Self-care behavior is considered important for preventing the progression of chronic kidney disease (CKD). Although lifestyle interventions are popular, they have not been sufficiently effective. According to studies on other chronic diseases, illness representation has been found to formulate a pattern, and self-care behavior could differ depending on the pattern, which suggests difference in self-care behavior based on illness representation. This study examined what kind of illness representational patterns exist among CKD patients and whether there is a difference in self-care behavior depending on the pattern. A survey was conducted from the beginning of June to the end of October 2019 on 274 CKD patients who were either outpatients or hospitalized at general hospitals in Western Japan. The Illness Perception Questionnaire-Revised was used to assess illness representation and the Japanese Chronic Kidney Disease Self-Care scale was used to assess self-care behavior. Two-stage cluster analysis was used to identify clusters. Cluster features were examined using analysis of variance and Tukey HSD tests. Differences in self-care behavior scores among identified clusters were investigated. Two hundred and forty-four questionnaires were received, and 212 were analyzed. Participants were aged 64.9±12.9, and the estimated glomerular filtration rate was 33.7±15.8. Three clusters were identified: Cluster 1 represented the difficulty of making sense of the changed condition caused by the disease and easily falling into misunderstanding; Cluster 2 represented patients with disease conditions that impacted their daily life and emotional responses; Cluster 3 represented the controllability and understandability of the disease. Total self-care behavior scores indicated a significant difference between Cluster 1 (52.1 ± 9.7) and Cluster 3 (57.7 ± 8.2). In conclusion, we showed that three representational patterns exist among CKD patients. In addition, a difference was found in self-care behavior depending on the illness representational pattern, suggesting the need to focus on illness representation.

Purpose: The aim of this study was to develop a physical activity self-management agency scale that includes cognitive selfmanagement skills, and to examine the usefulness of the scale. Method: The subjects were 193 students at University A. Scale development involved several phases, including item development, reliability testing, construct validity testing, and criterion-related validity testing. Furthermore, we examined the usefulness of the scale based on the relevancy of universal self-management agency and physical activity self-management agency. Results: Confirmatory factor analysis was conducted to assess the extent to which the physical activity self-management agency scale structure fit the present data. The fit indices were approximately satisfactory (GFI = .955, AGFI = .926, RMSEA = .038). The scale comprises four factors with 11 items. Cronbach's alpha for the total scale score was .865. A significant difference in the scale score was observed with the continuance of exercise. It was also shown that physical activity self-management agency had a greater effect on continuing to exercise than universal self-management agency (β = .319, p < .01). Conclusion: We developed a physical activity self-management agency scale and confirmed its construct validity, validity by external standards, and reliability. The usefulness of the scale was suggested.

The purpose of this study was to clarify these relationships among subjective symptoms, stress cognition and psychological status in inpatients. The subjects were eighty one inpatients (age, >20 years). The data were collected using questionnaire based on the following scales: the Illness Perception Questionnaire (IPQ) and the Cornell Medical Index (CMI), the Cognition Scale of Illness-related Anxiety, and the Hospital Anxiety and Depression Scale (HADS). Collected questionnaire data were first used to confirm the reliability and factorial validity of subjective symptoms and identify correlations among subjective symptoms, stress cognition and psychological status. Subsequently, we applied a causality model that defined psychological status as the dependent variable and assumed subjective symptoms directly affect the dependent variable or have an indirect influence through stress cognition. As a result of our analysis, it was found that subjective symptoms affected both stress cognition and psychological status. Furthermore, it was discovered that stress cognition have greater effect on psychological status. Therefore, these findings indicated that nurse must not only manage patients subjective symptoms but also recognize their stress cognition in order to intervene with appropriate care that maintains the psychological status of inpatients.

精神疾患としての痴呆患者を抱える家族への社会資源の導入に関する訪問看護師の認識を明らかにすることを目的に,精神疾患患者への社会資源の導入経験を有する31施設の常勤訪問看護師を対象にアンケート調査を実施し,52名(うち,女性50名.平均年齢41.2±6.7歳)より回答を得た(回収率56.5%).その結果,40名(76.9%)が社会資源の導入について家族より相談を受けた経験を有しており,23名(44.2%)が「家族の世間体」により社会資源の導入が妨げられた経験を有していた.また,社会資源の導入を阻止する要因として訪問看護師は,「家族の経済力」や「家族の価値観,信念」を挙げ,訪問看護師が必要と考える社会資源である精神疾患患者が利用可能なデイケア,利用者に添ったサービス,介護者の交流会,近隣の専門医,などが乏しい状況にあると認識していた

The purpose of the present study was to develop the Chronic Obstructive Pulmonary Disease (COPD) Activity Rating Scale (CARS) to measure life-related activity in patients with COPD, and to confirm its reliability and constructive validity in a factorial structure model. The subjects consisted of 114 patients with COPD. An 88-item life-related activity list, generated previously from a literature review, was administered. The secondary structural model consisted of four factors with 12 items. The results of the confirmatory factor analysis by structural equation modeling showed the fit criteria to be statistically significant. The internal consistency of the 12 items was highly reliable (Cronbach's α = 0.924). The CARS score was correlated with pulmonary function tests, breathlessness, and the health-related quality of life (QOL) scales in Pearson correlation coefficient. The results suggest that the COPD Activity Rating Scale is a valid scale for the assessment of life-related activity in patients with COPD.

慢性期看護実習の内容改善に役立てる基礎資料にするために,平成14年度の看護実習の目的に沿った具体的な実習目標において,学生の到達度及び実習目標間の関係を明らかにした.実習目標の到達度は,「自己の看護観や自己成長を培う」が最も高かく,「疾患の病態生理や検査・治療についての理解と看護援助」「セルフケアに向けた看護援助」「危機に直面している患者の看護援助」等の看護実践面に関する目標が高値を示していた.実習目標間の関係では,各実習目標との間に相関が見られ,それらは信頼性のある妥当な慢性期看護実習の目標であった

This study's purpose was the development of a illness-related anxiety cognition scale by which to determine efficacy of nursing intervention upon patient stress. The subjects were 258 patients in K hospital (144 males, 114 females; age-range 21-88, mean age 56.7). A 25-item illness-related anxiety checklist, generated from a review of available literature, was administered. Structure-equation modeling for statistical analysis was constructed to assess goodness fit, with validity achieved through confirmatory factor analysis. The secondary structure model consisted of five factors with 15 items. The results of confirmatory factor analysis revealed that the fit criteria were statistically significant (GFI=0.906, AGFI=0.868, CFI=0.945, and RMSEA=0.074). Intenal consistency of the 15 items was highly reliable (Cronbach's α =0.919). Concurrent validity was assessed using the locus of control scale. Results of the fit criteria are statistically significant.
