    顎骨が切除域に含まれる頭頸部がん患者の術後の咀嚼障害は,患者のQOLの低下につながり大きな問題であった。インプラント治療は咀嚼機能が改善する方法であるが,費用が高額であり,断念せざるを得ないケースが多かった。本邦において,先進医療の導入,その後に保険導入が行われたことは,患者にとってはきわめて有意義であると考える。今回,先進医療でインプラント埋入が行われた症例と保険導入後にインプラント治療が行われた症例を経験したので,その概要を報告する。症例1は,右側上顎がんの放射線治療および手術治療後に血管柄付き骨皮弁で再建術を施行され,その後先進医療で右上顎にインプラント埋入術を施行,ブリッジタイプの上部構造を装着した。症例2は,右側下顎歯肉がん術後に血管柄付き骨皮弁による再建術が施行され,その後,保険治療でオーバーデンチャータイプの下顎インプラント義歯を作製した。(著者抄録)

    We evaluated long-term dynorphin A-immunoreactivity in the rat area postrema (AP) after the administration of cisplatin. First, rats were given 1, 5 and 10 mg/kg body weight cisplatin (i.p.) and their behavior was monitored for 72 h. We observed a delayed increase in pica 24-72 h after injection, compared to the 24 h before injection. We attributed this to the cisplatin injection. Pica was defined as an increase in the intake of non-nutritional matter such as kaolin. Administration of 1, 5 and 10 mg/kg cisplatin led to an increase in kaolin intake on day 1. Administration of 5 and 10 mg/kg of cisplatin led to decreased intake of laboratory chow (MF) on days 1-3, but 10 mg/kg cisplatin causes an excessive aggravation of their condition. Following this behavioral experiment, we immunohistochemically examined the induction of dynorphin A in the AP at 24, 48 and 72 h post-administration of 1 and 5 mg/kg cisplatin. Administration of 5 mg/kg cisplatin caused dynorphin A to accumulate gradually in the neurosoma of the AP neurons, and the numbers of positive AP neurosomata at 48 and 72 h post-administration were higher than following an equal dosage of 0.9% NaCl. These findings suggest that dynorphin A increases in the central nervous system for a long time following administration, and causes certain behavioral and clinical changes, including those related to appetite and nausea.     原発巣や頸部転移巣からの出血を制御するため動脈塞栓術を施行した口腔癌末期の患者3例(症例1:63歳女,症例2:66歳男,症例3:73歳男)について検討した.症例1は術後の出血はみられず,全身状態が改善したため退院したが,退院2ヵ月後に肺炎により死亡した.症例2は術後の出血はみられなかったが,約4ヵ月後,腫瘍が上顎骨および左顔面から頸部の皮膚等の周囲組織に進展し,それに伴う悪液質により死亡した.症例3は術後の副作用及び出血を認めなかったが,4日後に頸部から大量出血し死亡した.塞栓術が奏効した症例1及び2の術前後のKPSと血清アルブミン値を比較検討した結果,出血直後に比べ塞栓後のKPS及び血清アルブミン値は改善しており,QOLは改善したと考えられた.口腔癌に対する血管内治療は患者のQOLの維持,改善と延命に有効な手段であると考えられた

    32歳女.両側上顎歯肉の有痛性腫脹に伴う咀嚼障害を主訴とした.上顎歯肉は全体的に腫脹,発赤が認められ,インプラント周囲より排膿が見られた.既存インプラントを保存したまま,前歯部に骨内インプラントを埋入し,骨膜下インプラントを除去後早期に骨内インプラント維持のオーバーデンチャーを装着する治療計画をたてた.審美性の回復は得られたが,固定性補綴物を求める傾向は強く残っており,サイナスリフトと骨移植を併用した骨造成後,骨内インプラントを埋入し機能回復を行うこととした.移植骨の吸収が認められたため,5本のインプラント体を両側小臼歯部に埋入した.7ヵ月後,アバットメント連結手術を施行し,患者の満足も得られている

    過去20年間に当科で皮弁・筋皮弁による再建術を行った口腔癌患者44名48例について検討した.年齢は28〜77歳(平均60.3歳),男37例,女11例であった.T分類ではT4症例が29例(65.9%)と最も多く,進行癌が殆どであった.再発例は7例で,rT2が2例,rT4が5例であった.再建部位は下顎歯肉9例,口底9例,舌17例,頬粘膜9例,顔面・頸部皮膚4例(二次再建例)であった.再建部位別皮弁・筋皮弁頻度は下顎歯肉ではD-P皮弁2例,大胸筋皮弁7例,口底ではD-P皮弁3例,大胸筋皮弁6例,舌ではD-P皮弁3例,大胸筋皮弁13例,前腕皮弁1例,頬粘膜ではD-P皮弁9例,顔面・頸部皮膚では前外側大腿皮弁3例,広背筋皮弁1例であった.皮弁・筋皮弁の生着率は生着良好は35例,生着不良は13例であった.生着良好例の平均年齢は59.0歳,生着不良例では平均63.8歳であった.生着不良例の内訳はD-P皮弁5例,大胸筋皮弁8例で,血管柄付き遊離皮弁は全例生着良好であった

    口腔悪性黒色腫4例について臨床的検討を行った.対象は男女各2例で,年齢は57〜75歳であった.原発部位は下顎歯肉1例,口蓋3例で,初診時のUICC(1997)分類はstage Ia期:1例,stage II期:1例,stage III期:2例であった.治療法は全例に外科療法を行い,その前後にダカルバジンを中心とした化学療法,および免疫療法,またはホルモン療法を併用した.局所再発,および頸部リンパ節転移,または遠隔転移を起こした2例が死亡例であった.他の2例は局所再発,および転移はなく,経過観察期間は14年11ヵ月と3年8ヵ月であった

    The mechanism of induction of emesis by X-ray irradiation remains largely unknown. The purpose of the present research was to clarify the neuronal basis of the induction of nausea induced by X-ray irradiation analyzing c-Fos expression in the nucleus tractus solitarii (NTS) as a marker of cellular excitation. We confirmed that the dose of X-ray irradiation (4 Gy) used for the present research could actually induce nausea by preliminary measurement of kaolin intake. Induction of c-Fos immunoreactivity in the NTS was observed in the animals that received X-ray irradiation of the whole body. The mean number of c-Fos positive cells in the animals that received irradiation was significantly larger than that in the non-irradiated animals. Partial exposure of the abdomen to X-rays showed significantly greater c-Fos expression than that of the head. These results indicated the presence of a certain route for transmitting information from the periphery toward the central nervous system by X-ray irradiation. The number of c-Fos positive cells induced by X-ray irradiation in animals vagotomized at the subdiaphragmatic level was lower than that in sham-operated animals. Animals receiving a serotonin subtype three (5-HT3, 5-hydroxytryptamine) receptor antagonist (tropisetron, ICS 205-930, 3-tropanyl-indole-3-carboxylate) showed a significant reduction in c-Fos protein expression compared to animals receiving a vehicle. These results strongly suggested that X-ray irradiation activates 5-HT3 receptors on the terminals of the abdominal vagal nerves to excite the afferent pathway, thereby inducing emesis.     We report a case of intra-osseous carcinoma arising from a mandibular cyst. An 81-year-old woman visited our hospital because of a right mandibular swelling. Radiographic examination showed a multilocular radiolucent lesion and bone expansion, extending from the midline to the right mandibular angle. The clinical diagnosis was a mandibular cyst and marsupialization was performed. The histopathological specimen showed a cyst wall with an epithelial layer and connective tissue. After marsupialization, the swelling and the radiolucent area of the mandible decreased. However, about 7 months later, the mesial region of the lesion became swollen again, and it was biopsied. At that time, we obtained a pathological diagnosis of squamous cell carcinoma.<BR>Marsupialization does not permit histopathological examination of all sites of the lesion. Therefore, when it is performed for large mandibular cysts such as in our case, we should consider the posibility of an intra-osseous carcinoma arising from the lesion.

    Recently, some patients, receiving long-term antipsychotic therapy, particularlywith phenothiazines, have been reported to die suddenly and unexpectedly. These patients may have underlying diseases of organs such as the liver, heart and endocrine glands. General anesthesia and surgery can be difficult in these patients. Therefore, care should be taken with regard to drug therapy (dosage and treatment period), laboratory findings, and general condition.

    A case of Schwannoma that occurred at the right palate was reported. The clinical impression was suspected pleomorphic adenoma, and the lesion was surgically excised under general anesthesia. It was noticed during the operation that the tumor adhered to the greater palatine nerve. Histopathologically it was diagnosed as Schwannoma (Antoni A type), so it was suggested that the tumor had a close relation to the greater palatine nerve. Clinical behaviors were compared with other Schwannomas already had reported. From histological, electronmicroscopical, and immunohistochemical findings, the origin of the tumorwas also discussed.
