アプリへの同期が可能となった。治療日記アプリの内容はこれまで用いていた身体状況やリハビリ状況に加え、心理状態に関する項目も加えた。食事の項目に関しては、食事摂取量だけではなく、食事内容が把握できるように食事の写真をアップロードできるようなシステムとした。食道切除術を受ける患者は50-70歳代の男性が多く、患者がより操作しやすい工夫が必要であると考えられるため、アプリの修正・改善点について検討中である。また、治療日記アプリ、ウェアラブルデバイスから得られたデータは多職種で活用していく予定であるため、医師・理学療法士・管理栄養士も試用し評価中である。

    本研究は、ACP等を通じて自らの意向をあらかじめ示せる人だけではなく、示せない人についても、尊厳ある生を最期まで送ることができるように、在宅医療・介護現場において人間の尊厳を守る話し合いを実現・促進するための具体的な方策を示すことを目的としている。
    初年度である今年度は、計画に沿って、主として以下の三点を実施した。
    一つ目は、ドイツ臨床倫理学における「尊厳」概念の検討である。とりわけ尊厳の概念を重視する「再構成的臨床倫理のマインツモデル」を検討した。
    二つ目は、前者のプロセスで明らかになった尊厳概念を、経験的な尊厳概念と比較検討する作業である。経験的尊厳概念に関しては、カナダの精神科医であるハーヴェイ・チョチノフによって示されたディグニティ・モデル（DM）を対象とした。DMは、カナダ人進行がん患者に対するインタビュー調査にもとづくものであり、患者本人が感じる「尊厳感覚」を中心にしており、尊厳感覚に影響を与える要素が詳細に示されている。しかし同時に、患者の視点を中心にしているために、ドイツの臨床倫理学において問題となっていた存在・生命や他者との関係という意味での尊厳が十分に捉えられていない。これらの尊厳を明らかにするには、尊重する側の視点から理解された尊厳に関して、インタビューを実施する必要が示唆された。なお、DMを検討する上では、DMにもとづいて考案されているディグニティ・セラピーを国内で実施している医療従事者にもインタビューを行った。
    三つ目は、在宅医療・ケア従事者を対象にした調査の計画立案および倫理委員会への申請である。

    We have developed a short-term preoperative intensive exercise nutrition intervention program aimed at increasing skeletal muscle mass in patients with esophageal cancer and esophagogastric junction cancer who are scheduled for esophagectomy. In this study, height, weight, blood test, skeletal muscle mass measurement (impedance method), body composition measurement, Cardio Pulmonary Exercise test (CPX), grip force, walking speed, and iliopsoas muscle area (L3) were measured before and after the intervention program. As an exercise program, we grasped the individual's exercise tolerance and performed exercise therapy that combined resistance exercise and aerobic exercise. In addition, the content and intake of meals were recorded on a record sheet, and a registered dietitian continuously evaluated and provided guidance. The analysis is performed when the planned case is reached.

    本研究の全体構想は、胸部食道がん術後長期サバイバーの日常生活上の困難の実態とQOL
    との関連について明らかにし、胸部食道がん術後長期サバイバーに対する日常生活支援看護モデルを構築することである。本研究における術後長期サバイバーとは、胸部食道がんの標準治療である頸部・胸部・腹部の3領域リンパ節郭清術を伴う食道切除術を受け、術後5年以上経過している者をいう。なお、本研究においては、再発・転移の有無は問わないこととする。
    2019年度に岡山大学に着任し、浜松医科大学病院でのデータ収集に加え、岡山大学病院をデータ収集フィールドに追加する予定で、調整を行っていたが、新型コロナウイルスの蔓延により、浜松医科大学病院への移動も不可能となり、加えて、岡山大学病院への立ち入りが制限される期間があり、研究フィールドの調整が完了していない状況である。2019年度より新型コロナウイルスの蔓延により、研究の進捗状況が遅れている状況である。2019年度までに、研究対象者に面接を行い、胸部食道がん術後長期サバイバーの日常生活上の困難の実態について明らかにする予定であったが、新型コロナウイルスの蔓延により、面接調査が完了していないため、2020年度に実施予定だった、胸部食道がん術後長期サバイバーの日常生活上の困難を定量的に評価するための尺度開発が行えていない。現在、胸部食道がん術後長期サバイバーの日常生活上の困難の実態について明らかにするための面接調査を継続途中であるが、新型コロナウイルスの蔓延により、面接を中止しているところである。
