    BACKGROUND: Multidisciplinary team (MDT) approach has become a standard for perioperative patient care, including in esophageal cancer. In our institution, the Perioperative Management Center (PERiO) has been doing an MDT approach for patients undergoing esophageal cancer surgery since 2009. On the other hand, neoadjuvant therapy has also been becoming standard for many malignancies, including esophageal cancer. In Japan, neoadjuvant chemotherapy (NAC) for esophageal cancer is standard now. However, there have been no reports about when is the best time to start the MDT approach for patients with neoadjuvant therapy. In this study, the best start time for the MDT approach for esophageal cancer patients with NAC was examined from the perspective of adverse events during chemotherapy and perioperative period. METHODS: All cases underwent thoracoscopic esophagectomy in the prone position (TEPP) after NAC. The PERiO Intervention group that started before NAC (n = 100) was compared with the PERiO Intervention group that started after NAC (n = 77). Eventually, 54 paired cases were matched by propensity score matching. RESULTS: The adverse event rate during chemotherapy, especially oral complications, was significantly decreased in the PERiO Intervention started before the NAC group (P = 0.007). Furthermore, weight loss during the period from chemotherapy to surgery was significantly reduced in the group that started before NAC (P = 0.033). CONCLUSION: The MDT approach should be started before NAC in patients undergoing esophageal cancer surgery to prevent adverse events during chemotherapy and provide safe perioperative conditions.

    BACKGROUND: The Prognostic Nutritional Index is useful for predicting surgical risk and overall survival based on preoperative immunological and nutritional status in patients undergoing digestive organ cancer surgery. The purpose of this study was to examine the association between the Prognostic Nutritional Index and dental status in patients with esophageal cancer who underwent esophagectomy. METHODS: This retrospective case-control study included 73 patients who underwent resection of esophageal cancer (69 males, 4 females; age 36-83). General and dental status were evaluated. The Prognostic Nutritional Index was calculated based on the serum albumin concentration and the total lymphocyte count, and subjects were divided into two groups based on index scores: a higher group, characterized by scores ≥ 45 (n = 54); and a lower group, characterized by scores < 45 (n = 19). Univariate analysis and multiple logistic regression analyses were used to compare between groups. RESULTS: Total protein, C-reactive protein, the number of sound and total decayed, missing and filled teeth, and the rate of patients with poor dental occlusal support showed significant differences between the lower and higher Prognostic Nutritional Index groups (p < 0.05). Stepwise logistic regression analysis by backward selection approach showed that low total protein, few sound teeth, and poor status of dental occlusal support were significantly associated with the lower Prognostic Nutritional Index (p = 0.007, 0.042, and 0.009, respectively). CONCLUSION: Dental status, especially dental occlusal support and the number of sound teeth, showed a positive relationship with the Prognostic Nutritional Index in esophageal cancer patients who underwent esophagectomy.

    BACKGROUND: Dysphagia is a prominent symptom after esophagectomy and may cause aspiration pneumonia. Swallowing evaluation after esophagectomy can predict and help control the incidence of postoperative pneumonia. The aim of this study was to clarify whether the change in tongue pressure was associated with any related factor and postoperative dysphagia/pneumonia in patients with esophageal cancer after esophagectomy. METHODS: Fifty-nine inpatients (41 males and 18 females; 33-77 years old) who underwent esophagectomy participated in this study. Measurement of tongue pressure and the repetitive saliva swallowing test (RSST) was performed before esophagectomy (baseline) and at 2 weeks postoperatively. The general data were collected from patients' medical records, including sex, age, type of cancer, cancer stage, location of cancer, operative approach, history of previous chemotherapy, surgical duration, amount of bleeding during surgery, incidences of postoperative complications, intubation period, period between surgery and initiation of oral alimentation, and intensive care unit (ICU) stay, blood chemical analysis, and lifestyle. RESULTS: Tongue pressure decreased significantly after esophagectomy (p = 0.011). The decrease of tongue pressure was significantly associated with length of ICU stay and preoperative tongue pressure on multiple regression analysis (p < 0.05). The decrease of tongue pressure in the RSST < 3 or postoperative pneumonia (+) group was significantly greater than in the RSST ≥ 3 (p = 0.003) or pneumonia (-) group (p = 0.021). CONCLUSIONS: The decrease in tongue pressure was significantly associated with the length of ICU stay, preoperative tongue pressure, and the incidence of dysphagia and pneumonia among inpatient after esophagectomy.

    OBJECTIVE: Acetaldehyde, associated with consumption of alcoholic beverages, is known to be a carcinogen and to be related to the tongue dorsum. The aim of this study was to investigate the relationship between acetaldehyde concentration in mouth air and bacterial characteristics on the tongue dorsum. METHODOLOGY: Thirty-nine healthy volunteers participated in the study. Acetaldehyde concentrations in mouth air were evaluated by a high-sensitivity semiconductor gas sensor. A 16S rRNA gene sequencing technique was used to compare microbiomes between two groups, focusing on the six samples with the highest acetaldehyde concentrations (HG) and the six samples with lowest acetaldehyde concentrations (LG). RESULTS: Acetaldehyde concentration increased in correlation with the increase in bacterial count (p=0.048). The number of species observed in the oral microbiome of the HG was higher than that in the oral microbiome of the LG (p=0.011). The relative abundances of Gemella sanguinis, Veillonella parvula and Neisseria flavescens in the oral microbiome of the HG were higher than those in the oral microbiome of the LG (p<0.05). CONCLUSION: Acetaldehyde concentration in mouth air was associated with bacterial count, diversity of microbiome, and relative abundance of G. sanguinis, V. parvula, and N. flavescens.

    Preoperative oral care is helpful to prevent postoperative complications in patients who are undergoing esophagectomy. Here, we report the case of an 81-year-old Japanese man with an upper limb disability caused by post-polio syndrome who was receiving neoadjuvant chemotherapy for esophageal cancer. He had poor oral health status and developed oral complications as a side effect of chemotherapy. He could not brush his teeth by himself. However, infection control by oral care provided by an interprofessional collaboration successfully improved his oral hygiene, and his follow-up involved no severe complications. Interprofessional collaboration is useful especially for patients with upper limb disability.

    Perioperative management is critical for positive neurosurgical outcomes. In order to maintain safe and authentic perioperative management, a perioperative management center (PERIO) was introduced to patients of our Neurosurgery Department beginning in June 2014. PERIO involves a multidisciplinary team consisting of anesthesiologists, dentists/dental hygienists/technicians, nurses, physical therapists, pharmacists, and nutritionists. After neurosurgeons decide on the course of surgery, a preoperative evaluation consisting of blood sampling, electrocardiogram, chest X-ray, and lung function test was performed. The patients then visited the PERIO clinic 7-14 days before surgery. One or two days before surgery, the patients without particular issues enter the hospital and receive a mouth cleaning one day before surgery. After surgery, postoperative support involving eating/swallowing evaluation, rehabilitation, and pain control is provided. The differences in duration from admission to surgery, cancellation of surgery, and postoperative complications between PERIO and non-PERIO groups were examined. Eighty-five patients were enrolled in the PERIO group and 131 patients in the non-PERIO group. The duration from admission to surgery was significantly decreased in the PERIO group (3.6 +/- 0.3 days), compared to that in the non-PERIO group (4.7 +/- 0.2 days). There was one cancelled surgery in the PERIO group and six in the non-PERIO group. Postoperative complications and the overall hospital stay did not differ between the two groups. The PERIO system decreased the duration from admission to surgery, and it is useful in providing high-quality medical service, although the system should be improved so as not to increase the burden on medical staff.

    医療系大学においては、全人的医療教育やコミュニケーション教育を目標として患者に付き添う実習が導入されている。しかしながら、その実習内容に振り返りを取り入れた報告は散見するにすぎない。岡山大学歯学部では、早期見学実習の一環として学生が何を学び、感じたかを分かち合う振り返りを取り入れ、外来初診患者を対象としたエスコート実習を実施してきた。本エスコート実習を受講した学生および実習に協力が得られた患者を対象にアンケートを実施し、本実習がどのように評価されているかを検討した。1年次生370名および患者422名の回答を分析した結果、以下の結論を得た。大多数(96.5%)の学生は本実習を有意義だと評価しており、学生は本実習により患者の視点を理解し、医療人にふさわしいコミュニケーションの技能も理解していた。また、本実習が学生にとって医療人を目指す者としての態度の修得や自覚をもつことを促す動機づけになっていた。大多数(96.4%)の患者が学生の態度について肯定的に評価をしており、85.0%がこの実習は有意義なものであると評価していた。その主な理由として、学生が患者の視点や医療人にふさわしいコミュニケーションの技能を理解できることなどが挙げられていた。以上より、本実習は学生が患者の視点を理解し、医療人を目指す者としての態度や自覚を身につけるという学習目標を達成しており、学生および患者から高く評価されていることが示唆された。(著者抄録)

    Objective: Acetaldehyde is the first metabolite of ethanol and is produced in the epithelium by mucosal ALDH, while higher levels are derived from microbial oxidation of ethanol by oral microflora such as Candida species. However, it is uncertain whether acetaldehyde concentration in human breath is related to oral condition or local production of acetaldehyde by oral microflora. The aim of this pilot study was to investigate the relationship between physiological acetaldehyde concentration and oral condition in healthy volunteers. Material and Methods: Sixty-five volunteers (51 males and 14 females, aged from 20 to 87 years old) participated in the present study. Acetaldehyde concentration in mouth air was measured using a portable monitor. Oral examination, detection of oral Candida species and assessment of alcohol sensitivity were performed. Results: Acetaldehyde concentration [median (25%, 75%)] in mouth air was 170.7 (73.5, 306.3) ppb. Acetaldehyde concentration in participants with a tongue coating status score of 3 was significantly higher than in those with a score of 1 (p&lt;0.017). After removing tongue coating, acetaldehyde concentration decreased significantly (p&lt;0.05). Acetaldehyde concentration was not correlated with other clinical parameters, presence of Candida species, smoking status or alcohol sensitivity. Conclusion: Physiological acetaldehyde concentration in mouth air was associated with tongue coating volume.

    The detection of infective endocarditis (IE) of oral origin has been previously discussed. However, there are few reports confirming this infection using molecular biological techniques. We herein describe the case of a 67-year-old man who developed IE. Blood culture samples and strains obtained from the gingival and buccal mucosa showed 100% identity to Enterococcus faecalis JCM 5803 on sequencing of 16S rRNA gene fragments. A random amplification of polymorphic DNA (RAPD) analysis showed the same pattern for these samples, thus confirming the identity of E. faecalis isolates in the blood and oral mucosa. Our observations provide novel information regarding the level of identity between IE pathogens and oral bacteria.

    We encountered a 74-year-old male patient with tongue laceration after convulsive seizures under intensive care. The tongue showed severe swelling, and the right ventral surface had been lacerated by his isolated and pointed right lower canine. Our university hospital has established a perioperative management center, and is promoting interprofessional collaboration, including dentists, in perioperative management. Dentists collaborating in the perioperative management center took dental impressions, with the support of anesthesiologists who opened the patient's jaw under propofol sedation, to produce a mouth protector. By raising the patient's bite, the completed mouth protector prevented the isolated tooth from contacting the tongue and protected the lacerated wound. Use of the mouth protector prevented the lacerated tongue from coming into contact with the pointed tooth, and the tongue healed gradually. These findings underscore that interprofessional collaboration including dentists can improve the quality of medical care.

    Occlusal support may be an important factor affecting nutritional support after major surgery. This report presents a patient who gained body weight after receiving a new prosthesis. The patient was an 82-year-old man with thoracic esophageal carcinoma. He did not have occlusal support because of multiple caries lesions. His body weight slowly increased after surgery, but almost stopped in the period of 54 to 68 days after surgery. After treatment with dentures (day 72 postsurgery), body weight gain was observed again, although his medical treatment had not changed. An appropriate prosthesis could contribute to perioperative nutrition support and may lead to earlier recovery after surgery.

    To investigate the associations of aural symptoms, headache and depression with the presence of temporomandibular disorder (TMD) symptoms in a young adult population in Japan. A personal interview survey was conducted on first-year university students (n = 1,930) regarding symptoms of TMD, aural problems, headache, shoulder pain and depression. Logistic regression was applied to assess the associations of these problems with the presence of TMD symptoms after controlling for age and gender. Among the 1,930 students, 543 students exhibited TMD symptoms and were classified into 7 groups: clicking only (Group I, n = 319), pain in the TMJ only (Group II, n = 21), difficulty in mouth opening only (Group III, n = 18), clicking and pain (Group IV, n = 29), clicking and difficulty in mouth opening (Group V, n = 48), difficulty in mouth opening and pain (Group VI, n = 11), and combination of three symptoms (Group VII, n = 97). The control group (n = 1,387) were subjects without any TMD symptoms. After adjusting for age and gender, a strong association was observed between TMD symptoms (Group II and IV) and tinnitus (OR = 12.1 and 13.2, respectively). TMD symptoms (Group I, II and III) were also associated with vertigo and headache. Otalgia and depression were significantly associated with the presence of clicking only. TMD symptoms were significantly correlated to aural symptoms and headache. A functional evaluation of the stomatognathic system should be considered in subjects with unexplained aural symptoms and headache.

    Background: Serum albumin concentration is known to be an independent predictor of survival in head and neck cancer. The previous studies suggested relationships between serum albumin concentration and oral health status in populations without serious systemic disorders. However, these relationships remain unclear in patients with head and neck cancer. The purpose of this study is to investigate the relationship between serum albumin concentration and oral health status in patients with head and neck cancer.
    Methods: Fifty individuals diagnosed with primary head and neck cancer, 25 individuals with normal serum albumin concentration (&gt;= 3.85 g/dL), and 25 age- and sex-matched individuals with lower serum albumin concentration (&lt;3.85 g/dL) were analyzed. General status, including cancer stage, body mass index, drinking and smoking habits, and biochemical serum markers, were evaluated. Oral health status, including periodontal condition and occlusion tooth pairs, were also evaluated.
    Results: Mean clinical attachment level (CAL) and C-reactive protein in the lower serum albumin concentration group were greater than those in the normal serum albumin concentration group (P = 0.009 and P = 0.002, respectively). However, there were no significant differences in any other oral and/or serum parameters between the two serum albumin groups. A logistic regression model showed that mean CAL was significantly associated with high or low levels of serum albumin concentration (odds ratio = 9.752; 95% confidence interval = 1.702 to 55.861; P = 0.011).
    Conclusions: This study suggests an association between periodontal disease and serum albumin concentration in patients with head and neck cancer. Longitudinal studies are necessary to examine the causal relationship between serum albumin concentration and periodontal condition. J Periodontol 2012;83:1110-1115.

    岡山大学の新入生を対象とした歯科健診の結果について、性差および経年的変化の検討を行った概要を報告する。2008年度から2011年度の新入生のうち18〜19歳の者を抽出し、総計5,097名(男性2,913名、女性2,184名)を対象に口腔健康状態を分析した。う蝕の保有者は全体の7.4%〜16.1%で、性差および経年的な傾向はなかった。一方、4mm以上の歯周ポケットを有する者の割合はCPIでは11.0%〜22.6%であり、経年的に軽症化にあった。男女比較では、プロービング時出血の割合、歯垢指数、歯石指数のすべてにおいて女性が有意に少なかった。さらに、智歯が1本も萠出していない者の割合は68.6%〜72.1%で、有意に女性が多かったが、年度間の差はなかった。不正咬合の割合は27.5%〜38.9%で経年的に減少し、歯科矯正治療経験者の割合は9.1%〜12.2%で経年的に増加していた。また、男女比較では両割合ともに女性が有意に多かった。以上より、大学新入生において、女性は男性に比べて歯周状態および口腔衛生状態が優れている一方で、不正咬合を有する者が多かった。また、経年的にはう蝕の保有者の割合には一定の傾向はなかったが、歯周状態と不正咬合は改善傾向にあった。今後も大学生の口腔の健康を保持・増進するために、歯科健診や保健指導を行う必要がある。(著者抄録)

    Periodontitis induces excessive production of reactive oxygen species in periodontal lesions. This may impair circulating pro-oxidant/anti-oxidant balance and induce the oxidation of low-density lipoprotein (LDL) in blood. The purpose of this study was to monitor circulating oxidized LDL and oxidative stress in subjects with chronic periodontitis following non-surgical periodontal treatment. Plasma levels of oxidized LDL and oxidative stress in 22 otherwise healthy non-smokers with chronic periodontitis (mean age 44.0 years) were measured at baseline and at 1 and 2 months after non-surgical periodontal treatment. At baseline, chronic periodontitis patients had higher plasma levels of oxidized LDL and oxidative stress than healthy subjects (p &lt; 0.001). Periodontal treatment was associated with a significant reduction in plasma levels of oxidized LDL (oxLDL)(p &lt; 0.001) and oxidative stress (p &lt; 0.001). At 2 months after periodontal treatment, the degree of change in the oxLDL was positively correlated with that in the oxidative stress (r = 0.593, p = 0.004). These observations indicate that periodontitis patients showed higher levels of circulating oxLDL and oxidative stress than healthy subjects. In addition, improved oral hygiene and non-surgical periodontal treatment were effective in decreasing oxLDL, which was positively associated with a reduction in circulating oxidative stress.

    口唇閉鎖力は、主に口輪筋の機能を反映している。口輪筋は発音や咀嚼などさまざまな口腔機能に関連している。口唇閉鎖力は咀嚼能力や不正咬合に関連がある。一方、裸眼視力1.0未満(&lt;20/20)の小学生は、日本においてこの20年間増え続けている。小学生の視力低下を予防することは、重要な課題である。視力は眼輪筋の強さに関連する可能性がある。眼輪筋は口輪筋とともに顔面筋の一つであり、両者ともに顔面神経の支配を受けており、頬筋などを介して機械的にもつながっており、協調して運動する。口輪筋の強さは、眼輪筋の強さを反映していると推測される。本研究では、口唇閉鎖力が視力に関連しているという仮説を立て、小学生の視力と口唇閉鎖力の関連性を横断的に検討することを目的とした。対象は岡山市内の7歳から12歳までの小学生396名(男子197名、女子199名)とした。口腔内状態と視力は、定期健康診断の結果を用いた。口唇閉鎖力は、LIP DE CUM/LDC-110(コスモ計器、東京)を用いて測定した。典型的な不正咬合を有する16名を除外し、高視力群(≧20/20)264名と低視力群(&lt;20/20)116名に分けた。統計分析には、Mann-Whitney U検定、χ2検定、ロジスティック回帰分析を用いた。高視力群では、口唇閉鎖力(p=0.024)と男子の割合(p=0.045)が有意に高く、年齢(p=0.001)とヘルマンの歯齢(p=0.002)が有意に低かった。性別、年齢、ヘルマンの歯齢で調整しても、低視力群では口唇閉鎖力が有意に低かった(OR 1.65、95% CI 1.04-2.64、p=0.004)。小学生において、視力と口唇閉鎖力には関連があることが示唆された。(著者抄録)

    Background and Objective: Elimination of pathogens is the main aim of periodontal treatment; however, modulation of the host immune response should also be considered. This study aimed to evaluate the effects of mechanical stimulation on periodontal healing in rats.
    Material and Methods: Before starting the experiment, lipopolysaccharide and proteases were applied once a day, for 4 wk, to both maxillary first molars of 30 rats to induce periodontal disease, and the application was stopped at the end of the 4-wk period. The experiment started immediately following this pretreatment. In the experiment, the left palatal gingiva was stimulated once daily using a powered toothbrush and the right gingiva served as a control (no mechanical stimulation). Pathological changes, and proliferation and cell death in periodontal tissues, were evaluated histometrically and immunohistochemically at baseline (0 wk), and at 1 and 3 wk of stimulation.
    Results: The control showed a reduction of polymorphonuclear leukocyte infiltration in connective tissue and an increase in the numbers of gingival and periodontal ligament fibroblasts. Mechanical stimulation reduced polymorphonuclear leukocyte infiltration and the area of destroyed collagen in connective tissue, and increased the number of gingival fibroblasts; however, it had no effect on alveolar bone and root resorption or on the number of periodontal ligament fibroblasts.
    Conclusion: Mechanical stimulation accelerated the healing of gingival inflammation by reducing the infiltration of polymorphonuclear leukocytes and enhancing fibroblast proliferation and collagen synthesis.

    OBJECTIVE: Obesity has been implicated as a risk factor for periodontitis and non-alcoholic fatty liver disease (NAFLD). In NAFLD, elevated alanine aminotransferase (ALT) is associated with obesity. Although a possible interrelationship between liver function and periodontitis has been reported among the middle-aged population, the correlation in young adults is little known. This study was designed to investigate the relationship between ALT and the presence of periodontitis in university students in Japan. MATERIAL AND METHODS: Medical and oral health data were collected in a cross-sectional examination conducted by the Health Service Center of Okayama University. Systemically healthy, non-smoking students aged 18 and 19 years old (n = 2225) were included. The protocol of the United States National Health and Nutrition Examination Survey was applied. Subjects with probing pocket depth >or= 4 mm were defined as having periodontitis. Logistic regression analysis was used to estimate the association between ALT, body mass index and periodontitis. RESULTS: The number of subjects with periodontitis was 104 (4.7%). In males, having periodontitis was significantly associated with an increased level of ALT (>or= 41 IU/l) in logistic regression analysis (adjusted odds ratio 2.3; 95% confidence interval 1.0-5.2; p < 0.05). However, there was no significant association between periodontitis and ALT in female students. CONCLUSIONS: Elevated ALT could be a potential risk indicator for periodontitis among young males. Monitoring hepatic abnormalities to prevent periodontitis must be better understood, even in the young adult population.

    P&gt;The purpose of this study was to examine the relationship of dietary preference to bite force and occlusal contact area in Japanese elementary school children. A total of 348 children, aged 7-12 years, from two public elementary schools located in Okayama Prefecture, Japan, participated in the study. Clinical examination included decayed, missing and filled teeth (dmft and DMFT), and total numbers of deciduous and permanent teeth. Bite force and occlusal contact area were measured using a pressure-detecting sheet. Dietary preference was assessed using a questionnaire in which the answers were given in like/dislike form. Mann-Whitney U-test and multiple logistic regression analysis were applied to analyse the data. In multiple logistic regression analysis after adjustment for age, gender and total number of teeth present, children who liked cabbage and celery showed significantly higher bite force (P = 0 center dot 05 and P &lt; 0 center dot 01, respectively) than those who disliked these. Children who liked cabbage and celery also showed higher occlusal contact area (P &lt; 0 center dot 05 and P &lt; 0 center dot 01, respectively) than those who disliked these. The Japanese elementary school children who liked hard foods such as cabbage and celery showed higher bite force and higher occlusal contact area than those who disliked these foods. A positive attitude towards harder food items might contribute to healthy development of the masticatory apparatus.

    The purpose of this study was to investigate the oral health status of Japanese university students and evaluate the relationship between periodontal health and related factors using a self-reported questionnaire. Undergraduate students (n=2,422) at Okayama University, aged 18-28 years, received oral examinations in 2008. Periodontal examination was conducted according to the protocol of the United States National Health and Nutrition Examination Survey III. The results showed that the number of teeth present was 28 (22-32) [median (range)]. The percentage of subjects with decayed teeth was 13.6%. Subjects with periodontal disease (at least one tooth with a probing pocket depth &ge;4mm) comprised 4.9%. According to the questionnaire survey, the prevalence of temporomandibular joint (TMJ) sounds was 26.5%; TMJ pain, 8.0%; and mouth opening limitation, 8.9%. The percentage of subjects with repeated stomatitis was 18.7%. There were gender differences in the number of decayed teeth, periodontal condition, and complaints of symptoms of temporomandibular disorders. There was a significant association between periodontal health and the following four questions: "Do you visit a dental office regularly?", "Do you notice bloody gums?", "Do you think that you have gingival swelling?", and "Do you think you have periodontal disease?"

    BACKGROUND: Elevated levels of blood reactive oxygen species (ROS) are associated with the severity of periodontitis. Therefore, improvement of periodontitis may result in a decrease in blood ROS. However, it is unclear how periodontal treatment affects blood ROS. Recently, reactive oxygen metabolites (ROMs) were recognized as a useful measure of blood ROS. The aim of this longitudinal study was to investigate the effect of non-surgical periodontal treatment on plasma ROMs in patients with chronic periodontitis. METHODS: Nineteen subjects with chronic periodontitis (mean age: 46.8 years) were monitored at baseline (prior to scaling and root planing) and 1 and 2 months after therapy. Dental health parameters were evaluated, and plasma was obtained at these time points from patients and controls (19 subjects without periodontitis; mean age: 45.3 years). The plasma ROM level was determined using a spectrophotometric technique. RESULTS: At baseline, patients with chronic periodontitis had higher plasma ROM level (441.8 +/- 71.1 Carratelli units) than the control subjects (324.4 +/- 34.0 Carratelli units; P <0.01). Probing depth, clinical attachment level, and bleeding on probing in patients with chronic periodontitis showed a significant improvement 2 months after non-surgical periodontal treatment, and this was accompanied by a significant reduction in plasma ROM level (P <0.01). CONCLUSIONS: In patients with chronic periodontitis, non-surgical periodontal treatment was effective at improving clinical parameters and reducing plasma ROMs. The improvement in chronic periodontitis by non-surgical periodontal treatment might offer clinical benefits by decreasing blood ROS.

    Background: The relationship between systemic antioxidative status and periodontal condition has been investigated in epidemiologic studies. However, little literature is available with regard to the correlation between systemic reactive oxygen species and periodontal condition. The purpose of this cross-sectional study was to investigate the relationship between plasma reactive oxygen metabolites (ROM) and periodontal condition in patients in the maintenance phase of periodontal treatment.
    Methods: Eighty-one subjects (mean age: 57.4 years) who had entered a periodontal maintenance program were examined for probing depth (PD) and clinical attachment level (CAL). Plasma levels of ROM and biologic antioxidant potential (BAP) were determined with a free radical electric evaluator.
    Results: The plasma level of ROM was positively correlated to mean CAL (r = 0.281; P = 0.011) and percentage of teeth with CAL A mm (r = 0.236; P = 0.034), but not mean PD (r = 0.196; P = 0.080). Logistic regression analysis showed that subjects with ROM levels &gt;400 Carratelli units (CARR U) had significantly higher mean CAL compared to subjects with ROM &gt;400 CARR U after adjusting for age, gender, and the number of teeth present (P = 0.011). However, the plasma level of BAP was not significantly correlated with the periodontal parameters.
    Conclusions: A positive association was found between plasma oxidative status and CAL in patients in the maintenance phase of periodontal therapy. A systemic increase in oxidative stress may influence the rate of progression of periodontal disease. J Periodontol 2008; 79:2136-2142.

    Objective: Gingival cells respond to periodontal pathogens by generating reactive oxygen species, and such a condition would increase circulating oxidative stress. Improvement of gingival inflammation by toothbrushing may offer clinical benefits on not only periodontal health but also the circulatory conditions. We examined the effects of mechanical stimulation on the plasma 8-hydroxydeoxyguanosine level in a rat periodontitis model.
    Design: In this experiment, male Wistar rats (n = 18) were divided into three groups. The control group received topical application of pyrogen-free water to the gingival sulcus for 8 weeks, while the other two groups received topical application of bacterial pathogens (lipopolysaccharide and proteases). After 4 weeks, half of the rats in the experimental groups received daily mechanical stimulation with an electric toothbrush for 4 weeks.
    Results: Rats treated with bacterial pathogens presented periodontal tissue damage and increased plasma levels of 8-hydroxydeoxyguanosine. Mechanical stimulation by toothbrushing decreased gingival inflammation and oxidative DNA damage indicated by a decrease in plasma 8-hydroxydeoxyguanosine.
    Conclusions: Mechanical stimulation of periodontally involved gingiva reduced 8-hydroxydeoxyguanosine in plasma and may contribute to a reduction in circulating oxidative stress associated molecules. (C) 2007 Elsevier Ltd. All rights reserved.

    Teenagers are particularly susceptible to gingivitis, and its prevalence increases with age. We proposed the use of electric toothbrushes for maintaining gingival health in high school students. Thus, we evaluated the relative ability of the oscillating/pulsating electric toothbrush Oral-B[○!R] ProfessionalCare[○!R] 8000 (PC) and side-to-side motion electric toothbrush Sonicare[○!R] elite[○!R] e9800 (SE). Moreover, this study investigated the perceived comfort and safety of and preference for electric toothbrushes of high school students using a questionnaire, designed to evaluate the characteristics of the two powered toothbrushes. Dental examination was performed at a high school in all 1st-3rd grade students (15-18 years old, 956 students). Sixty-five students were diagnosed with gingivitis. All subjects were informed about the aims of the study, and 59 gave signed, informed consent. Among the 59 students who entered the study, 48 completed the protocol. Eleven subjects were excluded from the study because they were wearing fixed orthodontic appliances (3 subjects), using antibiotics (3 subjects), or showed poor brushing compliance (5 subjects). This study followed a matched, parallel, examiner-blind design. Subjects were scored regarding the probing depth, bleeding on probing (BOP), and the Turesky et al. modification of the Quigley & Hein index (PlI) in 10 index teeth (17,16,11,26,27,37,36,31,46,47) at the baseline. The PC and SE groups were matched by school grade, sex, the percentage of BOP, PlI, and probing depth. Subjects were instructed to brush twice daily for 2 min without dentifrice. They were given professional instructions for approximately 5 min on the use of either toothbrush. The probing depth, percentage of BOP, P1I, and gingival abrasion were recorded after 2, 4, and 8 weeks. At the end of the study (after 8 weeks of brushing), all subjects completed a questionnaire. Statistical analysis was performed using the Mann-Whitney U test for non-parametric, unrelated samples, multiple comparisons test for non-parametric, related samples, and Fisher's exact probability test for subjective symptoms. Overall, there was no significant difference between the two groups in any indices. During the study period, the multiple comparisons test showed a significant improvement in all indices for both powered toothbrushes. There were 4 sites of gingival abrasion in PC and 0 in SE. At 8 weeks, no gingival abrasion was observed in either group. The number of subjects who exhibited bleeding on brushing and oral malodor in both powered toothbrush groups decreased from the baseline to 8 weeks. Some users felt that the head and sound of both electric toothbrushes were too large. In conclusion, PC and SE improved the percentage of BOP, PlI, and probing depth in high school students with gingivitis. The use of both electric toothbrushes can be considered safe untill 8 weeks. PC and SE had almost the same effects on gingivitis in high school students. For prospective electric toothbrushes, a smaller brush head and more pleasant sound while operating would be desirable.

    2007年10月〜2008年7月に耳鼻咽喉科または形成外科に入院し、カンファレンスで取り上げられた頭頸部癌チーム医療関連入院患者140名(男103名、女37名)を対象に予防歯科の関わりについて検討した。入院患者は月平均14名で、男女共に50歳代〜70歳代が多く、60歳代がピークであった。予防歯科に紹介された入院患者は32名であった。癌の部位別では上下歯肉癌が51.9%と最も多く、約半数の患者は補綴科に紹介された。唾液腺癌や甲状腺癌患者の予防歯科紹介は無く、喉頭癌患者の紹介は少なかった。治療法別では化学療法と手術併用療法が13名(40.6%)と最も多く、次いで手術のみが12名(38%)であった。治療開始日から紹介日までの期間は化学療法で平均6.5日後、放射線療法では17日後であり、手術適応患者では単独、併用療法ともに平均15日後に紹介されていた。

    Background: Epidemiologic studies suggest a relationship between periodontitis and liver diseases. A rat periodontitis model was used to investigate whether a causal relationship exists between periodontitis and liver diseases.
    Methods: Fourteen male Wistar rats (8 weeks old) were divided into two groups: a periodontitis group in which Escherichia coli lipopolysaccharide (LPS) and Streptomyces griseus proteases were applied into the gingival sulcus for 8 weeks, and a control group using pyrogen-free water instead. After blood samples were collected, periodontal tissues and liver specimens were analyzed.
    Results: Chronic administration of LPS and proteases to the gingival sulcus induced periodontitis and liver injury, including steatosis with inflammation and sinusoidal fibrosis. Apoptosis, enhanced concentration of 8-hydroxydeoxyguanosine, and activated production of tumor necrosis factor-alpha in liver were observed in the periodontitis group, with increased gingival inflammation, serum LPS, and reactive oxygen species.
    Conclusion: Periodontal inflammation in a rat model induced fatty liver disease through increased serum LPS.

    症例1:52歳男性。舌癌で放射線治療適応となり、組織内照射約70Gyを行った後、口腔乾燥を訴えるようになった。ガムテストでは6.2cc/10min.であった。放射線治療後の口腔乾燥症として塩酸ピロカルピン15mg/day投与と、齲蝕予防のために1〜2ヵ月に1回のフッ素塗布、連日のフッ素洗口を開始した。さらに家庭においてフッ化物配合歯磨剤を用いたブラッシングを勧めた。症例2:63歳男性。舌癌で放射線治療を希望し、外照射20Gyの後、組織内照射約70Gyを行った後、口腔乾燥を訴えるようになった。ガムテストでは5cc/10min.であった。放射線治療後の口腔乾燥症として塩酸ピロカルピン15mg/day投与と、齲蝕予防のために3週間に1回のフッ素塗布を開始した。さらに家庭においてフッ化物配合歯磨剤を用いたブラッシングを勧めた。症例1では自覚的にも他覚的にも口腔乾燥を脱し、副作用発現なく投与10週後に中止した。症例2では副作用の頻尿感などで投与10日後に中止となったが、2週間後には自覚的にも他覚的にも口腔乾燥感の改善がみられた。ともに齲蝕のコントロールは良好である。

    Epidemiological evidence exists for an association between vitamin C intake and periodontal health. Understanding of the effects of vitamin C on periodontitis is important to improving periodontal therapy. We studied the effects of topical application of a dentifrice containing vitamin C on periodontitis in the rat model. Periodontitis was induced in 8-week old male Wistar rats (n=50) with a combination of lipopolysaccharide and proteases. After 4 weeks, rats were divided into 5 groups, receiving topical application of dentifrice containing 0 (periodontitis group) , 1, 2, 3, or 5% vitamin C for 3 weeks. Collagen density increased and polymorphonuclear leukocyte number decreased with increasing concentration of vitamin C, and the 3% and 5% vitamin C treatment groups differed significantly from the periodontitis group in this regard. Gingival expression of 8-hydroxydeoxyguanosine (a marker of oxidative stress) was lower in the 1, 2, 3, or 5% vitamin C treatment groups than in the periodontitis group. These results show that topical application of a dentifrice containing&ge;3% vitamin C to the gingival margin decreases polymorphonuclear leukocyte numbers and induces collagen synthesis in rat periodontitis.     The objective of this study was to compare the effects of four pastes-regular toothpaste as a control (CT), regular toothpaste containing either dipotassium glycyrrhizinate (GLY), dipotassium glycyrrhizinate with Create Ion^[○!R] High Power (HP), or dipotassium glycyrrhizinate with Create Ion^[○!R] Number One (No 1), on periodontitis in the rat model. In addition, the effects of mechanical stimulation of gingiva when combined with the pastes were also evaluated. Under general anesthesia, lipopolysaccharide and bacterial protease solution were applied once a day into the palatal gingival sulcus of the upper right and left first molars of 40 male Wistar rats to induce periodontitis for 8 weeks. From the 5th to 8th week, 4 toothpastes (n=10) were applied in the gingivae of the right and left maxillary first molar once per day for 10 minutes respectively. Also, the palatal gingiva of the left molar was stimulated once a day using a power toothbrush. After 8 weeks, the teeth and gingivae were fixed, decal-cified and embedded in paraffin. Buccolingual sections were evaluated using hematoxylin-eosin staining. Statistical differences among the 4 groups and between stimulated and non-stimulated sides were tested by the ANOVA or Kruskal-Wallis test, and the Student's t-test or Mann-Whitney test, respectively. In the No 1 group, the number of polymorphonuclear leukocytes in the junctional epithelium was significantly decreased. In the HP group, the number of polymorphonuclear leukocytes in connective tissues was significantly less than that in the other groups. The stimulated side had a lower number of polymorphonuclear leukocytes in the junctional epithelium and connective tissue than the non-stimulated side in the GLY group, and the values were similar to those for the stimulated sides of the HP and No 1 groups. In conclusion, Create Ion^[○!R] High Power and Create Ion^[○!R] Number One with dipotassium glycyrrhizinate reduces polymorphonuclear leukocyte infiltration into connective tissues and junctional epithelium, respectively. Mechanical stimulation of gingiva combined with dipotassium glycyrrhizinate reduces polymorphonuclear leukocytes in junctional epithelium and connective tissue as strongly as dipotassium glycyrrhizinate and Create Ion^[○!R] without mechanical stimulation.

    Background: Apical migration of junctional epithelium (JE) occurs in association with periodontal pocket formation. The aim of this study was to investigate the gingival changes occurring during apical migration of the JE following application of factors associated with inflammatory periodontal disease pathogenesis.
    Methods: Six-week-old male Wistar rats were divided into six groups: three experimental groups to investigate gingival changes following 2, 4, and 8 weeks topical application of lipopolysaccharide (LPS) and proteases and three control groups using pyrogen-free water. After 2, 4 or 8 weeks, nuclear DNA fragmentation was detected in periodontal ligament (PDL) fibroblasts using the terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end-labeling (TUNEL) method, and proliferative activities of the basal cells and fibroblasts were evaluated through expression of proliferating cell nuclear antigen (PCNA). Collagen destruction was examined histologically.
    Results: Gingiva treated with LPS and proteases showed an increase in PCNA-positive basal cells but not the fibroblasts. Collagen destruction was observed at 2 weeks; apical migration of the JE and TUNEL-positive fibroblasts was seen at 4 weeks.
    Conclusions: Following application of LPS and proteases to rat gingival sulci, the apical migration of the JE appears to occur simultaneously with the apoptosis of PDL fibroblasts, which in turn follows proliferation of the basal cells and collagen destruction.

    Objectives: Lipopolysaccharide (LPS) and proteases have been implicated as important factors in the initiation and progression of human periodontal diseases. A single application of LPS or proteases is insufficient to induce periodontal pocket formation or periodontitis. The aim of the present study was to assess the combined effect of lipopolysaccharide and proteases on rat periodontal tissues, and create a periodontal disease model.
    Materials and methods: Forty male Wistar rats were divided into four groups: combination group (treated with both LPS and proteases solutions); LPS group; proteases group; and control. Each solution was introduced daily into the palatal gingival sulcus of maxillary molars for 8 weeks. The tissues were evaluated histometrically and immunohistochemically.
    Results: In the LPS group, elongation of rete ridge, apical migration of junctional epithelium (JE), increased numbers of B cells in connective tissue, and resorption of alveolar bone were observed. In the proteases group, the increase in the number of infiltrating polymorphonuclear leukocytes and blood vessels in the connective tissue was greater than that of the LPS group.
    Conclusions: The effects of LPS on periodontal tissues differed from those of proteases. The addition of proteases augmented and increased the effects of LPS, which were apical migration, intraepithelial cleavage of JE, and increased B cell density. The lesions in the combination group resembled established lesions of human periodontitis, with the exception of the low density of plasma cells.

    <p> Abstract Okayama University Dental School implemented an outpatient escort practicum as a form of early clinical exposure. It is useful to clarify how students perceived the practicum after they finished their clinical training to inform improvements. The purpose of this study was to evaluate an outpatient escort practicum implemented with first-grade students from the perspective of those students after they had completed clinical training.</p><p> The evaluation was performed twice, with the same students completing the questionnaires. The first evaluation was conducted immediately after the outpatient escort practicum in the students' first grade, and the second after they had completed their clinical training (sixth grade). The participants were 82 dental students who completed both questionnaires about the outpatient escort practicum.</p><p> Overall, the second evaluation was significantly lower than the first evaluation. Students who positively evaluated the practicum in their first grade but gave a negative evaluation in their sixth grade reported less appreciation for the practicum in terms of understanding the patients' perspectives and understanding the communication skills required by future healthcare professionals. Students who positively evaluated the practicum both times highlighted that the practicum helped them to understand the communication skills required by future dentists. The study findings indicated that the practicum was more positively evaluated by students immediately after they had completed it than by the same students 5 years later. However, the practicum was positively evaluated by the participating students after they had completed clinical training to some extent.</p>

    本研究は，「術後絶食中のガム咀嚼が，食道がん手術後の予後を改善できるのか」という「問い」を検証するものである。
    今年度は、研究計画の再検討、倫理委員会申請書類作成を行った。
    科研申請時は、食道がん手術を受ける患者を対象に，術後絶食期間中に，従来の口腔衛生管理のみを行う「対照群」と，術後絶食中にガム咀嚼による口腔機能訓練を加えて行う「介入群」にランダムに割付け，2群間で比較して，食道がん術後絶食中のガム咀嚼による咀嚼機能や嚥下機能の維持効果，予後改善効果を検討する、としていた。
    その後、具体的な研究方法について、倫理委員会、各科の医師・歯科医師、パラメディカル、統計専門家等と議論の結果、過去の我々の研究でえた結果を対照群とし、周術期にガム咀嚼を行う1群のみの介入研究とすることとなった。当初、2年半ほどの期間で目標症例数は70例程度と考えていたが、統計専門家との議論の結果、多くても40症例程度で結果がでることが明らかになった。そのため、当初の予定より、研究実施期間は短くて済む予定である。
    現在、研究分担者・協力者、本学倫理委員会と協議し、書類作成を進めている。倫理委員会で承認され次第、研究を始める予定である。
    なお、今のところ、新型コロナウイルス感染拡大に伴い、新たな研究開始の自粛が求められている。新型コロナウイルス感染予防対策には十分に配慮し、研究を進めていく方針である。

    本研究の目的は、頭頸部がんの治療を受けた患者における、口腔機能の改善と、QOLの改善との関係を明らかにすることである。2019年度は、頭頸部がん治療が、口腔機能の低下を介して、どのように治療後のQOLに影響を与えているかを検討した。
    具体的には、頭頸部がん治療後の患者に対し、QOLを評価するアンケート（European organization for research and treatment of cancer quality of life core questionnaire-30）を実施した。また、口腔機能では、口の中の乾燥度、舌の力、口の開き具合、咬み合わせ、オーラルディアドコキネシス（1秒間に「パ」「タ」「カ」を発声できる回数）を評価した。さらに、がんのステージや、治療方法を評価した。得られた結果をもとに、QOLとその他の因子の関係を調べるため、パス解析を行った。
    91名の頭頸部がん治療後の患者を評価した。そのうち75名の患者について分析を行った。平均年齢は64.5歳、男性43名（57.3％）であった。パス解析によって、頭頸部がんのステージが進行していること、および頸部郭清術・再建手術・化学療法を含むがん治療が、舌の力を低下させ、口を開けづらくさせ、「パ」「タ」「カ」を言いづらくさせることを介して、治療後のQOLを低下させることが明らかとなった。以上の結果により、術後のQOL改善に、舌の力や、口の開き具合、オーラルディアドコキネシスを改善させることが有効であることが示唆された。

    To promote multi-disciplinary team approach for perioperative oral management, we considered an efficient method using the simple self-check sheet, consisted a sentence “Do you have concerns with your mouth when undergoing surgery?”, which developed to assess requirements of professional dental interventions. To evaluate of the availability of the developed simple self-check sheet, the sensitivity and specificity were investigated. The results showed the simple self-check sheet had low sensitivity but high specificity. Therefore, patients, with some oral concerns by simple self-check sheet, should be examined and managed by dental professionals, at least. And routine pre-examination under general anesthesia by dental professionals is desirable for surgical safety, because of low sensitivity.

    In the esophageal cancer patients who underwent esophagectomy, we investigeted that the relationship between oral health status and prognosis index, the status of oral flora during neoadjuvant chemotherapy, the relationship between oral health status and nutritional status after surgery.
    In consequence, these were suggested that the significant relationship between prognosis index and occlusal support, a tendency for decreased biodiversity in the patients who had oral mucosal disorder during neoadjuvant chemotherapy, and possibility of the positive relationship between exacerbations of nutritional status after surgery and occlusal support.

    The purpose of the study is to investigate 1) the relationship between acetaldehyde concentration and microbiome on tongue, and 2) the related factors of tongue pressure decreasing and the effect of tongue excise. The relative abundance of Neisseria flacescens in high acetaldehyde group were significantly greater than those in the low group. The decline of tongue pressure significantly associated with length of intensive care unit (ICU) stay in patients after esophagectomy. The tongue pressure of patients who performed tongue excise were not significantly difference compared with no excise patients. In conclusion, the concentration of acetaldehyde in mouth air was related with Neisseria flavescens. The change of tongue pressure was associated with the length of ICU stay and was not effective from tongue excise in the patients after esophagectomy.

    The purpose of this study was to clarify the relation between attitude about visiting dental clinic and demand of dental treatment before surgery, and to clarify oral characteristics of the patients in the department of Digestive tract surgery, Cranial nerve surgery, Respiratory surgery, Gynecology, and Brest thyroid surgery. Other purpose of this study was, in the patients who undergo esophageal surgery, to investigate relation between oral status and a prognostic factor, and the change of a prognostic factor during neoadjuvant chemotherapy.
    As the results, the ratio of the patients who have to be treated before surgery was low in the patients who visit dental clonic at regular intervals, and the oral status of the patients who undergo Esophageal surgery was worse than that of the patients who undergo other surgery. The oral status in the patients who undergo Esophageal surgery was related with prognostic factor, and the change of a prognostic factor during neoadjuvant chemotherapy.

    The aim of this study was to investigate the relationship between acetaldehyde concentration in mouth air and tongue coating volume in healthy volunteers. Acetaldehyde concentration in participants with a tongue coating score of 3 was significantly higher than those with a score of 1. After removing tongue coating, acetaldehyde concentration was significantly decreased.
    Oral microflora on the tongue may be the main source of local production of acetaldehyde. Removing tongue coating may be a method inhibiting the production of acetaldehyde.

    Recently, it is noticed the importance of comprehensive food education (Shokuiku) against the lifestyle-related disease. In this study, we investigated whether eating until full, eating quickly, or both eating behaviors were associated with being overweight, the relationship between subjective and objective methods to evaluate eating quickly and to numerically characterize the kinesis of eating quickly. Also, we investigated the effects of intervention using a pamphlet or the combination of a pamphlet and dietary records on improvement of eating quickly. Furthermore, we investigated the relationship between knowledge about comprehensive food education and dental caries experience.

    Oral mucositis is one of side effect of radiation therapy and chemotherapy. The aim of this study was to clarify the preventive mechanism of oral mucositis from the view point of oxidative stress and microorganisms. As the results, oral mucositis had no relationship with oral microorganisms, oxidative stress in plasma and general status.
    Moreover, oral mucositis had relationshipwith frequency of radiochemotherapy and chemotherapy more than oxidative stress in plasma. It was showed that initiation and severity of mucositis was not influenced by oxidative stress in plasma.

    Our previous study demonstrated that topical application of bacterial pathogens, i.e., lipopolysaccharide (LPS) and bacterial proteases, to gingival sulcus induced fatty liver. The purpose of this research project was to clarify the mechanism of fatty liver in rat periodontitis model, especially, to investigate the relationship between LPS and reactive oxygen species (ROS).
    In the year 2006, serum levels of LPS, ROS and tumor necrosis factor (TNF)-alpha were found to be elevated in rats with periodontitis compared to rats without periodontitis. Moreover, both concentrations of 8-hydroxydeoxyguanosine (8-OHdG) in liver homogenate and densities of 8-OHdG-positive liver cells were larger in the rats with periodontitis than in those without These results suggest that topically applied ITS affects liver directly, that cells in the periodontium respond LPS and produce ROS, that the ROS produced in periodontium affects liver directly, and that cytokines play some roles in the pathogenesis of fatty liver induced by periodontitis.
    In the year 2007, the effects of high cholesterol diet on the markers of oxidative stress in blood and liver of rats with periodontitis were investigated. Serum levels of reactive oxygen metabolites and hexanoyl-lysine and liver level of 8-OHdG were increased after feeding high cholesterol diet in the rats with periodontitis. These results suggest that high cholesterol diet has additive effects on periodontitis-induced oxidative damage in blood and liver.
