    Background/Objectives: Halitosis is a condition characterized by an unpleasant malodor. Intra-oral halitosis is caused by volatile sulfur compounds (VSCs) and can be associated with oral dryness. Trehalose is one of the materials used to relieve oral dryness. The aim of the present study was to investigate the effect of trehalose on halitosis. Methods: This prospective, double-blinded, placebo-controlled, cross-over study enrolled volunteers from Okayama University Hospital. The participants were randomly divided into two groups, with one group receiving trehalose (a 10% trehalose solution) and the other receiving a placebo (distilled water) in a 1:1 allocation. The primary study outcome was the subjective organoleptic test. The secondary outcomes were the concentrations of the VSCs, which were measured using a portable gas chromatography device, and the oral moisture status, which was measured using an oral moisture meter. The planned sample size was 10 participants based on the previous study. Results: The final intention-to-treat analysis was performed using the data from 9 participants. After applying 10% trehalose as an oral spray, the organoleptic score decreased in a time-dependent manner. However, no significant differences were seen between the trehalose and placebo groups. In terms of secondary outcomes, the oral moisture levels increased immediately after the trehalose spray application, and significant differences in the amount of change from the baseline were seen between the trehalose and placebo groups (p = 0.047). No significant differences were seen in any of the other variables (p > 0.05). Conclusions: We could not identify any positive effects on halitosis from a one-time 10% trehalose application as an oral spray in this prospective, double-blinded, placebo-controlled, cross-over study. However, the trehalose application immediately improved the oral moisture levels and was useful for treating oral dryness.

    Background: A high prognostic nutritional index (PNI) is associated with good prognosis in patients with esophageal cancer. However, nutritional status often decreases during neoadjuvant therapy. Functional tooth units (FTUs) provide an index for the status of posterior occlusal support. We have previously reported that low PNI is related to low FTUs. Objectives: The purpose of this study was to retrospectively examine whether the status of occlusal support relates to changes in PNI during neoadjuvant therapy in patients with esophageal cancer. Methods: This study included 34 patients who underwent neoadjuvant therapy before esophagectomy (32 men, 2 women; age, 36-82 years) in 2012 at Okayama University Hospital. Patients were divided into the good occlusal support group (FTUs ≥ 11, n = 18) or poor occlusal support group (FTUs < 11, n = 16), and changes in PNI during neoadjuvant therapy were investigated. Results: PNI decreased significantly after neoadjuvant therapy, particularly in the good occlusal support group, and became more dispersed after neoadjuvant therapy. Decreases in PNI after neoadjuvant therapy showed a significant positive correlation with good occlusal support by multiple regression analysis (p = 0.03). The proportions of patients provided with nutritional intervention (p = 0.02) or early dental intervention (p = 0.04) were lower in the good occlusal support group than in the poor occlusal support group. Conclusions: Even in patients with esophageal cancer with good occlusal support experienced significant declines in PNI during neoadjuvant therapy, potentially due to delayed nutritional and dental interventions. Early multidisciplinary interventions are thus recommended for all patients, regardless of preoperative dental or nutritional status.

    PURPOSE: Treatment for head and neck cancer (HNC), such as surgery and chemoradiotherapy, can reduce oral function and affect quality of life (QoL). However, whether HNC treatment affects QoL via the decline of oral function remains unclear. This study aimed to investigate the relationship among cancer treatment, QoL, and actual oral function in HNC survivors. METHODS: A total of 100 HNC survivors who had completed definitive treatment for HNC at least 6 months prior to enrollment were enrolled in this cross-sectional study. QoL was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 summary score. Oral diadochokinesis (ODK), tongue pressure, moisture level on the mucosal surface, and mouth opening were measured. Information on age, sex, tumor site, tumor stage, history of HNC treatment, height, body weight, and lifestyle were collected from medical records. Structural equation modeling (SEM) was conducted to analyze the indirect/direct associations among HNC treatment, QoL, and oral function. RESULTS: In total, 100 HNC survivors (58 males and 42 females; age range, 30-81 years, median, 67 years) were analyzed. Overall, 63 patients (63.0%) were diagnosed as oral cancer, 66 (66.0%) developed advanced cancer (stage 3/4), and 58 (58.0%) underwent reconstruction surgery in 100 HNC survivors. The SEM results supported the hypothesized structural model (root mean square error of approximation = 0.044, comparative fit index = 0.990, Tucker-Lewis index = 0.986). Surgery with neck dissection and reconstruction for advanced cancer had indirect effects on lower QoL via ODK and mouth opening. CONCLUSION: HNC treatment is indirectly associated with QoL via oral function in HNC survivors.

    Abstract

    Tongue pressure (TP) decreases significantly after esophagectomy in esophageal cancer patients (ECPs). Meanwhile, 2 weeks of gum-chewing training (GCT) significantly increased TP in healthy university students. We examined whether perioperative GCT would decrease the proportion of patients exhibiting a decline in TP at 2 weeks postoperatively, and prevent postoperative complications, in thoracic ECPs (TECPs). This was a single-center interventional study, and nonrandomized study with a historical control group (HCG). TECPs who underwent first-stage radical esophagectomy were recruited. Thirty-two patients of 40 in the gum-chewing group (GCG) were completed perioperative GCT in 3 times daily. Propensity score matching was performed with covariates related to TP including preoperative age, sex, body mass index, and the repetitive saliva swallowing test result, and yielded a matched cohort of 25 case pairs. Eleven GCG patients [44.0%] exhibited significantly lower TP at 2 weeks postoperatively than before esophagectomy was significantly fewer than that of 19 patients [76.0%] in the HCG. The median number of fever days (&gt; 38 °C) in the 2 weeks after esophagectomy in the GCG was significantly fewer than those in the HCG. Perioperative GCT may prevent postoperative TP decline and postoperative dysphagia-related complications after esophagectomy.

    BACKGROUND: Perioperative oral management contributes to the prevention of dental/systemic complications. However, a professional dental checkup before surgery is generally not performed and relies on the patient's answer to a simple question by medical professionals other than dentists: "Do you have any concerns regarding your mouth related to undergoing surgery?" Here, we evaluated the sensitivity and specificity of this question for predicting perioperative oral health problems in patients with primary esophageal and primary lung cancer. METHODS: We performed an oral cavity check in all patients before scheduled surgery for primary esophageal and lung cancer. A total of 183 patients were enrolled (M, 112; F, 71; 24-88 years, median, 69 years), consisting of 61 with primary esophageal cancer (M, 46; F, 15; 24-85 years, median, 69 years) and 122 with primary lung cancer (M, 66; F; 56; 33-88 years, median, 69 years). All subjects provided a response to this question, and an oral cavity check was performed by dentists. The sensitivity and specificity of this question for detecting oral health problems were evaluated retrospectively. RESULTS: Overall sensitivity and specificity for detecting oral health problems were 0.263 and 0.898, respectively. There were no significant differences by sex or disease (primary esophageal or lung cancer). CONCLUSION: This simple question has low sensitivity but high specificity for detecting oral health problems. Although challenging to detect surgical patients with oral health problems by simply asking questions, the results indicated that patients with oral complaints are more likely to have problems during surgery.

    OBJECTIVES: An increasing number of patients visiting the dental office have a growing need for perioperative oral management (POM) to prevent postoperative complications. Therefore, it is necessary to determine which patients should receive preferential POM. This study investigated the dental status of patients scheduled to undergo surgery and addressed the priority for POM. METHODS: This retrospective study included a total of 150 patients who were scheduled to undergo surgery at the Department of Respiratory Surgery (DRS), Department of Neurological Surgery (DNS), Department of Gynecological Surgery (DGS), Department of Breast and Endocrine Surgery (DBES), and Department of Esophageal Surgery (DES) managed by the Perioperative Management Center of Okayama University Hospital. We compared the general and dental status of patients among the five groups. RESULTS: The DES group had significantly fewer teeth than the DBES group (p = 0.012), more severe periodontitis than both the DBES (p = 0.005) and DNS groups (p = 0.020), and poorer molar occlusal support status than both the DBES (p = 0.002) and DGS groups (p = 0.041). The DES group exhibited a significantly higher median age (p = 0.002), a higher ratio of males (p < 0.001), a higher prevalence of malignant tumors (p < 0.001), and higher proportions of smokers (p < 0.001) and drinkers (p < 0.001) than the other groups. CONCLUSION: Patients who underwent surgery at the DES had more dental problems than other surgery patients. Accordingly, these patients should be given the highest priority for POM triage.

    BACKGROUND: Multidisciplinary team (MDT) approach has become a standard for perioperative patient care, including in esophageal cancer. In our institution, the Perioperative Management Center (PERiO) has been doing an MDT approach for patients undergoing esophageal cancer surgery since 2009. On the other hand, neoadjuvant therapy has also been becoming standard for many malignancies, including esophageal cancer. In Japan, neoadjuvant chemotherapy (NAC) for esophageal cancer is standard now. However, there have been no reports about when is the best time to start the MDT approach for patients with neoadjuvant therapy. In this study, the best start time for the MDT approach for esophageal cancer patients with NAC was examined from the perspective of adverse events during chemotherapy and perioperative period. METHODS: All cases underwent thoracoscopic esophagectomy in the prone position (TEPP) after NAC. The PERiO Intervention group that started before NAC (n = 100) was compared with the PERiO Intervention group that started after NAC (n = 77). Eventually, 54 paired cases were matched by propensity score matching. RESULTS: The adverse event rate during chemotherapy, especially oral complications, was significantly decreased in the PERiO Intervention started before the NAC group (P = 0.007). Furthermore, weight loss during the period from chemotherapy to surgery was significantly reduced in the group that started before NAC (P = 0.033). CONCLUSION: The MDT approach should be started before NAC in patients undergoing esophageal cancer surgery to prevent adverse events during chemotherapy and provide safe perioperative conditions.

    BACKGROUND: The Prognostic Nutritional Index is useful for predicting surgical risk and overall survival based on preoperative immunological and nutritional status in patients undergoing digestive organ cancer surgery. The purpose of this study was to examine the association between the Prognostic Nutritional Index and dental status in patients with esophageal cancer who underwent esophagectomy. METHODS: This retrospective case-control study included 73 patients who underwent resection of esophageal cancer (69 males, 4 females; age 36-83). General and dental status were evaluated. The Prognostic Nutritional Index was calculated based on the serum albumin concentration and the total lymphocyte count, and subjects were divided into two groups based on index scores: a higher group, characterized by scores ≥ 45 (n = 54); and a lower group, characterized by scores < 45 (n = 19). Univariate analysis and multiple logistic regression analyses were used to compare between groups. RESULTS: Total protein, C-reactive protein, the number of sound and total decayed, missing and filled teeth, and the rate of patients with poor dental occlusal support showed significant differences between the lower and higher Prognostic Nutritional Index groups (p < 0.05). Stepwise logistic regression analysis by backward selection approach showed that low total protein, few sound teeth, and poor status of dental occlusal support were significantly associated with the lower Prognostic Nutritional Index (p = 0.007, 0.042, and 0.009, respectively). CONCLUSION: Dental status, especially dental occlusal support and the number of sound teeth, showed a positive relationship with the Prognostic Nutritional Index in esophageal cancer patients who underwent esophagectomy.

    The purpose of this study was to investigate the oral health status of Japanese university students and evaluate the relationship between periodontal health and related factors using a self-reported questionnaire. Undergraduate students (n=2,422) at Okayama University, aged 18-28 years, received oral examinations in 2008. Periodontal examination was conducted according to the protocol of the United States National Health and Nutrition Examination Survey III. The results showed that the number of teeth present was 28 (22-32) [median (range)]. The percentage of subjects with decayed teeth was 13.6%. Subjects with periodontal disease (at least one tooth with a probing pocket depth &ge;4mm) comprised 4.9%. According to the questionnaire survey, the prevalence of temporomandibular joint (TMJ) sounds was 26.5%; TMJ pain, 8.0%; and mouth opening limitation, 8.9%. The percentage of subjects with repeated stomatitis was 18.7%. There were gender differences in the number of decayed teeth, periodontal condition, and complaints of symptoms of temporomandibular disorders. There was a significant association between periodontal health and the following four questions: "Do you visit a dental office regularly?", "Do you notice bloody gums?", "Do you think that you have gingival swelling?", and "Do you think you have periodontal disease?"

    2007年10月〜2008年7月に耳鼻咽喉科または形成外科に入院し、カンファレンスで取り上げられた頭頸部癌チーム医療関連入院患者140名(男103名、女37名)を対象に予防歯科の関わりについて検討した。入院患者は月平均14名で、男女共に50歳代〜70歳代が多く、60歳代がピークであった。予防歯科に紹介された入院患者は32名であった。癌の部位別では上下歯肉癌が51.9%と最も多く、約半数の患者は補綴科に紹介された。唾液腺癌や甲状腺癌患者の予防歯科紹介は無く、喉頭癌患者の紹介は少なかった。治療法別では化学療法と手術併用療法が13名(40.6%)と最も多く、次いで手術のみが12名(38%)であった。治療開始日から紹介日までの期間は化学療法で平均6.5日後、放射線療法では17日後であり、手術適応患者では単独、併用療法ともに平均15日後に紹介されていた。

    Teenagers are particularly susceptible to gingivitis, and its prevalence increases with age. We proposed the use of electric toothbrushes for maintaining gingival health in high school students. Thus, we evaluated the relative ability of the oscillating/pulsating electric toothbrush Oral-B[○!R] ProfessionalCare[○!R] 8000 (PC) and side-to-side motion electric toothbrush Sonicare[○!R] elite[○!R] e9800 (SE). Moreover, this study investigated the perceived comfort and safety of and preference for electric toothbrushes of high school students using a questionnaire, designed to evaluate the characteristics of the two powered toothbrushes. Dental examination was performed at a high school in all 1st-3rd grade students (15-18 years old, 956 students). Sixty-five students were diagnosed with gingivitis. All subjects were informed about the aims of the study, and 59 gave signed, informed consent. Among the 59 students who entered the study, 48 completed the protocol. Eleven subjects were excluded from the study because they were wearing fixed orthodontic appliances (3 subjects), using antibiotics (3 subjects), or showed poor brushing compliance (5 subjects). This study followed a matched, parallel, examiner-blind design. Subjects were scored regarding the probing depth, bleeding on probing (BOP), and the Turesky et al. modification of the Quigley & Hein index (PlI) in 10 index teeth (17,16,11,26,27,37,36,31,46,47) at the baseline. The PC and SE groups were matched by school grade, sex, the percentage of BOP, PlI, and probing depth. Subjects were instructed to brush twice daily for 2 min without dentifrice. They were given professional instructions for approximately 5 min on the use of either toothbrush. The probing depth, percentage of BOP, P1I, and gingival abrasion were recorded after 2, 4, and 8 weeks. At the end of the study (after 8 weeks of brushing), all subjects completed a questionnaire. Statistical analysis was performed using the Mann-Whitney U test for non-parametric, unrelated samples, multiple comparisons test for non-parametric, related samples, and Fisher's exact probability test for subjective symptoms. Overall, there was no significant difference between the two groups in any indices. During the study period, the multiple comparisons test showed a significant improvement in all indices for both powered toothbrushes. There were 4 sites of gingival abrasion in PC and 0 in SE. At 8 weeks, no gingival abrasion was observed in either group. The number of subjects who exhibited bleeding on brushing and oral malodor in both powered toothbrush groups decreased from the baseline to 8 weeks. Some users felt that the head and sound of both electric toothbrushes were too large. In conclusion, PC and SE improved the percentage of BOP, PlI, and probing depth in high school students with gingivitis. The use of both electric toothbrushes can be considered safe untill 8 weeks. PC and SE had almost the same effects on gingivitis in high school students. For prospective electric toothbrushes, a smaller brush head and more pleasant sound while operating would be desirable.

    症例1:52歳男性。舌癌で放射線治療適応となり、組織内照射約70Gyを行った後、口腔乾燥を訴えるようになった。ガムテストでは6.2cc/10min.であった。放射線治療後の口腔乾燥症として塩酸ピロカルピン15mg/day投与と、齲蝕予防のために1〜2ヵ月に1回のフッ素塗布、連日のフッ素洗口を開始した。さらに家庭においてフッ化物配合歯磨剤を用いたブラッシングを勧めた。症例2:63歳男性。舌癌で放射線治療を希望し、外照射20Gyの後、組織内照射約70Gyを行った後、口腔乾燥を訴えるようになった。ガムテストでは5cc/10min.であった。放射線治療後の口腔乾燥症として塩酸ピロカルピン15mg/day投与と、齲蝕予防のために3週間に1回のフッ素塗布を開始した。さらに家庭においてフッ化物配合歯磨剤を用いたブラッシングを勧めた。症例1では自覚的にも他覚的にも口腔乾燥を脱し、副作用発現なく投与10週後に中止した。症例2では副作用の頻尿感などで投与10日後に中止となったが、2週間後には自覚的にも他覚的にも口腔乾燥感の改善がみられた。ともに齲蝕のコントロールは良好である。

    Epidemiological evidence exists for an association between vitamin C intake and periodontal health. Understanding of the effects of vitamin C on periodontitis is important to improving periodontal therapy. We studied the effects of topical application of a dentifrice containing vitamin C on periodontitis in the rat model. Periodontitis was induced in 8-week old male Wistar rats (n=50) with a combination of lipopolysaccharide and proteases. After 4 weeks, rats were divided into 5 groups, receiving topical application of dentifrice containing 0 (periodontitis group) , 1, 2, 3, or 5% vitamin C for 3 weeks. Collagen density increased and polymorphonuclear leukocyte number decreased with increasing concentration of vitamin C, and the 3% and 5% vitamin C treatment groups differed significantly from the periodontitis group in this regard. Gingival expression of 8-hydroxydeoxyguanosine (a marker of oxidative stress) was lower in the 1, 2, 3, or 5% vitamin C treatment groups than in the periodontitis group. These results show that topical application of a dentifrice containing&ge;3% vitamin C to the gingival margin decreases polymorphonuclear leukocyte numbers and induces collagen synthesis in rat periodontitis.     The objective of this study was to compare the effects of four pastes-regular toothpaste as a control (CT), regular toothpaste containing either dipotassium glycyrrhizinate (GLY), dipotassium glycyrrhizinate with Create Ion^[○!R] High Power (HP), or dipotassium glycyrrhizinate with Create Ion^[○!R] Number One (No 1), on periodontitis in the rat model. In addition, the effects of mechanical stimulation of gingiva when combined with the pastes were also evaluated. Under general anesthesia, lipopolysaccharide and bacterial protease solution were applied once a day into the palatal gingival sulcus of the upper right and left first molars of 40 male Wistar rats to induce periodontitis for 8 weeks. From the 5th to 8th week, 4 toothpastes (n=10) were applied in the gingivae of the right and left maxillary first molar once per day for 10 minutes respectively. Also, the palatal gingiva of the left molar was stimulated once a day using a power toothbrush. After 8 weeks, the teeth and gingivae were fixed, decal-cified and embedded in paraffin. Buccolingual sections were evaluated using hematoxylin-eosin staining. Statistical differences among the 4 groups and between stimulated and non-stimulated sides were tested by the ANOVA or Kruskal-Wallis test, and the Student's t-test or Mann-Whitney test, respectively. In the No 1 group, the number of polymorphonuclear leukocytes in the junctional epithelium was significantly decreased. In the HP group, the number of polymorphonuclear leukocytes in connective tissues was significantly less than that in the other groups. The stimulated side had a lower number of polymorphonuclear leukocytes in the junctional epithelium and connective tissue than the non-stimulated side in the GLY group, and the values were similar to those for the stimulated sides of the HP and No 1 groups. In conclusion, Create Ion^[○!R] High Power and Create Ion^[○!R] Number One with dipotassium glycyrrhizinate reduces polymorphonuclear leukocyte infiltration into connective tissues and junctional epithelium, respectively. Mechanical stimulation of gingiva combined with dipotassium glycyrrhizinate reduces polymorphonuclear leukocytes in junctional epithelium and connective tissue as strongly as dipotassium glycyrrhizinate and Create Ion^[○!R] without mechanical stimulation.

    ブラッシングの機械的刺激が,ラット歯肉の接合上皮基底細胞と線維芽細胞をDNA合成期(S期)に進行させて細胞分裂を促進しているか確認し,さらに細胞周期の変化をこれまでの研究に用いられたproliferating cell nuclear antigen(PCNA)とS期の細胞を特異的に検出できるbromodeoxyuridine(BrdU)の2種の細胞増殖マーカーを用いて検討した.ブラッシングによる機械的刺激は,接合上皮基底細胞と結合組織の線維芽細胞のDNA合成を促進した.機械的刺激により,上皮基底細胞ではS期以外,特にDNA合成準備期の細胞が増加した.接合上皮基底細胞と結合組織の線維芽細胞ともに,BrdU陽性率とPCNA陽性率の間に有意な相関はなかった

    Preoperative oral care is helpful to prevent postoperative complications in patients who are undergoing esophagectomy. Here, we report the case of an 81-year-old Japanese man with an upper limb disability caused by post-polio syndrome who was receiving neoadjuvant chemotherapy for esophageal cancer. He had poor oral health status and developed oral complications as a side effect of chemotherapy. He could not brush his teeth by himself. However, infection control by oral care provided by an interprofessional collaboration successfully improved his oral hygiene, and his follow-up involved no severe complications. Interprofessional collaboration is useful especially for patients with upper limb disability.

    <p> Abstract Okayama University Dental School implemented an outpatient escort practicum as a form of early clinical exposure. It is useful to clarify how students perceived the practicum after they finished their clinical training to inform improvements. The purpose of this study was to evaluate an outpatient escort practicum implemented with first-grade students from the perspective of those students after they had completed clinical training.</p><p> The evaluation was performed twice, with the same students completing the questionnaires. The first evaluation was conducted immediately after the outpatient escort practicum in the students' first grade, and the second after they had completed their clinical training (sixth grade). The participants were 82 dental students who completed both questionnaires about the outpatient escort practicum.</p><p> Overall, the second evaluation was significantly lower than the first evaluation. Students who positively evaluated the practicum in their first grade but gave a negative evaluation in their sixth grade reported less appreciation for the practicum in terms of understanding the patients' perspectives and understanding the communication skills required by future healthcare professionals. Students who positively evaluated the practicum both times highlighted that the practicum helped them to understand the communication skills required by future dentists. The study findings indicated that the practicum was more positively evaluated by students immediately after they had completed it than by the same students 5 years later. However, the practicum was positively evaluated by the participating students after they had completed clinical training to some extent.</p>

    本研究では、有効な治療薬剤がなく予後不良なCOPD合併肺がんについて、微生物叢の破綻（ディスバイオーシス）を標的とした新たながんの治療法の開発に挑む。COPD合併肺がん患者について、細菌やウイルス、真菌を含む種の多様性とそれらの機能的役割を検討可能な微生物叢解析（ショットガンメタゲノム解析）を臓器横断的に行い、それぞれの関連や機能を明らかにすることで、難治性肺がんにおいてディスバイオーシスが果たす役割を解明する。
    初年度は、肺がん周術期に採取し岡山大学病院バイオバンクに保管したCOPD合併・非合併肺がん患者の口腔内・腸内検体、および手術で採取した非がん部および肺がん部の肺組織について、微生物叢解析（ショットガンメタゲノム解析）を行った。同定された微生物については、PCR等によって適宜確認を行うとともに、メタゲノム解析結果と既往症を含む患者背景、肺がんの臨床病理学的特徴との関連についても検討を加え、生体内における微生物叢の多様性・不均一性などについて臓器横断的に比較を行った。

    本研究は、歯科治療が大腸がんの重症化予防に寄与するか、を検討する研究である。
    歯周病原菌の一つであるFusobacterium nucleatum (F. nucleatum) は、大腸がんの発がんや悪性度増強に寄与しているとされ、様々な研究が行われている。
    大腸がん組織におけるF. nucleatumの供給源は口腔内である。大腸がん患者の唾液とがん組織で共通のF. nucleatum株が検出され、大腸がん組織のF. nucleatumが口腔内に由来することが示された（Komiya et al., GUT, 2018）。また、大腸がん患者の腸内細菌叢や口腔細菌叢はバイオマーカーとして有用と考えられている（Yachida et al., Nature Medicine, 2019、Flemer et al., GUT, 2018）。さらに、大腸がん患者に対して歯周治療を手術前の3か月間行い、歯周状態改善群と非改善群で比較したところ、歯周状態改善群では糞便中F. nucleatumが減少したことから、歯周治療が大腸がんの発症・重症化予防に役立つ可能性が示された（Yoshihara et al., SciRep, 2021）。一方、この研究では、歯周状態改善群において唾液中のF. nucleatumは減少しなかったというパラドックスがある。
    本研究では、歯肉縁下歯垢中のF. nucleatumやリポ多糖（LPS）等の構成成分が、血流を介して大腸がん組織に作用し、大腸がんの悪性化に寄与しているのではないか、と考え、歯周治療が血流を介したF. nucleatumやその構成成分の大腸がんへの促進的作用をブロックし、大腸がん重症化予防に寄与する可能性があるのか、を検討する。
    2023年度は、研究計画の立案、倫理委員会への提出書類の作成、細菌叢の解析の方法の検討等を行った。書籍の購入、学会参加、分析に必要な機器の準備、情報収集等を行った。
    今年度は、倫理委員会への申請、承認を経て、研究を開始する予定である。

    Japan is a super-aging society, with about 30% of its citizens already aged > 65 years. As a result, an increase in older HNC patients and survivors is expected in Japan.
    The findings of this study suggest that returning to wark is associated with QoL of pain and swallowing in HNC survivors. The need rehabilitation to improve oral function may be increasing after HNC treatment to return to work.

    In the thoracic esophageal cancer patients, perioperative gum chewing training prevented decreased tongue pressure 2 weeks after esophagectomy. Furthermore, tongue pressure at 2 weeks after esophagectomy was increased compared to before esophagectomy in most of the patients who performed perioperative gum chewing training. These results suggest that perioperative gum chewing training in thoracic esophageal cancer patients may prevent postoperative decrease in tongue pressure and progression of postoperative frailty.

    The aims of this study were to investigate the risk factors for decreased quality of life (QOL) and the effects of change in oral function on QOL in head and neck cancer (HNC) patients. The changes in oral functions, body weight and QOL for a year were evaluated. Structural equation modeling revealed that patients who kept body weight and increased tongue pressure had good oral function and good QOL. Our results suggest that keeping body weight and receiving oral rehabilitation are related with the improvement of QOL in HNC patients.

    In the esophageal cancer patients who underwent esophagectomy, we investigeted that the relationship between oral health status and prognosis index, the status of oral flora during neoadjuvant chemotherapy, the relationship between oral health status and nutritional status after surgery.
    In consequence, these were suggested that the significant relationship between prognosis index and occlusal support, a tendency for decreased biodiversity in the patients who had oral mucosal disorder during neoadjuvant chemotherapy, and possibility of the positive relationship between exacerbations of nutritional status after surgery and occlusal support.

    To promote multi-disciplinary team approach for perioperative oral management, we considered an efficient method using the simple self-check sheet, consisted a sentence “Do you have concerns with your mouth when undergoing surgery?”, which developed to assess requirements of professional dental interventions. To evaluate of the availability of the developed simple self-check sheet, the sensitivity and specificity were investigated. The results showed the simple self-check sheet had low sensitivity but high specificity. Therefore, patients, with some oral concerns by simple self-check sheet, should be examined and managed by dental professionals, at least. And routine pre-examination under general anesthesia by dental professionals is desirable for surgical safety, because of low sensitivity.

    The purpose of the study is to investigate 1) the relationship between acetaldehyde concentration and microbiome on tongue, and 2) the related factors of tongue pressure decreasing and the effect of tongue excise. The relative abundance of Neisseria flacescens in high acetaldehyde group were significantly greater than those in the low group. The decline of tongue pressure significantly associated with length of intensive care unit (ICU) stay in patients after esophagectomy. The tongue pressure of patients who performed tongue excise were not significantly difference compared with no excise patients. In conclusion, the concentration of acetaldehyde in mouth air was related with Neisseria flavescens. The change of tongue pressure was associated with the length of ICU stay and was not effective from tongue excise in the patients after esophagectomy.

    The purpose of this study was to clarify the relation between attitude about visiting dental clinic and demand of dental treatment before surgery, and to clarify oral characteristics of the patients in the department of Digestive tract surgery, Cranial nerve surgery, Respiratory surgery, Gynecology, and Brest thyroid surgery. Other purpose of this study was, in the patients who undergo esophageal surgery, to investigate relation between oral status and a prognostic factor, and the change of a prognostic factor during neoadjuvant chemotherapy.
    As the results, the ratio of the patients who have to be treated before surgery was low in the patients who visit dental clonic at regular intervals, and the oral status of the patients who undergo Esophageal surgery was worse than that of the patients who undergo other surgery. The oral status in the patients who undergo Esophageal surgery was related with prognostic factor, and the change of a prognostic factor during neoadjuvant chemotherapy.

    The aim of this study was to investigate the relationship between acetaldehyde concentration in mouth air and tongue coating volume in healthy volunteers. Acetaldehyde concentration in participants with a tongue coating score of 3 was significantly higher than those with a score of 1. After removing tongue coating, acetaldehyde concentration was significantly decreased.
    Oral microflora on the tongue may be the main source of local production of acetaldehyde. Removing tongue coating may be a method inhibiting the production of acetaldehyde.

    Oral mucositis is one of side effect of radiation therapy and chemotherapy. The aim of this study was to clarify the preventive mechanism of oral mucositis from the view point of oxidative stress and microorganisms. As the results, oral mucositis had no relationship with oral microorganisms, oxidative stress in plasma and general status.
    Moreover, oral mucositis had relationshipwith frequency of radiochemotherapy and chemotherapy more than oxidative stress in plasma. It was showed that initiation and severity of mucositis was not influenced by oxidative stress in plasma.

    Our previous study demonstrated that topical application of bacterial pathogens, i.e., lipopolysaccharide (LPS) and bacterial proteases, to gingival sulcus induced fatty liver. The purpose of this research project was to clarify the mechanism of fatty liver in rat periodontitis model, especially, to investigate the relationship between LPS and reactive oxygen species (ROS).
    In the year 2006, serum levels of LPS, ROS and tumor necrosis factor (TNF)-alpha were found to be elevated in rats with periodontitis compared to rats without periodontitis. Moreover, both concentrations of 8-hydroxydeoxyguanosine (8-OHdG) in liver homogenate and densities of 8-OHdG-positive liver cells were larger in the rats with periodontitis than in those without These results suggest that topically applied ITS affects liver directly, that cells in the periodontium respond LPS and produce ROS, that the ROS produced in periodontium affects liver directly, and that cytokines play some roles in the pathogenesis of fatty liver induced by periodontitis.
    In the year 2007, the effects of high cholesterol diet on the markers of oxidative stress in blood and liver of rats with periodontitis were investigated. Serum levels of reactive oxygen metabolites and hexanoyl-lysine and liver level of 8-OHdG were increased after feeding high cholesterol diet in the rats with periodontitis. These results suggest that high cholesterol diet has additive effects on periodontitis-induced oxidative damage in blood and liver.
