    背景:左腋窩リンパ節生検において、捺印細胞診が診断の契機となったALK(Anaplastic lymphoma kinase)陽性ALCL(Anaplastic large cell lymphoma)の一例を経験したので報告する。症例:40代、女性。近医にて、発熱および左腋窩リンパ節腫大で経過観察していたが、痛みが出現し、当院血液内科に紹介された。血液検査で可溶性IL-2レセプターが6,764U/mLと高値であり、リンパ節生検が施行された。組織像ではリンパ節の構造は破壊され、中型主体のリンパ腫様細胞が出現し、一部で大型異型細胞が見られた。免疫染色の結果よりT-cell lymphomaが疑われた。リンパ節生検時に作製した捺印細胞診標本では、馬蹄形核やドーナッツ状細胞など多形性に富んだ大型異型細胞が少数孤立散在性に観察された。いわゆるhallmark cellと考えられ、ALCLを疑う所見であった。これを受けて組織標本にて追加免疫染色を施行したところ、CD30、ALK、TIA/GranzymeBが広く陽性を呈した。フローサイトメトリー(以下FCM)および染色体G-band検査の結果と総合し、ALK陽性ALCLと診断された。まとめ:本症例では、組織標本ではALCLに特徴的な異型細胞が少数で観察しづらかったが、捺印細胞診では個々の細胞を詳細に観察することができ、少数であってもhallmark cellと考えられる異型細胞が確認できた。それによりALCLを疑い、FCMや免疫染色などの追加検索に繋がり、診断確定への一助となった。細胞診断を併用することの有用性が改めて認識できた一例であった。(著者抄録)

    乳児の予期しない急死の予防に資する目的で、2008年〜2015年に岡山大学大学院医歯薬学総合研究科法医学分野において解剖鑑定された予期せぬ乳児急死事例64例の実態を調査した。その結果、当該事例には第2子以上の男児が多く含まれていた。ほとんどの死亡は生後6ヵ月以内に発生し、多くの児は自宅において深夜の睡眠中に死亡していた。また、呼吸器感染症による急死例が少なからず含まれており、臨床診断がなされていない先天心奇形も認められた。予期せぬ乳児急死予防に向けては、ごく軽度の風邪様症状等に対しても、児への気配りが重要であると思われた。(著者抄録)

    Infective endocarditis (IE) of infants is rare, most of which occur associated with congenital heart disease or its cardiac surgery. We experienced a case of sudden death of a four-month-old male infant without congenital heart disease. It was elucidated by postmortem examination that the dead had suffered severe IE, which led him to death. In the microbiological genetic analysis using histological section, the pathogen causing inflammation in the present case was identified as Lactococcus lactis subspecies, although Staphylococci have been reported to be common and important one. Previously reported infectious diseases by Lactococcus lactis subspecies were all adult cases and this is the first report of an infantile death due to Lactococcal IE according to our knowledge. Any fatal disease may be included in sudden death cases targeted for forensic autopsy, even if it is rare. It is expected for forensic pathologists that they note such case and share each experience among themselves and other medical fields to develop a strategy for prevention.

    Unknown factors or diseases that were not recognized to cause sudden infant death were investigated. It was thought that infection of Cronobacter sakazaki as cause of sudden death could be simply diagnosed only by the growth of that pathogen in the culture. Fulminant type Ⅰ diabetes could also be diagnosed by the extreme low level of blood 1,5-AG concentration, whereas evaluation of stress as the factor of sudden death should be examined further using saliva chromogranin A concentration.
    According to the investigation of the ratios of area of the left ventricle to those of the right ventricle and the total left heart in the heart cross-section, it was suggested that the the left heart dysfunction could be another possible unknown factor/cause of infantile sudden death.
