Basic and clinical studies on temafloxacin in urogenital infections Reviewed

The penetration into prostatic fluid, antibacterial activity and clinical usefulness of temafloxacin (TMFX), a new quinolone, were studied. 1) The concentrations of TMFX in the prostatic fluid were 0.33, 0.71 and 0.66 μg/ml at 1, 2 and 4 hours after a single administration of 300 mg of TMFX, and the ratios to serum level (prostatic fluid/serum) were 0.32, 0.35 and 0.43, respectively. 2) We determined the MICs of TMFX against clinical isolates (209 strains of 14 species) from urinary tract infections, and compared them with those of ofloxacin (OFLX), ciprofloxacin (CPFX), enoxacin (ENX) and norfloxacin (NFLX). The antibacterial activity of TMFX was inferior to that of CPFX but almost equal to those of other control drugs. 3) The overall clinical efficacy rate, evaluated according to the criteria of the Japanese UTI Committee, was 100% (2/2) in acute uncomplicated cystitis and 69.2% (9/13) in chronic complicated UTI. Bacteriologically, 12 of 14 strains (85.7%) were eradicated. Two patients with chronic prostatitis were evaluated as good and fair by the doctors. 4) No drug-related side effects, including abnormal laboratory findings, were observed in any of the 17 cases. © 1993, Japanese Society of Chemotherapy. We assessed the pharmacokinetics and dosage regimen of vancomycin in patients with renal insufficiency. 1) Vancomycin (VCM), 500 mg, was administered to healthy volunteers and patients with varying degrees of impaired renal function by 60-minute drip infusion. Serum levels of VCM after a single dose were assayed by FPIA and urinary levels by bioassay. Pharmacokinetic parameters were determined using a two-compartment model. In healthy volunteers, mean T1/2(β) was 3.08h and mean β was 0.233/h. As the degree of renal insufficiency became more severe, T1/2(β) became more prolonged and β decreased. The mean 24-h urinary excretion rate was 96.3% in the healthy volunteers and decreased in accordance with the severity of the renal insufficiency. 2) To plan the dosage regimen, β was selected as a parameter which varied in accordance with the degree of impaired renal function as determined on the basis of creatinine clearance. We investigated pharmacokinetics of cefozopran (CZOP), a new cephem antibiotic, in patients with renal insufficiency. Serum levels of CZOP after a single administration were assayed by HPLC and urinary levels by Bioassay. 1) 1.0g CZOP were administered to healthy volunteers and patients with varying degrees of impaired renal function by 30minutes drip infusion. Pharmacokinetic parameters were determined using a two-compartment model. In healthy volunteers, the mean values of T1/2 (β) was 2.20h and β was 0.321/h. As proceeded the renal insufficiency, T1/2 (β) was prolonged and β was decreased. The mean urinary excretion rate within 24h was 70.0% in healthy volunteers and decreased to 32.5% in Patients with severe renal insufficiency whose creatinine clearrance (Ccr) were below 30ml/min. 2) To plan a regimen, β was selected as the parameter which varied in accordance with the degree of impaired renal function as measured by Ccr. We studied the antibacterial activity and efficacy of loracarbef (LCBF), a new carbacephem antibiotic, in the urological field. 1) Antibacterial activity: The MICs of LCBF were measured against 205 clinical isolates of 14 species from urinary tract infections and compared with those of cefixime (CFIX), cefteram pivoxil (CFTM-PI) and cefaclor (CCL). In general, the antibacterial activities of LCBF were almost equal to those of CCL. 2) Clinical efficacy in complicated urinary tract infection was evaluated according to the criteria of the Japanese UTI Committee. The clinical response in 8 evaluable patients was considered excellent in 4, moderate in 3 and poor in 1. Seven (87.5%) of 8 strains isolated were eradicated. 3) The concentration of LCBF in expressed prostatic secretion (EPS) was studied after oral administration of the drug at a dose of 200 mg. Four hours after the administration of LCBF, the concentration in EPS reached 0.48±0.16 μg/ml (n=3), and the ratio between serum and EPS was 0.28±0.12. 4) Side effects: Diarrhea and abdominal pain were observed in 1 patient. Both symptoms were mild and transient. Based on the above findings, it is considered that LCBF is useful in the treatment of urinary tract infection. © 1993, Japanese Society of Chemotherapy. A total of 36 cases with chronic non-bacterial and non-chlamydial prostatitis or prostatodynia underwent 5 weekly, 1-hour sessions of transrectal microwave hyperthermia (43°C) to the prostate.<br>All patients had a long history of the condition and failed to respond to a variety of conventional treatments. The efficacy of the treatment was evaluated by the effects on subjective symptoms and/or on white blood cells in expressed prostatic secretions. Concerning the overall clinical efficacy, excellent results were obtained in 11 (30.6%), good in 8 (22.2%), fair in 8 (22.2%) and poor in 9 (25.2%). Although minor complications were noted in 5 cases (anal pain; 2, hematospermia; 1, hematuria; 1), all cases received full sesseions. These results indicated the usefulness of the hyperthermia for this benign condition, which has so far responded poorly to the conventional therapy.

We studied the antibacterial activity and efficacy of S-1108, a new cephem antibiotic, in urinary tract infections. 1) Antibacterial activity : The MICs of S-1006 were measured against 202 clinical isolates of 14 species from urinary tract infections and compared with those of cefaclor (CCL), cefixime (CFIX) and cefteram (CFTM). In general, the antibacterial activity of S-1006 was almost equal to that of CFIX and CFTM, but superior to that of CCL. 2) Clinical efficacy : According to the criteria of the Japanese UTI Committee, the overall clinical efficacy rate was 84.2% (16/19) for chronic complicated UTI. Bacteriologically, 21 of the 22 strains isolated (95.4%) were eradicated. 3) Side effects : No clinical adverse reactions and no abnormal laboratory findings were observed. © 1993, Japanese Society of Chemotherapy. We studied the antibacterial activity and clinical efficacy of cefozopran (CZOP), a new cephalosporin antibiotic, in the urological field. 1) Antibacterial activity : We measured MICs of CZOP against clinical isolates (206 strains of 14 species) from urinary tract infections, and compared them with those of imipenem (IPM), ceftazidime (CAZ), latamoxef, and cefoperazone. In general, the antibacterial activities of CZOP were almost equal to those of CAZ but superior to those of other control cephems, and slightly inferior to those of IPM. 2) Clinical efficacy: According to the criteria of the Japanese UTI Committee, the overall clinical efficacy rate was 78.7 % (37/47) for chronic complicated UTI. Bacteriologically, 41 of 47 strains (87.2 %) were eradicated. As regards 2 cases of acute bacterial prostatitis, overall clinical efficacy was moderate and strains were eradicated in both cases. 3) Side effects: No clinical side effects were observed. Concerning laboratory findings, transient elevation of GOT and GPT were noted in two cases, eosinophilia in one case and a transient elevation of s-Cr in one case. © 1993, Japanese Society of Chemotherapy.
