In the near future, opportunities for dental treatment for the elderly requiring long-term care will increase, and its importance will increase significantly.
Therefore, in order to train dentists who can provide such dental care, we created a training scenario assuming dental care for the elderly requiring long-term care and repeatedly improved it. Scenarios were created for (1) wheelchair transfer / securing posture, (2) oral cleaning / conservative treatment training, (3) prosthetic treatment training, and (4) swallowing function test training. A questionnaire survey was conducted after training the trainee dentists based on these scenarios, and it was found that a significant improvement in knowledge and skills was observed, and that it was effective as a dental care education method in a super-aging society. -

This study was conducted to verify how the empathy of the trainee dentists and the medical interview skills change as the trainees gain clinical experiences during the clinical training period.
The trainee dentists conducted initial medical interviews with simulated patients (SPs) twice, at the beginning and the end of their clinical training. The trainees completed a questionnaire to measure empathy just before each medical interview. The SPs and clinical instructors evaluated the trainees’ communication using questionnaires immediately after the medical interviews. The videotaped dialogue from the medical interviews was analyzed. There was no change in the empathy of the trainee dentists in their one-year postgraduate training, and it was suggested that the trainees becoming more focused on collecting dental information for diagnosis in medical interviews. -

A training dentist always experiences an inexperienced matter and is learning newly by dental clinical education. I can think the incident a beginner experiences by basic dental technology is universal in the learning process, but every time a learner will change for an educator, great educational effort has to tell repeatedly, and is needed. A training dentist is clinical by this research, and own learning system that I'll can utilize others' experience as the experience has been developed by vicarious experience animation-izing the incident I face. Reduction in educational effort is planned for for an educator as well as a main system can expect that I contribute to safety's and relief's being able to execute odontology clinical education.

The text data of clinical records accumulated in the electronic portfolio system which we introduced into dental practice training education in Okayama University Hospital were collected and classified in every treatment. The clinical problems that dental practice beginners faced were analyzed using the text mining method. As a result, a lot of keywords related to dental procedures were listed in the item of "the impression" about contents of the treatment. On the other hand, there were many keywords related to the learning such as "study", "exercise" and "review" in the item of "the future task". Then, after sorting to reduce the quantity of the text data, they were refined and accumulated as knowledge data. An IT learning support system in clinical dentistry was produced experimentally using this knowledge database. When this system was evaluated, a relatively good evaluation was provided from trainee dentists.

We have built an electronic portfolio system as an active learning support system in the knowledge / attitude domain. In this research we aimed to construct a learning support system of the skill area. We created a demonstration video and released it on the electronic portfolio. We have developed a training environment using a simulator as a learning support system. We conducted a questionnaire survey for the trainee dentist and improved the environment. In addition, 13 items of evaluation in the skill area were formulated.

The aim of this study is to evaluate the efficacy of new knowledge databases system. We established knowledge databases system associated with periodontology in the 2013 academic year and educate dental residents on Postgraduate Clinical Training Course in Okayama University Hospital in the 2014 academic year. The evaluation of this system were conducted by compared the contents of portfolios between in the 2013 and 2014 academic year and by the questionnaire surveys at June and February in the 2015 academic year.
As a result, using this system does not affect the contents of portfolios. On the other hand, the questionnaire surveys demonstrate that this system is used very often at the beginning of Postgraduate Clinical Training Course, and almost dental residents agree with positive educational effect of this system.
We conclude that the knowledge databases system is very useful at the beginning of Postgraduate Clinical Training Course. -

It is known widely that the pure titanium can form a direct interface between an implant and bone without intervening soft tissue, in so-called -osseointegration. On the other hand, the Ti6Al4V alloy used as a temporary implant, there is little information about interface observation with a living body tissue. Then, both interface observation was performed using the rats. The XPS revealed that the amounts of aluminum(Al) and vanadium(V) were below the detection limit on the superficial surface of the Ti6Al4V alloy implant. On the other hand, The EDS map of Ti disclosed that the outer layer contained more Ti than the inner layer.
From this research, this was imagined to be one of the causes by which the good clinical results are obtained also in the Ti6Al4V alloy implant. -

It was reported that 10-MDP promoted formation of self-assembled layering at resin- hydroxyapatite interface when it was applied. Firstly, this study showed that the commercial 10-MDP based adhesives also formed the self-assembled layering of two 10-MDP molecules, joined by stable MDP-Ca salt formation, at the interface between human dentin and resin as similar to 10-MDP solution. The three-dimensional observation with FIB-SEM revealed that MDP-Ca salt was formed at the interface, extending into the adhesive (resin) layer. When we prepared adhesives containing MDP-Ca salts as a model of adhesion layer and measured its properties, polymerization shrinkage decreased and some of properties increased by adding the salt. It is concluded that MDP-containing adhesives do form nano-layering at the adhesive interface and MDP-Ca salt spread through the adhesive layer. The properties of adhesive layer with resin, salts and other residual tooth substances may influence the adhesion.

歯科医師臨床研修においては,多くの研修歯科医が大学病院等の大規模施設に集中し,必修化当初より研修内容を充実させることが求められていた.そこで岡山大学病院では,多数の研修歯科医に効果的かつ効率的に研修を行うためのツールとして,電子化ポートフォリオシステムを構築した.本システムにより,多くの研修歯科医また指導歯科医に対して,研修歯科医個々の臨床能力を把握させ,自発的,能動的に臨床能力を向上させることができた.

機能性モノマーのPhenyl-P, 10-MDP, 4-METの人工アパタイト, 歯質表面への作用を調べた。10-MDPはアパタイトに吸着し10-MDP分子が2分子向かい合った層状構造のカルシウム塩を形成した。4-METはカルシウム塩形成を認めず、その反応性は10-MDPより著しく劣る。また, 象牙質との反応では10-MDPでは層状構造を形成したが, Phenyl-Pでは認められなかった。アミノ酸モノマーについての検討は十分ではないが, これらの結果よりモノマーの分子構造によって歯質との反応は様々であることが示唆された。

Role of a novel transcription factor for TNF-α, LITAF, on the pathogenesis of periodontitis

LPS-induced TNF-α factor (LITAF) is a recently identified novel transcription factor controlling TNF-α gene expression in human monocytic cells (Myokai et al., Proc Natl Acad Sci USA, 96: 4518-4523, 1999). To characterize LITAF promoter, we isolated a 1.2 kb fragment of human genomic DNA containing 5'-flanking sequence of the LITAF gene. Primer extension analysis revealed that a transcription start site situated 234 bases upstream from the translation start site in the LITAF gene. The promoter sequence contained the similar consensus sequences for AP-1 and NF-IL6 which were known to be responsible for signaling by LPS. For reporter gene assays in human T-cell leukemia cell line (Jurkat), a series of constructs with fragments of increasing length of the LITAF promoter were coupled. to the firefly luciferase gene. A 34-bp sequence domain located from nucleotides -76 to -43 in the promoter, in which the consensus binding site for AP-1 and NF-IL6 was missing, exhibited the highest reporter gene activity. Moreover, the domain did not include any known consensus sequences. These results suggest that the new sequence domain contributes the up-regulation of LITAF gene transcription.
In the following study, we aimed to examine the localization and kinetics of LITAF protein in THP-1 cells stimulated with LPS. By immunological staining using anti-LITAF monoclonal antibody, an accumulation of LITAF protein was detected in the nuclei of the LPS-stimulated cells whereas it was detected in the cytoplasm of static control cells. Western blot analysis revealed the kinetics of protein in both nuclei and cytoplasm of THP-1 cells stimulated with or without LPS. The nuclear LITAF protein was increased followed by 2 h stimulation, whereas it was recovered its basal level even by the stimulation between 2 and 24 h (student t test; p<0.05). No significant change in the cytoplasmic LITAF protein level was detected followed by the stimulation between 0.5 and 24 h. These results suggested that LITAF protein was transported form the cytoplasm to the nuclei by in the LPS stimulation. Some DNA binding proteins may serve the transportation of the LITAF protein, because the LITAF protein is lack of nuclear localization signal. -
歯周病の発症と進行に関わる交叉免疫応答を誘導する抗原蛋白

Prophyromonas gingivalis由来の分子量53kDaの外膜蛋白(Ag53)は,多くの歯周炎患者由来のIgG抗体およびヘルパーT細胞株によって共通して認識される領域(Ag53p141-161)を有する。本研究において我々は、歯周病細菌抗原由来蛋白を認識するT細胞が他の抗原蛋白と交叉応答し得ることを明らかにすることを目的とした。昨年度我々は、早期発症型歯周炎患者の末梢血単核球からAg53p141-161をHLA-DRB1^*1501拘束的に認識するTh細胞クローン(HT8.3)を樹立した。さらに,Ag53p141-161のアミノ酸配列と相同性を示す領域を有する蛋白5種類をデータベースから探り当てた。しかし,相同性を示す領域の合成ペプチドを作成したが,これらペプチドに対してHT8.3は応答性を示さなかった。このことは,Ag53p141-161の領域において,どの部位がT細胞の抗原認識に関わるかについての詳細を明らかにすることが必須であることを示唆するものである。
以上のことから本年度は,1)HT8.3の抗原認識に関わる詳細な部分を知ること,さらには2)Ag53p141-161において抗原認識に関わる詳細な部分のアミノ酸置換ペプチドに対するTh細胞の応答性を調べることを行なった。その結果,1)Ag53p141-161において,T細胞の抗原認識に関わる領域はAg53p144-155であること,2)Ag53p144-155においてp147(V)を1leにp151(A)をGlyにそれぞれ1残基置換したペプチドは,野性型ペプチドよりも低濃度でHT8-3の増殖応答を誘導することを突き止めた。
これらのペプチドの発見によって,歯周病細菌抗原由来蛋白を認識するT細胞が他の抗原蛋白と交叉応答し得ることの可能性,さらにはアナログペプチドを用いた免疫療法の進展への可能性が示唆された。 -
Factor for induction of root resorption

Using subtractive hybridization, we isolated genes from human periodontal ligament (PDL) cells that were differentially expressed in response to mechanical stress. MRG X gene which related to morality factor 4 gene was cloned from a human cDNA library by subsequent screening. It belongs to a novel family of transcription factors regulating cellular proliferation and senescence. Cellular proliferation is initiated by growth factor binding to specific receptors to initiate signal transduction. Transcripts for the MRG X gene were increased following mechanical stress in cultured PDL cells. The target genes for the MRG X are unknown. Transforming growth factor (TGF)-β1 is mitogenic to PDL cells and regulates the osteoblast-like phenotype of PDL cells. TGF-β1 and its type I receptor (TβR-I) genes were co-expressed following mechanical stress in cultured PDL cells. To examine the effects of the MRG X on the expression of TGF-β1 gene, we performed functional studies aimed at interfering with MRG X mRNA production in cultured PDL cells. Inhibition of MRG X gene mRNA expression in PDL cells resulted in transcription of both the TGF-β1 and TβR-I genes. An application of mechanical stress resulted in a marked reduction of the transcription of TGF-β1 gene in MRG X antisense-expressing cells. The expression level of endogenous MRGX mRNA was elevated by the mechanical stress in the cells. These findings suggest that MRG X acts as a negative regulator of transcription for both TGF-β1 gene in PDL cells.

歯肉線維芽細胞が産生するグルタミン酸脱炭酸酵素(GAD)の歯周病病因に果たす役割

グルタミン酸脱炭酸酵素(Glutamic Acid Decarboxylase, GAD)は,抑制性神経伝達物質であるγ-アミノ酪酸(γ-amino butyric acid, GABA)をグルタミン酸から合成する酵素で,脳や膵β細胞に高濃度に存在する。膵β細胞に存在するGADのうち分子量65kDaの蛋白に対するGAD抗体は,自己免疫疾患である1型糖尿病の発症と進行に関わる重要な自己抗体と考えられている。
申請者は,GAD65mRNAが,ヒト歯肉線維芽細胞に発現することを発見した。そして,昨年までの研究で,GAD65のmRNAの発現は,炎症性サイトカインであるIL-1刺激によって増加すること,また全身疾患を有しない歯周病患者の血清中に,GAD抗体が存在することがわかった。
本年度は,歯周組織において,Gmm65がタンパクレべルで検出出来るかどうかを,Western-blot法,および免疫染色法を用いて確認した。
その結果,歯肉線維芽細胞は,GAD65蛋白を産生すること,さらに歯周病患者の歯周組織においてもGAD65が存在することが確認できた。
この結果は,歯周病患者の歯周組織で産生されるGAD65が抗原となり,GAD抗体が産生される可能性が示唆された。歯周病などの炎症性疾患が、GAD65の産生量に影響する可能性を示唆しているものと考察される。GAD65が自己免疫的機序により歯周病の発症に関わっていると考えた。
この結果は,糖尿病を有していない歯周病患者でも、歯周組織の歯肉線維芽細胞が産生するGAD65によって,GAD65抗体が誘導される可能性を示唆するものであると考える。GAD65は,1型糖尿病の診断や治療法を決定する上で重要なマーカーであるの,歯周病を有する糖尿病患者では注意が必要である。また、歯周病および糖尿病の病態に,GAD65抗体を介する自己免疫応答が関わる可能性があると考える。 -
自己抗体も誘導する歯周組織由来抗原の性状と役割

平成9年度において,1)インスリン依存性糖尿病(IDDM)患者の血清中に歯周組織構成線維芽細胞(歯肉線維芽細胞,歯根膜線維芽細胞)由来抗原に反応する自己抗体が存在すること,2)歯周組織構成線維芽細胞由来抗原は市販のGAD(グルタミン酸脱炭酸酵素)65を認識する抗GAD抗体と反応することを示した。GAD65は膵β細胞に発現し,GAD65に対する抗GAD抗体はIDDMの発症前からIDDM患者で高頻度に検出され,膵β細胞の破壊やIDDM診断のマーカーとして近年非常に重要視されている。また抗GAD抗体はIDDMやある種の全身疾患を有する患者にのみ検出されることが知られている。
平成10年度においては,歯周病が抗GAD65抗体の産生にどのように関わっているかを調べるため1)培養歯肉および歯根膜線維芽細胞におけるGAD65の発現をRT-PCR法で調べた。2)増幅cDNAとGAD65の相同性を増幅cDNAの塩基配列から調べた。3)歯周病患者63人,IDDM17人,そして全身疾患を有さない63人の血中抗GAD抗体価をRIA法で測定した。
結果は1)RT-PCRによって683bpのGAD65と考えられるmRNAを歯肉および歯根膜線維芽細胞が発現していた。2)RT-PCRにより増幅されたcDNAはGAD65と99.8%の相同性を有していた。3)2人(3.2%)の歯周病患者と11人(64.7%)のIDDM患者で抗GAD抗体を検出した。
以上より歯周組織構成線維芽細胞はGAD65を産生し,歯周病患者において抗GAD抗体を検出する事実は,歯周病が抗GAD抗体産生に関わっている可能性と,IDDMの診断や発症予知に影響を与える可能性を示唆すると考える。
