    本研究は、自閉スペクトラム症特性と精神的健康の関連において、自己理解がどのような役割を果たすかを明らかにすることを目的とした。日本の成人604名のデータを利用した二次分析の結果、自閉スペクトラム症特性の高さと精神的健康の悪化との間には関連が認められた。この関連は、自己理解の肯定的側面によって部分的に媒介されることが示された。特にこの自己理解の保護的な効果は、男性よりも女性においてより強い可能性が示唆された。一方で、自己理解の否定的側面は媒介効果を示さなかった。これらの結果から、自閉スペクトラム症特性を持つ人々への支援において、肯定的な自己理解を促進することが重要であり、性差を考慮したアプローチの必要性が示唆された。
    This study examined whether positive and negative dimensions of self-understanding mediate the association between autistic traits and mental health in the general population. Analyzing cross-sectional data from 604 non-clinical Japanese adults, we found that higher autistic traits were significantly associated with poorer mental health. This association was partially mediated by the positive dimension of self-understanding, whereas the negative dimension did not mediate. Exploratory analyses suggested that this protective effect may be more pronounced in women than in men. These findings identify positive self-understanding as an actionable target for support and underscore the value of gender-informed approaches.

    AIM: Digital therapeutics (DTx) is an emerging treatment modality for enhancing psychosocial interventions via software programs, such as smartphone applications (apps). We developed a DTx named ALM-003 to support treatment for people with alcohol dependence who are pursuing reduced drinking goals. ALM-003 was designed to promote behavioral change through daily self-monitoring with personalized feedback, interactive psychoeducational modules, and automated treatment support tools for both patients and physicians. In this multicenter, open-label, randomized controlled trial (RCT), we aimed to evaluate the efficacy and safety of ALM-003 in various clinical settings. METHODS: In this RCT, participants with alcohol dependence without serious physical, mental, or social problems due to excessive drinking, and exhibiting high or very high drinking risk levels were randomized into an intervention group (psychosocial intervention enhanced by ALM-003) or a control group (psychosocial intervention with a basic drinking diary app) for 24 weeks. The primary outcome was the change in the number of heavy drinking days (HDDs) over 28 days (Week 0 to Week 12). RESULTS: Data analysis of the primary outcome for 136 intervention and 142 control participants was performed. Baseline HDDs were 19.4 and 19.1 days/28 days in the intervention and control groups, respectively. At Week 12, the intervention and control groups showed a reduction in HDDs of -12.2 and -9.5 days from baseline, with a between-group difference of -2.79 days/28 days (95% confidence interval: -4.67 to -0.90; P = 0.004). Adverse events occurred in 32.9% of participants in the intervention group and 33.6% in the control group. No adverse events were attributed to app use. CONCLUSION: ALM-003 demonstrated efficacy in reducing heavy drinking days among people with alcohol dependence at a high or very high drinking risk.     OBJECTIVE: To evaluate the effectiveness of a doctor delivered screening and ultra-brief intervention (<1 minute) compared with simplified assessment only for reducing alcohol intake among patients with hazardous drinking in primary care. DESIGN: Pragmatic cluster randomised controlled trial. SETTING: 40 primary care clinics in Japan that did not provide routine screening and brief intervention for hazardous drinking or treatment or self-help groups for alcohol dependency. PARTICIPANTS: 1133 outpatients aged 20-74 years with hazardous drinking (AUDIT-C (alcohol use disorders identification test-consumption) scores ≥5 for men and ≥4 for women). Clinic clusters were allocated to a study arm using a computer generated random sequence. Participants and staff who collected participant reported outcomes remained blinded to assignment. INTERVENTIONS: Primary care clinics were randomised to ultra-brief intervention (21 clinics, 531 patients) or simplified assessment only (19 clinics, 602 patients) groups. The intervention group comprised screening with AUDIT-C followed by brief oral advice and an alcohol information leaflet delivered in <1 minute. The control group comprised simplified assessment with AUDIT-C only. MAIN OUTCOME MEASURES: The primary outcome was total alcohol consumption in the four weeks preceding the 24 week follow-up. Secondary outcomes included total alcohol consumption in the four weeks preceding the 12 week follow-up, and readiness to change drinking behaviour, measured at 12 and 24 weeks. RESULTS: At 24 weeks, the difference in total alcohol consumption between the ultra-brief intervention group (1046.9 g/4 weeks (g/4wk), 95% confidence interval (CI) 918.3 to 1175.4) and control group (1019.0 g/4wk, 893.5 to 1144.6) was 27.8 g/4wk (-149.7 to 205.4, P=0.75), with a Hedges' g of 0.02 (95% CI -0.10 to 0.14). At 12 weeks, the difference in total alcohol consumption between the intervention group (1034.1 g/4wk, 919.6 to 1148.7) and control group (979.3 g/4wk, 866.1 to 1092.4) was 54.9 g/4wk (-104.1 to 213.9, P=0.49), with a Hedges' g of 0.04 (-0.08 to 0.16). CONCLUSION: This trial found no evidence to support the effectiveness of a doctor delivered ultra-brief intervention for hazardous drinking compared with simplified assessment only in primary care in Japan.     OBJECTIVE: We aimed to assess the prevalence of hazardous drinking and potential alcohol dependence among Japanese primary care patients, and their readiness to change and awareness of others' concerns. METHODS: From July to August 2023, we conducted a multi-site cross-sectional study as a screening survey for participants in a cluster randomized controlled trial. The trial included outpatients aged 20-74 from primary care clinics. Using the Alcohol Use Disorders Identification Test (AUDIT) alongside a self-administered questionnaire, we evaluated the prevalence of hazardous drinking and suspected alcohol dependence, patients' readiness to change, and their awareness of others' concerns. RESULTS: Among the 1388 participants from 18 clinics, 22% (95% confidence interval (CI): 20% to 24%) were identified as engaging in hazardous drinking or suspected of being alcohol dependent. As the AUDIT scores increased, so did their readiness to change. However, only 22% (95%CI: 16% to 28%) of those with scores ranging from 8 to 14 reported that others, including physicians, had expressed concerns about their drinking during the past year. For those with scores of 15 or higher, the figure was 74%. CONCLUSIONS: This study underscores the need for universal or high-risk alcohol screening and brief intervention in Japanese primary care settings.     心神喪失等の状態で他害行為を行った者に対する共通評価項目の評定者間一致度の検証を行った。評定者間一致度を算出するため、まず評定用の事例を作成した。入院時15例(急性期)、回復期へのステージ移行期20例、社会復帰期へのステージ移行期10例、退院申請時16例の4つのステージの計61事例の作成を行った。共通評価項目に関する14時間のワークショップを受講した個人によって、事例を評定してもらった。17の中項目では、十分な一致度とされる0.6を超えた項目が10項目、それ以下の一致度となった項目が7項目だった。61個の小項目の級内相関係数には大きなばらつきがあり、0.6以上の項目は40項目であった。0.61以上の項目は全体の64.1%であった。0.41以上の項目は88.4%であり、ほとんどの項目は中等度以上の評定間一致度であった。評定間一致度が低い項目もあり、実証データに裏づけされた尺度として構成をし直す必要があることが示唆された。

    <title>Abstract</title>
    <sec>
    <title>Background</title>
    The Levels of Emotional Awareness Scale (LEAS) was developed to assess five levels of emotional awareness: bodily sensations, action tendencies, single emotions, blends of emotion, and combinations of blends. It is a paper and pencil performance questionnaire that presents 20 emotion-evoking scenes. We developed a Japanese version of the LEAS (LEAS-J), and its reliability and validity were examined.

    <sec>
    <title>Methods</title>
    The LEAS-J level was independently assessed by two researchers who scored each response according to the LEAS scoring manual. High inter-rater reliability and internal consistency were obtained for the LEAS-J. Measures were socioeconomic status, LEAS-J, Toronto Alexithymia Scale-20 (TAS-20), Interpersonal Reactivity Index (IRI), and NEO Five-Factor Inventory (NEO-FFI). TAS-20, IRI and NEO-FFI were the measures used to explore the construct validity of LEAS-J, as it was predicted that higher scores on the LEAS-J would be related to fewer alexithymic features, greater empathetic ability, and a greater sense of cooperation with others. Questionnaires were completed by 344 university students.

    <title>Results</title>
    The criterion-referenced validity was determined: a significant negative relationship was found with the externally-oriented thinking scores of TAS-20, and positive relationships were found with fantasy, perspective taking, and empathic concern on IRI and with extraversion, openness to experience, and agreeableness on NEO-FFI.

    <sec>
    <title>Conclusions</title>
    Consistent with our expectations, the findings provide evidence that the LEAS-J has good reliability and validity. In addition, women had significantly higher scores than men on LEAS-J, showing that the gender difference identified in the original LEAS was cross-culturally consistent.

    本研究では，日本版「たくましさ希求」尺度の開発を目的とした。その結果，「行動」「態度」の2因子構造であることが確認され，これは男女ともにほぼ同様の構造であった。構造は原尺度と同様であるが，性差がない点は原尺度と異なっており，文化的な背景の違いがあるのではないかと考えられた。また，本尺度は十分な内的整合性を有しているものの，「行動」において得点が下限値に近く，原尺度より低い傾向にあった。日本における「行動」は，筋肉増強にかかわる具体的問題行動を指す可能性が高いと示唆された。妥当性については，性差や専攻による差から十分な併存的妥当性が得られた。収束的妥当性については，身体イメージ，自尊感情，自己愛傾向，性役割意識との関連から導き出された。弁別的妥当性については，「行動」との間にのみ，摂食障害傾向と弱い関連性が認められ，本尺度が痩せ志向とは異なる概念を測定している一方で，摂食障害傾向と行動的共通性があるという予測に一致していた。以上より，本尺度の有用性が実証された。今後，項目表現の再検証を行うこと，文化による違いを解明すること，問題傾向群の弁別に適用可能かどうかを明らかにすることが求められる。

    We examined developmental aspects of the emotional awareness of adolescents by evaluating their responses to a self-reported questionnaire based on the Toronto Alexithymia Scale-20 (TAS-20).

    <title>Methods</title>
    The items of the TAS-20 were modified to make them more understandable by adolescents, and nine new items related to a limited capacity for imagination were added. The Japanese Linguistic Ability Test and the multi-dimensional empathy scale for adolescents were administered to examine concurrent validity. Two hundred and two normative young adolescents and thirty-two adolescent patients with psychosomatic and/or behavioral problems participated in the study. Eighty junior high school students also participated in a separate examination of test-retest reliability.


    </sec>
    <sec>
    <title>Results</title>
    Thirteen items were extracted after exploratory and confirmatory factor analyses, and four core factors were identified in the resulting scale: Difficulty Identifying Feelings (DIF), Difficulty Describing Feelings (DDF), Externally-Oriented Thinking (EOT) and Constricted Imaginal Capacities (CIC). Interestingly, scores on the multi-dimensional empathy scale correlated positively with DIF and DDF, but negatively with EOT and CIC. Higher DDF scores were associated with higher Japanese linguistic abilities. DIF/DDF scores were higher for females than males, irrespective of linguistic ability. Test-retest reliability coefficients were significant. The patient group showed significantly higher DIF scores than the normative students.

    <sec>
    <title>Conclusion</title>
    The present findings indicated that subjective difficulties in identifying and describing feelings are associated with empathetic and linguistic abilities. The developmental aspect to emotional awareness herein described suggests that self-reported questionnaires for alexithymia must be carefully constructed and examined, even for adults.

    本研究の目的は､中学生を対象に､社会的スキルとストレッサーに対する認知的評価､ストレス反応の関係性を明らかにし､社会的スキルが､認知的評価のストレス反応への影響を調整する効果(調整効果)について検討することであった｡その結果､ストレス反応に対する社会的スキルと認知的評価の影響には男女差が認められ､男子ではストレス反応と｢関係参加行動｣・「関係維持行動｣という基礎的なスキルの間に負の関連が認められた｡一方女子では､それら2つの基礎的なスキルに加え､認知的評価の｢コントロール可能性｣とストレス反応の間にも負の関連が､｢関係向上行動｣とストレス反応との問には正の関連が認められた｡調整効果については､女子において部分的に確認された｡The purpose of this study was to determine the relations between social skills and cognitive appraisal and stress responses in junior high school students. Moreover, we examined whether social skills coordinate influence on stress responses of cognitive appraisal(coordination effects). As results, gender differences were observed in influences on stress responses of cognitive appraisal and social skills. In males, stress responses were negatively associated with fundamental social skills such as "Behavior for participating in relations" and "Behavior for maintaining relations". In female sample, stress responses had negative associations with "controllability" of cognitive appraisal in addition to these fundamental social skills. Furthermore, stress responses were positively association with "Behavior for developing relations" in female. The coordination effects were confirmed in female partially. These findings were discussed.

    <sec>
    <title>Background</title>
    Patients with anorexia nervosa-restricting type (AN-R) sometimes develop accompanying bulimic symptoms or the full syndrome of bulimia nervosa (BN). If clinicians could predict who might change into the bulimic sub-type or BN, preventative steps could be taken. Therefore, we investigated anthropometric and psychological factors possibly associated with such changes.

    <sec>
    <title>Method</title>
    All participants were from a study by the Japanese Genetic Research Group for Eating Disorders. Of 80 patients initially diagnosed with AN-R, 22 changed to the AN-Binge Eating/Purging Type (AN-BP) and 14 to BN for some period of time. The remaining 44 patients remained AN-R only from the onset to the investigation period. Variables compared by ANOVA included anthropometric measures, personality traits such as Multiple Perfectionism Scale scores and Temperament and Character Inventory scores, and Beck Depression Inventory-II scores.

    <sec>
    <title>Results</title>
    In comparison with AN-R only patients, those who developed BN had significantly higher current BMI (p &lt; 0.05) and maximum BMI in the past (p &lt; 0.05). They also scored significantly higher for the psychological characteristic of parental criticism (p &lt; 0.05) and lower in self-directedness (p &lt; 0.05), which confirms previous reports, but these differences disappeared when the depression score was used as a co-variant. No significant differences were obtained for personality traits or depression among the AN-R only patients irrespective of their duration of illness.


    </sec>
    <sec>
    <title>Conclusion</title>
    The present findings suggest a tendency toward obesity among patients who cross over from AN-R to BN. Low self-directedness and high parental criticism may be associated with the development of BN by patients with AN-R, although the differences may also be associated with depression.

    近年，自閉スペクトラム症（ASD）と診断される者が増えており，特に青年期・成人期の支援に注目が集まっている。この年代のライフステージ上の課題の一つは社会参加と就労である。しかし，ASD者の就労に関する実証的な研究は少なく，標準化されたアセスメントツールも限られている。そこで本研究では，ASD者が安定的に就労するために必要な要因を明らかにし，ASDに特化した就労準備性のアセスメントツールを開発することを目的としている。
    初年度の目的は，ASD者が安定した就労をするために必要な就労準備性を明らかにすることであった。調査協力が得られた医療機関の倫理委員会での承認後に，医療機関や福祉・労働機関での就労支援を経て，1年以上就労が継続できているASD者を対象に，半構造面接によるインタビュー調査を行った。具体的には，インタビューガイドを作成し，現在の就労状況やこれまで受けた支援の内容，仕事に就くために必要だった準備，支援を受けていく中で身に着けたこと，今仕事を続けていく上で大切だと思うこと等についての質問を行った。候補者に対するリクルートを行った結果，年度末までに9名から研究参加の同意が得られ，半構造化面接を行った。そのデータの逐語記録を作成し，質的な解析を行っている最中である。
    次年度はデータの解析を進めるとともに，予備的検討として就労を継続している者を対象とした更なる調査を行い，アセスメントツール開発のための質問項目のリストを作成する予定である。
