本研究は，地域で暮らす高齢者のQOLを支える在宅ケアモデルの構築を目指し，認知症高齢者が人生経験を通してどのような老いの生き方・暮らし方を望んでいるのか，さらに家族やケア提供者が，認知症高齢者の望む老いの生き方・暮らし方をどのようにとらえ，どのような支援をしているのかを明らかにしたうえで，認知症高齢者が人生の最後まで望む老いの生き方・暮らし方を可能にする在宅ケアモデルを構築し，ケア提供者による支援のあり方を検討することを目的としている。
前年度までに，在宅で生活している認知症高齢者にケアを提供している訪問看護師，理学療法士，介護支援専門員に，認知症高齢者の望む生き方・暮らし方をどのようにとらえ，どのように支援しているのかについて面接調査を実施した。その分析結果を認知症看護認定看護師や訪問看護師，介護支援専門員に意見を求め，分析の妥当性を保証した。次に，面接調査の分析結果に基づいて作成した質問紙調査票を分析し，統計学の専門家に意見を求めながら分析を進めている。
本年度は，在宅で生活している認知症高齢者の望む生き方・暮らし方の認識と支援の実態をとらえる在宅ケアモデルを想定し検証を繰り返した。また，面接調査による分析結果を論文として報告し，質問紙調査の分析結果はケアモデルの検討過程として学術集会において報告した。
本研究の成果は，在宅で生活している認知症高齢者の望む生き方・暮らし方を支えるために日常的に実践しているケアに活用できる指標として有用性が期待できる。また，認知機能が低下し，自分の思いを言葉で語ることが難しくなる認知症高齢者の望みを引き出し，支え，意思決定を支援しながらQOLを保障するケアのあり方を検討するうえで非常に重要な示唆を得ることができる。 -

In this study, for the construction of the home care model to support the QOL of elderly people, way of life of health and old age of the elderly living alone, we conducted the interviews about the life experience, a questionnaire survey on the basis of the analysis result create, were performed. To distribute the questionnaire to the elderly living alone 1058 people living in Chugoku and Shikoku area in Japan, were obtained valid responses from 735 people. The Elderly living alone has a hope that more than half. In addition, it has been shown that hope is associated with live and life satisfaction and the social role satisfaction.

The experience of home-based medical and nursing care for elderly couples comprised "Intimacy and mutual support between husband and wife","Prospect of an unsteady future and lifestyle adjustment","Reliable people and services for the couple" and "How to live in old age and synthesis of beliefs about life". Elderly couples reflected on their lives, reconstructed their relationship as a couple, and decided how to live in old age by synthesizing their values and beliefs about life. In addition to sound techniques in nursing care support, care providers need to support elderly couples by enabling them to express their thoughts about their lives and by providing close support adapted to the lifestyle of the elderly couple in accordance with the capacity of the carer and the person receiving care.
