    This paper presents the results of a series of surveys conducted from July 2021 to March 2023 to investigate the post-vaccination adverse reactions to the mRNA-1273 (Moderna) vaccine among faculty, staff, and students at Okayama University. These studies complement the official surveys conducted by the Ministry of Health, Labour and Welfare (MHLW) and provide a more representative picture of adverse reactions in the general population including large numbers of healthy young people. Pain, swelling, redness at the injection site, fever, headache, and malaise were the main adverse reactions reported. The proportion of adverse reactions was generally higher after the second vaccination and decreased with each additional vaccination. No statistically significant differences in the adverse reactions were found for males and females and those with/without a history of allergy, but a lower proportion of fever was observed in older participants and those with underlying medical conditions. We also evaluated the association between adverse reactions and antibody titers after the third vaccination and found no significant differences in antibody levels one month after vaccination. This series of studies highlights the importance of conducting surveys in diverse populations to provide a more representative picture of post-vaccination adverse reactions during a pandemic.

    To investigate adverse reactions and attitudes toward the vaccine during the first month after mRNA- 1273 vaccination in a larger sample including younger men and women in Japan, we distributed a 1-month post-vaccination questionnaire using a Google form to 8,566 people who received a second dose of mRNA-1273 at Okayama University. The response rate was about 40.2% (3,447 responses), the sex ratio was about the same, and 73.3 % (2,528 respondents) were students in their twenties or younger. Poisson regression with robust variance was performed to calculate the prevalence ratio of each symptom by different attributes. The most common adverse reactions after the second vaccine dose were local pain (80.4%), fever (85.1%), malaise (82.0%), headache (64.0%), and chills (57.4%). Approximately 99% of respondents reported that their adverse reactions resolved within 1 week. Over 80% of respondents were satisfied with their vaccination (87.2%), expressed interest in receiving the third vaccination (83.3%), and would recommend vaccination to their loved ones (80.2%). However, among them, 22.0% (757 respondents) would recommend and 28.4% (980 respondents) also stated that they would consider the type of vaccine in these decisions.

    COVID-19 has been prevalent worldwide since 2019. Increasing COVID-19 vaccination coverage is an important measure to combat the disease. An online survey was conducted with university students and personnel who were vaccinated against COVID-19 at a mass vaccination event to examine the factors promoting vaccination among young adults. The online survey was conducted with persons vaccinated at Okayama University from June 5 to September 27, 2021. Although the number of those who had fever >37.5°C increased after the second vaccination compared to the first, the vaccinated persons got more satisfied after the second shot.

    BACKGROUND: Glycosylphosphatidylinositol-anchored high-density lipoprotein binding protein 1 (GPIHBP1) plays a crucial role in lipolytic processing. Previous studies have shown that GPIHBP1 mutations cause severe hypertriglyceridemia and that serum GPIHBP1 levels are marginally higher in patients with coronary heart disease; however, the role of GPIHBP1 in type 2 diabetes mellitus (T2DM) remains unknown. OBJECTIVE: We investigated the association between circulating GPIHBP1 levels and the prevalence of microvascular complications in T2DM. METHODS: A total of 237 subjects with T2DM and 235 non-diabetic control subjects were enrolled in this study. Their serum GPIHBP1 levels were evaluated using ELISA assays. RESULTS: Circulating GPIHBP1 levels were higher in patients with T2DM (952.7 pg/mL [761.3-1234.6], p < 0.0001) than in non-diabetic subjects (700.6 [570.8-829.6]), but did not differ in T2DM patients with or without hypertriglyceridemia. Serum GPIHBP1 levels were significantly higher in patients with T2DM with diabetic retinopathy (DR), diabetic nephropathy (DN), and microvascular complications than in those without these complications. Multivariable logistic regression and receiver operating characteristic (ROC) curve analyses revealed that the presence of microvascular complications, but not macrovascular complications, was independently associated with serum GPIHBP1 levels, which could predict the presence of diabetic microvascular complications. CONCLUSIONS: Elevated GPIHBP1 levels are associated with microvascular complications in T2DM and may help to predict their progression.

    <bold>Background:</bold> Although various biomarkers predict cardiovascular event (CVE) in patients with diabetes, the relationship of urinary glycan profile with CVE in patients with diabetes remains unclear.

    <bold>Methods:</bold> Among 680 patients with type 2 diabetes, we examined the baseline urinary glycan signals binding to 45 lectins with different specificities. Primary outcome was defined as CVE including cardiovascular disease, stroke, and peripheral arterial disease.

    <bold>Results:</bold> During approximately a 5-year follow-up period, 62 patients reached the endpoint. Cox proportional hazards analysis revealed that urinary glycan signals binding to two lectins were significantly associated with the outcome after adjustment for known indicators of CVE and for false discovery rate, as well as increased model fitness. Hazard ratios for these lectins (+1 SD for the glycan index) were UDA (recognizing glycan: mixture of Man5 to Man9): 1.78 (95% CI: 1.24–2.55, <italic>P</italic> = 0.002) and Calsepa [High-Man (Man2–6)]: 1.56 (1.19–2.04, <italic>P</italic> = 0.001). Common glycan binding to these lectins was high-mannose type of <italic>N</italic>-glycans. Moreover, adding glycan index for UDA to a model including known confounders improved the outcome prediction [Difference of Harrel's C-index: 0.028 (95% CI: 0.001–0.055, <italic>P</italic> = 0.044), net reclassification improvement at 5-year risk increased by 0.368 (0.045–0.692, <italic>P</italic> = 0.026), and the Akaike information criterion and Bayesian information criterion decreased from 725.7 to 716.5, and 761.8 to 757.2, respectively].

    <bold>Conclusion:</bold> The urinary excretion of high-mannose glycan may be a valuable biomarker for improving prediction of CVE in patients with type 2 diabetes, and provides the rationale to explore the mechanism underlying abnormal <italic>N</italic>-glycosylation occurring in patients with diabetes at higher risk of CVE.

    本学では留学生を積極的に受け入れる方針としており、最近5年間で増加している。今回、2014〜2018年度に入学し定期健診で胸部X線検査を受けた留学生のべ3107名のX線所見を調査し、経年的変化について検討した。各年度の有所見者割合は7.0〜18.5%でバラツキがあり、2016年度が最も高かった。要精査率は2.2〜4.0%で年度間の大差は認めなかった。結核高負担国からの留学生と高負担国以外からの留学生とで有所見率を比較すると、高負担国以外のほうがやや高く、要精査率についても同様であった。有所見や要精査と判定された部位を高負担国と高負担国以外で比較すると、高負担国では肺や心臓の占める割合が高く、高負担国以外では肺や心臓の占める割合は高負担国ほどではなく、椎骨の占める割合が高かった。

    本研究はSAP導入後の効果を検討するため、CSIIからSAPに変更後1年以上経過した1型糖尿病22症例を解析した後ろ向き観察研究である。HbA1cはSAP前と比較して6ヵ月(6M)では不変、12Mで有意に低下した(各々7.6±0.7%、7.4±0.9%、7.1±0.9%)。6Mの時点でHbA1c0.1%以上低下した群を6M低下群、変化なし又は0.1%以上上昇した群を6M不変・上昇群と定義したところ、6M不変・上昇群で0MのHbA1cが有意に低値で、重回帰分析では基礎インスリン比率の変化(6M)がHbA1c変化量(6M)の有意な決定因子として採用され、負の相関を示した。また、6M不変・上昇群では0MのSMBGで低血糖頻度が有意に高く、1Mで有意に低下した。臨床的にSMBGでの低血糖が多い症例ではSAP導入後にHbA1c上昇を呈す事もあり、注意を要する。(著者抄録)

    新入生が学生生活に良好に適応していくためには、大学や教職員がどのような配慮をすればよいかを探る目的で、新入生への無記名アンケートを実施した。調査対象は2017年度岡山大学の全新入生で、そのうち2051名(回収率90.4%)の回答を得た。入学後に不安を感じる事柄は、勉強面76.0%、人間関係43.8%、生活一般23.0%であった。勉強面で不安なことは、学力面53.1%、履修登録37.6%、部活動との兼ね合い23.5%であった。履修登録では、授業選択が困難63.8%、履修登録制度が判りにくい31.7%、web入力が困難12.7%であった。自力で履修登録ができた者は45.2%であった。パソコンが苦手・少し苦手な学生は78.1%であった。人前で話すことが苦手な学生の割合は37.7%であり、人前で話すことが苦手でない学生に比べて、勉強面への不安を持つものが有意に多かった。今回の調査結果から、新入生には、勉強面での不安を感じている学生が圧倒的に多いことが判明した。勉強面において、履修登録の教職員側からのさらなる支援、パソコン習熟授業の必修科目化、人前で話すことが苦手な学生を援助機関に繋げる啓発活動と合わせて授業内での教職員の配慮は、新入生のメンタルヘルス不調の発生を抑制すると推測された。このように新入生へのメンタルヘルスの講義と並行して、就学環境・システムに対する働きかけが、メンタルヘルス一次予防にとって重要であると考える。(著者抄録)

    本学では医療系学生全員に対し、臨床実習開始前に定期検査としてインターフェロンγ遊離試験を実施している。試験方法は、2007〜2010年度はクォンティフェロンTB-2Gを使用し、2011年度、2012年度、2016年度はクォンティフェロンTBゴールド(QFT-3G)、2013〜2015年度はTスポットTB(T-SPOT)を使用した。今回、QFT-3Gを使用した年度とT-SPOTを使用した年度とで陽性率や事後措置を比較検討した。結果、QFT-3GはT-SPOTに比べて陽性および判定保留の頻度が有意に高かった。ただしQFT-3Gは陽性率・判定保留率とも年度による差が大きいことから、結果の解釈に注意を要すると考えられた。

    Phosphatidylethanolamine N-methyltransferase (Pemt) catalyzes the methylation of phosphatidylethanolamine (PE) to phosphatidylcholine (PC) mainly in the liver. Under an obese state, the upregulation of Pemt induces endoplasmic reticulum (ER) stress by increasing the PC/PE ratio in the liver. We targeted the Pemt gene in mice to explore the therapeutic impact of Pemt on the progression of diabetic nephropathy and diabetes, which was induced by the injection of streptozotocin (STZ). Although the blood glucose levels were similar in STZ-induced diabetic Pemt+/+ and Pemt-/-mice, the glomerular hypertrophy and albuminuria in Pemt-/- mice were significantly reduced. Pemt deficiency reduced the intraglomerular F4/80-positive macrophages, hydroethidine fluorescence, tubulointerstitial fibrosis and tubular atrophy. The expression of glucose-regulated protein-78 (GRP78) was enriched in the renal tubular cells in STZ-induced diabetic mice, and this was ameliorated by Pemt deficiency. In mProx24 renal proximal tubular cells, the treatment with ER-stress inducers, tunicamycin and thapsigargin, increased the expression of GRP78, which was reduced by transfection of a shRNA lentivirus for Pemt (shRNA-Pemt). The number of apoptotic cells in the renal tubules was significantly reduced in Pemt-/- diabetic mice, and shRNA-Pemt upregulated the phosphorylation of Akt and decreased the cleavage of caspase 3 and 7 in mProx24 cells. Taken together, these findings indicate that the inhibition of Pemt activity ameliorates the ER stress associated with diabetic nephropathy in a model of type 1 diabetes and corrects the functions of the three major pathways downstream of ER stress, i.e. oxidative stress, inflammation and apoptosis.

    学生健診における尿糖陽性者への2次健診について、その意義と尿糖陽性者への事後処置の妥当性を、本施設(大学保健管理センター)で糖負荷試験を施行していた2004年以前と、随時血糖、75g糖負荷試験、尿検査を行い、HbA1c 6.0%以上や血糖著明高値の者は近隣の病院へ紹介を行っている2005年以降の結果の比較から検討した。その結果、尿糖陽性者に対する2次健診として、75g糖負荷試験群とHbA1c群を比較すると、75g糖負荷試験群でより高い頻度で耐糖能異常が指摘でき、HbA1cを指標とした場合、耐糖能異常を見逃す可能性が高いことが分かった。

    Purpose. The unique circulating microRNAs (miRNAs) observed in patients with type 2 diabetes (T2D) are candidates as new biomarkers and therapeutic targets. In order to identify circulating miRNAs relevant to the disease process in case of type 2 diabetes, we performed the Illumina sequencing of miRNAs derived from the serum, liver and epididymal white adipose tissue (WAT) of diet-induced obese male C57BL/6J mice.
    Basic Procedures. We selected four miRNAs, miR-101, miR-335, miR-375, and miR-802, which are increased in the sera and tissues of obese mice, and measured the serum levels of miRNAs in T2D and subjects with normal glucose tolerance (NGT).
    Main Findings. The serum concentrations of miRNAs, log10miR-101, log(10)miR-375, and log(10)miR-802, were significantly increased in the T2D patients compared with NGT subjects (1.41 +/- 2.01 u.s. -0.57 +/- 1.05 (P = 1.36 x 10(-5)), 0.20 +/- 0.58 v.s. 0.038 +/- 1.00 (P = 3.06 x 10(-6)), and 2.45 +/- 1.27 u.s. 0.97 +/- 0.98 (P = 0.014), respectively). The log(10)miR-335 values did not demonstrate any significant differences between the T2D and NGT groups (-1.08 +/- 1.35 v.s. 0.38 +/- 1.21 (P = 0.25)). According to the stepwise regression analysis, the HbA1c was an independent predictor of miR-101. Regarding the serum miR-802 levels, eGFR, HbA1c and HDL-C values were identified as significant determinants.
    Principal Conclusions. The present findings demonstrated that the circulating miR-101, miR-375 and miR-802 levels are significantly increased in T2D patients versus NGT subjects and they may become the new biomarkers for type 2 diabetes. (C) 2015 Elsevier Inc. All rights reserved.

    A 50-year-old man, whose second of two daughters was treated for type 1 diabetes, seen for polyuria and weight loss in August 2008 had been found to have HbA1C of 6.2% in an annual physical cheakup in April 2008. He was hospitalized for markedly elevated HbA1C of 10.3% and plasma glucose of 463 mg/dl detected in a blood test. Laboratory studies showed a urinary CPR concentration of 21.9 &mu;g/day and serum CPR of 0.2 ng/ml before and 0.4 ng/ml 6 minutes after glucagon administration, indicating decreased insulin secretion. Intensive insulin therapy was initiated. Although GAD antibody was negative, IA-2 antibody was positive (9.1 U/ml) , leading to a diagnosis of type 1 diabetes mellitus. HLA typing was DRB1*010101 and 090102, type 1 diabetes-susceptibility gene, identical to the typing of his second daughter. This case of elderly-onset of type 1 diabetes is interesting, manifesting negative GAD antibody and positive IA-2 antibody, especially with a family history of type 1 diabetes.<br>

    C57BL6/J mice were subjected to high fat-high sucrose (HFHS) and standard chow (STD), and total RNAs isolated from visceral adipose tissues, liver and sera. Small RNA library was subjected to Illumina sequencing. MiR-342 was identified by its up-regulation in adipose tissues and in liver. We generated miR-342 knockout (miR-342-/-) mice and investigated the role of miR-342 in obesity and glucose metabolism.
    MiR-342-/- mice were fed with HFHS and STD, and body weight, glucose tolerance, and insulin sensitivity were compared. Body weight in miR-342-/- mice was lower compared with miR-342+/+. By glucose tolerance tests, glucose intolerance was ameliorated in miR-342-/- mice. Insulin sensitivity was improved in miR-342-/- mice demonstrated by insulin tolerance tests. Taken together, obesity, glucose tolerance and insulin sensitivity were improved in miR-342-/- compared with miR-342+/+ mice under HFHS chow. miR-342 may be a new therapeutic target for the treatment of metabolic syndrome.
